Triple carcinomas in Cronkhite-Canada syndrome.

The present report describes a 69-year-old man displaying the clinical features of the Cronkhite-Canada syndrome. After taking medicine for the common cold, he suffered hypogeusia and watery diarrhea, eruptions on the lower extremities and an 8 kg loss in body weight. All his finger and toenails began to fall out. He underwent an upper gastrointestinal examination, upon which multiple polyps of the stomach were detected. Three years later, he again developed diarrhea, bloody stools, body weight loss and eruptions on the lower extremities. An upper gastrointestinal series showed a diverticulum of the esophagus and multiple polyps in the stomach. A barium enema examination revealed polyps throughout the entire colon. Endoscopical biopsy specimens revealed juvenile type polyps and adenomas. The patient was treated with predonine therapy and, in a few days, his symptoms improved. Following the predonine therapy, an upper gastrointestinal endoscopy revealed superficial esophageal cancer and early gastric cancer. The patient received successful surgical treatment. Macroscopically, the esophageal cancer was of the superficial type, and its histologic type was that of moderately-differentiated squamous cell carcinoma. The gross finding on the stomach cancer was one of superficial depressed type, and its histologic type was that of well-differentiated tubular adenocarcinoma. One year later, lung cancer was detected. The gross appearance of the resected lung tumor was one of a grayish-white color and the neoplasm was histologically diagnosed as undifferentiated carcinoma, small and large cell type. The coexistence of carcinoma of the gastrointestinal tract with Cronkhite-Canada syndrome has been reported in 21 cases. We have found no report, however, of lung cancer associated with Cronkhite-Canada syndrome. The case described herein is, therefore, the first case of Cronkhite-Canada syndrome to be associated with esophageal, gastric and lung cancer.

[MRI of pheochromocytoma].

MRI features of 8 patients were reviewed and correlated with pathological findings. It was characteristic finding that the intensity of pheochromocytoma on T2-weighted image and proton image was very high. The detectability of pheochromocytoma by MRI and CT was almost equal and MRI was also useful for determining aberrant pheochromocytoma. MRI was more helpful for detection of necrotic lesion than CT. We considered that the double ring sign on MRI was characteristic for pheochromocytoma. In the cases which are difficult for diagnosis, other diagnostic studies such as endocrine and other imaging studies are necessary.

[Hemangioma of the hypopharynx; a case report and value of MRI].

A case of 23-year-old female with histologically verified hemangioma of the hypopharynx was reported. MRI revealed slight hyper-intense mass on T1-weighted image while it was very high-signal intensity on T2-weighted image. The dynamic MRI showed peak contrast at 80 seconds after administration of Gd-DTPA. In the present paper the value of MRI for diagnosing hemangioma of the hypopharynx is discussed.

[A case of pleural effusion secondary to breast carcinoma responding to treatment based upon thoracoscopic findings].

Patients with metastatic breast carcinoma often present symptomatic pleural effusions. A 67-years-old woman, who had undergone right radical mastectomy 7 years previously for right breast carcinoma, developed massive right pleural effusion. Thoracoscopy revealed local metastasis in the center of the parietal mediastinal pleura. On biopsy, it was found to be adenocarcinoma which was considered to have invaded the pleura directly from the mediastinum. She was treated with linac X-ray irradiation focused on the pleural metastasis and systemic chemotherapy (CMF), and resulted in a gradual decrease and final disappearance of the pleural effusion.

Synchronous early gastric cancer.

Synchronous early gastric cancer (EGC) has a prevalence of approximately 9%. The barium study findings of 73 patients with 163 synchronous EGC tumors were retrospectively reviewed, and the size, location, and morphologic type of each lesion considered malignant were analyzed and correlated with the endoscopic and final pathologic findings. Barium studies and endoscopy each correctly demonstrated 116 of the 163 synchronous EGC lesions. Those lesions not demonstrated by either technique were small, shallow, or flat and confined to the mucosa. Our results indicate that a careful preoperative evaluation with radiography and endoscopy currently can depict about 70% of these lesions.

[Method for simultaneous determination of six metabolites of toluene, xylene and ethylbenzene, and its application to exposure monitoring of workers in a printing factory with gravure machines].

For the biological monitoring of exposure to solvent composed of toluene, xylene, and ethylbenzene used in a printing factory with gravure machines, we developed a HPLC method for the simultaneous determination of urinary metabolites of this solvent, i.e. hippuric acid, o-, m-, and p-methylhippuric acid, mandelic acid and phenylglyoxylic acid. Except for phenylglyoxylic acid, urinary concentrations of the metabolites determined by the present method correlated well with the air concentrations of the respective solvent components. Hence the present method is useful in monitoring solvent exposure. In 91 workers of the printing factory and 53 control subjects, we also determined the concentrations of some phenolic metabolites and confirmed that o-cresol is a useful indicator for monitoring toluene exposure.

Determination of erythrocyte porphyrins by reversed-phase high-performance liquid chromatography using capsule-type silica gels coated with silicone polymer.

A simple, rapid and reliable method is described for determining erythrocyte porphyrins using a new type of reversed-phase silica whose surface is coated with silicone polymer. After a single-step extraction of blood with N,N-dimethylformamide, zinc protoporphyrin IX and protoporphyrin IX were separately quantified in a single run in 5 min. The column life was so long that no alteration in elution profile or retention time was apparent after 1500 injections of samples. The method described would be useful for the screening of lead exposure or iron-deficiency anaemia, and also for the diagnosis of erythropoietic protoporphyria.

Determination of pyrimidine 5'-nucleotidase (P5N) activity in whole blood as an index of lead exposure.

A simple method for determining pyrimidine 5'-nucleotidase (P5N) activity in whole blood has been developed, inhibiting the plasma activity for UMP-hydrolysis by concanavalin (Con A). Con A specifically inhibits the activity of plasma 5'-nucleotidase (5N) but does not affect erythrocyte P5N activity. The anticoagulant EDTA partially inhibits 5N activity but slightly activates P5N. P5N activity determined by the present method with heparinised blood and Con A was comparable with that by the method reported previously and correlated well with blood lead concentrations. The mean value and SD for P5N activity in normal subjects (n = 72) not exposed to lead are 16.2 and 2.5 mumole/h/g Hb, respectively. The present method can eliminate not only the isolation step of RBC but also Hb determination, the activity being expressed as mumole/h/l blood or RBC. Thus the procedures are so simplified that the assay may be used as a routine test for mass screening of lead exposure.

[Automated HPLC method for determining zinc protoporphyrin IX and protoporphyrin IX in erythrocytes of workers exposed to lead].

We developed an automated HPLC method for determining erythrocyte protoporphyrin. After a single extraction with N,N-dimethylformamide (DMF), zinc protoporphyrin IX (ZP) and protoporphyrin IX (PP) were separated by the column packed with a new type of reversed phase silica (capcell type) within 5 min. The column life was markedly extended to facilitate sample injection for about 1,000 times. The concentrations of ZP or TP (total protoporphyrin = 0.9 ZP + PP) determined by the present method correlated well with those determined by the conventional methods, i.e. by hematofluorometer (HF) or acid extraction method (FEP). In 150 workers exposed to lead, the ratio of 0.9 ZP/TP varied between 0.43 and 0.99 with the ratio being less than 0.6 in only 5 workers. The correlation coefficient between Pb-B and ZP was significantly higher than that between Pb-B and PP. Both anticoagulants, heparin and EDTA, could be used in the present method as well as in HF and FEP methods. Reference values for TP by HPLC were between 31.2 and 120.9 micrograms/dl RBC. Samples stored at 37 degrees C for 3 days were also used in the present method and the same results were obtained as in the samples stored at 4 degrees C.

Activation and cleavage of the carbon-cobalt bond of adeninylethylcobalamin by diol dehydrase.

Adeninylethylcobalamin (AdeEtCbl) underwent cleavage of the C-Co bond by interaction with apoprotein of diol dehydrase from Klebsiella pneumoniae ATCC 8724, although this analog was quite inactive as coenzyme. Spectroscopic observation indicates that AdeEtCbl was converted to the enzyme-bound hydroxocobalamin without intermediates. The conversion was stoichiometric (1:1) and obeyed the second-order reaction kinetics (k = 0.027 min-1 microM-1 at 37 degrees C) depending upon concentrations of apoprotein and AdeEtCbl. This suggests that the complex formation is the rate-determining step and that AdeEtCbl undergoes rapid C-Co bond cleavage once it binds to the apoenzyme. Substrates and oxygen did apparently not affect the rate of the C-Co bond cleavage. The experiments using [adenine-U-14C]AdeEtCbl and [1(3)-3H]glycerol demonstrated that 9-ethyladenine was the only product formed from the adeninylethyl group of AdeEtCbl during the conversion and that an additional hydrogen atom in the 9-ethyladenine is not derived from the substrate. 1H NMR measurement of the 9-ethyladenine formed enzymatically from AdeEtCbl and DL-1,2-[1,1,2-2H3]propanediol also led to the same conclusion. All of these results indicate that the C-Co bond of AdeEtCbl is activated by diol dehydrase and undergoes heterolysis forming Co(III) and a carbanion or a carbanion-like species, in clear contrast to the homolysis of the C-Co bond of adenosylcobalamin in the normal catalytic process. 9-Ethyladenine formed remained tightly associated with the enzyme. Longer chain homologs, i.e. adeninylpropylcobalamin, adeninylbutylcobalamin, and adeninylpentylcobalamin did not undergo such cleavage of the C-Co bond by diol dehydrase.

Identification of a dephosphorylated oxidation product of the molybdenum cofactor as 2-(1,2-dihydroxyethyl)thieno[3,2-g]pterin.

A new method was developed for the synthesis of 2-(1,2-dihydroxyethyl)thieno[3,2-g]pterin and related 2-substituted thienopterins. A dephosphorylated fluorescent oxidation product of the molybdenum cofactor isolated from xanthine oxidase (EC 1.2.3.2) was identified as 2-(1,2-dihydroxyethyl)thieno[3,2-g]pterin by comparison of electronic and fluorescence spectra and TLC behaviors with those of the synthetic compound.

Chronic manganese poisoning: a neuropathological study with determination of manganese distribution in the brain.

An autopsy case of a 52-year-old man suffering from chronic manganese poisoning (CMP) is reported with determination of the manganese distribution in the brain. The patient had been working in a manganese ore crushing plant since 1965. In 1967 he began to complain of difficulties in walking and diminished libido. Later, he developed various neuropsychiatric symptoms including euphoria, emotional incontinence, masked face, monotonous speech, "cock-walk", increased muscle tone, weakness of upper and lower extremities, tremor of the eye lids, and exaggeration of knee jerks. The major neuropathological change was degeneration of the basal ganglia, in which the pallidum was severely affected. The pallidum disclosed a loss and degeneration of nerve cells, which was especially marked in the medial segment, a prominent decrease of myelinated fibers, and moderate astrocytic proliferation. The substantia nigra was intact. Distribution of manganese in the brain of the present case of CMP was determined using flameless atomic absorption spectrometry and compared with control cases and also a case of Parkinson's disease (PD). There was no significant difference between the control cases and the case of PD in average concentration of manganese and its distribution in the brain. The present case of CMP showed no elevation in average concentration of manganese in the brain. However, there were some changes in its distribution. Thus, the continuance of neurological disorders in CMP is not linked to an elevated manganese concentration itself in the brain. CMP appears to be different from PD in neuropathology and manganese behavior in brain.

A case of advanced gastric cancer with long-term survival as the results of combined radiation therapy and chemotherapy.

A case report on a long-term surviving patient with advanced gastric cancer with supraclavicular lymph node metastasis treated by radiation and chemotherapy is presented. The Borrman type 2 of advanced gastric cancer was found on the greater curvature of the antrum at the first radiological examination. Radiation was administered to the supraclavicular lymph node at 60 Gy and to the stomach at 64 Gy. The patient received mitomycin C (24 mg) and Tegafur (230 g). After completion of the combined therapy the endoscopy revealed an irregular mucosal change with erosion and hemorrhage. Radiological examination revealed atrophic and hyperplastic areas throughout the stomach. These findings lasted more than six years. The patient died of unknown cause in February 1983. She had survived nine years and seven months after her initial diagnosis. Radiotherapy may play a role as a means of radical treatment in certain cases of advanced gastric cancer.
