The ultrastructural characteristics of eccrine sweat glands in a Fabry disease patient with hypohidrosis.

As an investigation of the pathogenetic mechanism of diminished sweating in Fabry disease, an electron microscopy ultrastructural study was conducted on specimens of eccrine sweat glands from a typical patient with Fabry disease who had hypohidrosis, a low skin moisture content, and diminished thermoregulation ability. Numerous characteristic cytoplasmic inclusions were observed in the eccrine sweat glands, the lamellar pattern of which was considerably variable in various types of gland cells. Large vacuolar inclusions predominated in clear cells of secretory coil; lesser vacuoles were also seen in the coiled duct, and the basal cells of the straight duct toward the coiled duct displayed mulberry-like figures. There were some clear cells showing cell damage and necrosis in the secretory coil. Lamellated inclusions were noted in the unmyelinated axons innervating the eccrine sweat glands. The small blood vessels around the eccrine glands were narrowed by swollen endothelial cells with heavy inclusions. These intracytoplasmic deposits may be responsible for the decreased sweating ability in Fabry disease. The factors related to hypohidrosis are also discussed.

Comparison of Helicobacter pylori infection between Fukuoka, Japan and Chinju, Korea.

BACKGROUND: Helicobacter pylori is the causative agent of type B chronic gastritis, and plays a major role in the pathogenesis of gastroduodenal ulcer and gastric cancer. Because gastric cancer has been the leading cause of cancer mortality in Japan and Korea, we conducted a seroepidemiological study to estimate the prevalence of H. pylori infection in Japan and Korea in order to explain the current change in the gastric cancer incidences between two countries. MATERIALS AND METHODS: Samples used for this study included 1204 sera from Chinju, Korea and 580 sera from Fukuoka, Japan. Immunoblotting, using a sonicated crude H. pylori antigen and 1:5 dilution of serum, was performed, considering the immunoblot shows reactivity to the 120 Kd antigen of H. pylori as a specific marker of H. pylori infection. RESULTS: Seroepidemiology data from Fukuoka, Japan showed a prevalence of H. pylori infection of 20% before school age, 40% by teenage years, and over 80% beyond 20 years of age. Seroepidemiology data from Chinju, Korea, showed a 50% infection rate in preschool ages, and over 80% prevalence rate after 7 years of age. CONCLUSIONS: Lower rates of childhood H. pylori infection in Fukuoka may explain the recent decline and shift in the incidence of stomach cancer in Japan, supporting the hypothesis that H. pylori is a major determinant in the pathogenesis of stomach cancer.

Alternative use of medial and posterior upper-arm flaps.

There are such marked variations in the vascular anatomy of the nutrient vessels in medial and posterior upper arm flaps, that widespread usage of these flaps has been delayed. The authors have evaluated both medial and posterior upper arm flaps and have concentrated on the status of the skin perforators. Both types of flaps were considered for use as free transfers in 25 cases; island flaps were successfully elevated in 22 patients (11 medial and 11 posterior flaps). Because of the lack of suitable perforating vessels, this solution was abandoned in three patients. Perforators in both areas were found in 44 percent of the cases, in one or the other area in 44 percent, and not at all in 12 percent of the cases. Using the determination of perforators, the success rate of free-flap transfers from the medial and posterior upper arm increased from 64 and 68 percent, respectively, to 88 percent. When harvesting a flap from the medial or posterior upper arm, the risks attending questions of vascular stability in free-flap transfer, may be overcome by clinician experience.

Amplification of int-2 and bcl-1 genes in squamous cell carcinoma of the larynx.

We studied int-2 and bcl-1 gene amplification in 21 operated patients with cancer of the larynx. In 9 cases, the int-2 gene was amplified (42.9%) and in 4 cases. the bcl-1 gene was amplified (25.0%). Co-amplification of int-2 and bcl-1 was observed in three cases (18.8%), all of which were glottic carcinomas. Other amplified cases were supraglottic carcinomas. Tumor specimens with int-2 amplification were associated with a significantly worse prognosis. We suggest that int-2 amplification is one of the prognostic factors in laryngectomized patients with cancer of the larynx.

Closure of large pharyngo-oesophageal fistulas with free flap transfer after resections for cancer.

Postoperative salivary fistulas still remain a serious and potentially lethal problem in head and neck reconstruction particularly if the fistula is large and involving one half or more of the circumference of the pharyngo-oesophagus. Pedicled flaps have traditionally been the flaps of choice for closure of these fistulas, but the results are often disappointing. During the period 1982 to 1995, we have used either a radial forearm free flap or a jejunal free flap to close large and complex pharyngo-oesophageal fistulas after resection for cancer in 15 patients. Although two patients developed major fistulas that required additional operations for closure, successful closure was achieved in all but one case: the success rate was therefore 14/15 (93%). We consider that jejunal flaps are suitable for circumferential pharyngo-oesophageal reconstruction and forearm flaps for non-circumferential defects.

Risk factors for low birthweight in Japanese infants.

The purpose of our study was to identify risk factors for low birthweight (LBW; birthweight < 2500 g) in Japanese infants. The data was collected from questionnaires completed by the parents of 23132 infants who underwent a standardized well baby check-up for 1-month-old infants, conducted by the Fukuoka City Medical Association from 1987 to 1995. The following eight factors and their second-order interaction terms were examined as potential risk factors for LBW: maternal age at delivery, history of live-born LBW infant, history of abortion in previous pregnancies, maternal smoking, coffee and alcohol consumption during pregnancy, prenatal training and live birth order. The results of multiple logistic regression analysis showed that the following three factors and one interaction term significantly contributed to LBW: history of live born LBW infant, maternal smoking, live birth order and the interaction between maternal smoking and live birth order. The smoker-related risk for LBW was quite different in each of the three groups stratified by live birth order. Efforts should be made, for example, to increase the accessibility of early, high-quality prenatal care for the high-risk groups with previous LBW babies and to implement smoking intervention, ranging from specific medical procedures to broad-scale public health and health-related educational programs in schools.

Influences of cardiopulmonary bypass and fentanyl anesthesia on hepatic circulation and oxygen metabolism in beagles.

UNLABELLED: Decreases in hepatic blood flow (HBF) have been reported in patients and in animal experiments during cardiopulmonary bypass (CPB). We examined changes in HBF and hepatic oxygen metabolism during CPB in 16 beagles anesthetized with fentanyl. Hepatic arterial blood flow (HABF) and portal venous blood flow (PVBF) were measured by using an electromagnetic flowmeter before and during normothermic and hypothermic CPB with 10 microg x kg(-1) x h(-1) (F-10 group; n = 8) or 50 microg x kg(-1) x h(-1) (F-50 group; n = 8) of fentanyl anesthesia. CPB was conducted with membrane oxygenation and a nonpulsatile pump flow of 2.4 L x m(-2) x min(-1). Hepatic oxygen delivery (HDO2) and consumption (HVO2) were calculated from HBF and oxygen content in arterial, portal venous, and hepatic venous blood. HABF did not change during normothermic CPB in the F-10 group, but it decreased significantly during hypothermic CPB in both groups, especially the F-50 group. During CPB, PVBF and total HBF decreased significantly in both groups-more so with the larger dose of fentanyl--whereas HDO2 decreased significantly because the arterial and portal venous blood oxygen levels decreased. The HVO2 was stable in the F-10 group but was significantly depressed during CPB in the F-50 group. Our results indicate that during hypothermic nonpulsatile CPB larger doses of fentanyl are associated with reduced HBF and impaired HDO2 and HVO2. IMPLICATIONS: Hepatic dysfunction after cardiopulmonary bypass (CPB) has been frequently reported and could be partly attributed to hepatic circulatory disturbance during CPB. We found that, in beagles, large doses of fentanyl were associated with greater decreases in hepatic blood flow and hepatic oxygen metabolism during hypothermic CPB than smaller doses of fentanyl.

Lateral supramalleolar flap for heel coverage in a patient with Werner's syndrome.

The repair of intractable ulcer over the Achilles' tendon in a patient with Werner's syndrome is described. The ulcer was covered successfully with a lateral supramalleolar flap combined with a delay procedure. Partial necrosis of the flap occurred; however, the necrotic area could be repaired with a skin graft because the underlying adipofascia of the flap survived. Histopathological studies revealed marked dermal fibrosis spreading to the subcutis, however the deep adipofascia was not so afflicted. The lateral supramalleolar flap, if combined with a delay procedure, can be used as a first choice in the repair of an ulcer in the heel region as long as apparent arteriosclerotic changes do not exist.

Enhanced poly(ADP-ribosyl)ation after focal ischemia in rat brain.

Nitric oxide from neuronal cells plays detrimental roles in glutamate neurotoxicity and in focal brain ischemia. Nitric oxide directly damages DNA, and breaks in the DNA strands activate poly(ADP-ribose) polymerase (PARP), which brings poly(ADP-ribosyl)ation of the nuclear proteins. The excessive activation of PARP is thought to cause depletion of ATP and the energy failure resulting in cell death. To clarify the involvement of poly(ADP-ribosyl)ation in ischemic insult, we examined poly(ADP ribosyl)ation by immunohistochemical methods and the protective effect of 3-aminobenzamide, which is a PARP inhibitor, on focal brain ischemia using an intraluminal permanent middle cerebral artery occlusion model in rats. Poly(ADP ribosyl)ation was widely and markedly detected 2 hours after the ischemic insult in the cerebral cortex and striatum in which infarction developed 24 hours later. The enhanced immunoreactivity of poly(ADP-ribose) gradually decreased, and 16 hours later, no immunoreactivity was detected. Intraventricular administration of 3-aminobenzamide (1 to 30 mg/kg) 30 minutes before the ischemic insult decreased infarction volume in a dose-dependent manner along with the immunohistochemical reduction of poly(ADP-ribosyl)ation. Pretreatment with 7-nitroindazole (25 mg/kg, intraperitoneally), a selective neuronal nitric oxide synthetase inhibitor, partially reduced poly(ADP-ribosyl)ation. These data suggest the involvement of poly(ADP-ribosyl)ation in the development of cerebral infarction.

Neurovascular free muscle transfer combined with cross-face nerve grafting for the treatment of facial paralysis in children.

Microneurovascular free muscle transfer is fast becoming a standardized procedure in the treatment of established or long-standing facial paralysis. However, there is no general agreement as to whether muscle transfer should be employed for children who still are growing. A patient's natural growth may influence or disturb reproduction of a smile, or a child may not be able to collaborate with postoperative rehabilitation sufficiently well to obtain a satisfactory result. One may hesitate to employ the procedure for children aged under 10 out of fear of its technical difficulties. To investigate the influence of age, the results experienced by 23 patients under the age of 15 who underwent free muscle transfer combined with cross-face nerve grafting were compared with those of adult patients. The treatment employed was a two-stage operation consisting of cross-face nerve grafting and subsequent free muscle transfer.

One-stage transfer of the latissimus dorsi muscle for reanimation of a paralyzed face: a new alternative.

The two-stage method combining neurovascular free-muscle transfer with cross-face nerve grafting is now a widely accepted procedure for dynamic smile reconstruction in cases with long established unilateral facial paralysis. Although the results are promising, the two operations, about 1 year apart, exert an economic burden on the patients and require a lengthy period before obtaining results. Sequelae such as hypoesthesia, paresthesia, and conspicuous scar on the donor leg for harvesting a sural nerve graft also cannot be disregarded. To overcome such drawbacks of the two-stage method, we report a refined technique utilizing one-stage microvascular free transfer of the latissimus dorsi muscle. Its thoracodorsal nerve is crossed through the upper lip and sutured to the contralateral intact facial nerve branches. Reinnervation of the transferred muscle is established at a mean of 7 months postoperatively, which is faster than that of the two-stage method. In our present series with 24 patients, 21 patients (more than 87 percent) believed that their results were excellent or satisfactory, which also compares well with the results of the two-stage method combining free-muscle transfer with cross-face nerve graft.

Infrequent mutations in the PTEN/MMAC1 gene among primary breast cancers.

Recently PTEN/MMAC1, a candidate tumor suppressor gene, was isolated from chromosome 10q23-24 and somatic mutations of this gene were detected in several malignancies including brain, prostate, and breast tumors. To investigate further the potential role of this gene in mammary carcinogenesis, we examined 69 primary breast cancers for mutations in PTEN/MMAC1 by means of polymerase chain reaction single-strand conformation polymorphism and sequencing analysis. We detected only one somatic missense mutation, a change from T to C at codon 59 (TCA to CCA) resulting in substitution of Pro for Ser in the predicted protein. This site is located outside of phosphatase or phosphate-acceptor motifs, but this codon encodes a residue that is conserved in homologous proteins, tensin and auxilin and is likely to be crucial for normal function of PTEN/MMAC1. Among the 69 tumors examined, three low-frequency polymorphisms were found as well, one in the non-coding region of exon 1 and one each in introns 2 and 7. Our results suggested that mutation of the PTEN/MMAC1 gene is not a major factor in the development of most primary breast cancers.

Inhibitory effects of a cyclosporin derivative, SDZ PSC 833, on transport of doxorubicin and vinblastine via human P-glycoprotein.

The inhibitory effects of SDZ PSC 833 (PSC833), a non-immunosuppressive cyclosporin derivative, on the P-glycoprotein (P-gp)-mediated transport of doxorubicin and vinblastine were compared with those of cyclosporin A (Cs-A). The transcellular transport of the anticancer drugs and PSC833 across a monolayer of LLC-GA5-COL150 cells, which overexpress human P-gp, was measured. Both PSC833 and Cs-A inhibited P-gp-mediated transport of doxorubicin and vinblastine in a concentration-dependent manner and increased the intracellular accumulation of doxorubicin and vinblastine in LLC-GA5-COL150 cells. The values of the 50%-inhibitory concentration (IC50) of PSC833 and Cs-A for doxorubicin transport were 0.29 and 3.66 microM, respectively, and those for vinblastine transport were 1.06 and 5.10 microM, respectively. The IC50 of PSC833 for doxorubicin transport was about 4-fold less than that for vinblastine transport, suggesting that the combination of PSC833 and doxorubicin might be effective. PSC833 itself was not transported by P-gp and had higher lipophilicity than Cs-A. These results indicated that the inhibitory effect of PSC833 on P-gp-mediated transport was 5- to 10-fold more potent than that of Cs-A, and this higher inhibitory effect of PSC833 may be related to the absence of PSC833 transport by P-gp and to the higher lipophilicity of PSC833.

Fluorescence in situ hybridization analysis of 12;21 translocation in Japanese childhood acute lymphoblastic leukemia.

Fluorescence in situ hybridization (FISH) analysis was applied to detect t(12;21) using two yeast artificial chromosome probes and cosmid probes covering the TEL(ETV6) and the AML1 gene to clarify the incidence of abnormality of t(12;21) in Japanese childhood acute lymphoblastic leukemia (ALL). We detected seven TEL/AML1 fusion positive patients (9.5%), all of whom were diagnosed as B-lineage ALL, among 74 childhood ALL. On the other hand, no TEL/AML1 fusion positive patients were found among 37 adult ALL. The incidence among Japanese seemed to be lower than that among other nations. Of the seven patients with the TEL/AML1 fusion, five exhibited normal karyotype, one was t(8;12)(q11;p13), i(21q) and the remaining one exhibited a near-triploid karyotype in conventional G-banding. The FISH method clearly demonstrated that all patients with the TEL/AML1 fusion had subpopulations of leukemic cells with deletion of the normal TEL allele, which is significant for understanding the progression of leukemia with t(12;21).

Testicular metastases from carcinoma of the bile duct: a case report.

A 41-year-old man was admitted to our hospital with chief complaints of painless tumor in the left scrotal contents and loss of body weight. As the tumor was suspected of being a neoplastic lesion, left radical inguinal orchiectomy was carried out. Histopathological examination showed metastatic adenocarcinoma of the bile duct. Metastatic carcinoma to the testicle, epididymis, and their tunics is unusual and testicular metastases from adenocarcinoma of the bile duct is extremely rare. A case of testicular metastases from carcinoma of the bile duct is reported and the literature reviewed.
