[Effect of thrombolytic therapy with intravenous streptokinase on the size of myocardial infarction estimated by the ECG QRS score system].

The impact of short-term intravenous large dose streptokinase injection on the myocardial infarction size estimated by a QRS score system was studied in 57 patients with primary myocardial infarction admitted to hospital at least 6 hours following the onset of the disease. There was a lower myocardial infarction size despite its localization and the time of treatment initiation when thrombolysis was successful, as evidenced by non-invasive criteria. The reliability of the QRS score system used to estimate the myocardial infarction size increased when the patients with anterior or inferior myocardial infarction and those with successful and unsuccessful systemic streptokinase thrombolysis.

[Assessment of the diagnostic significance of R wave amplitude changes in patients with ischemic heart disease in computer-assisted monitoring analysis of ECG during treadmill test].

During treadmill exercise, 57 patients with documented coronary heart disease (CHD) were studied for the change patterns in R wave amplitudes in V5 lead by analyzing ECG on a computer. With exercise, 86% of CHD patients showed higher or no R wave amplitude changes, whereas 73.7% of healthy subjects displayed its lower changes. Increased or no R wave amplitude changes in CHD patients was accompanied by more marked quantitative ischemic parameters than their decrease. A significant correlation was found between higher R wave amplitude and extended exercise-induced myocardial ischemia. Decreased R wave amplitude was associated with local ischemic shifts. Comparison of the data obtained from the computer-assisted ECG analysis and radiocardiometric findings during the exercise test demonstrated that an increase m R wave amplitude in CHD patients was followed by higher cardiac volume, which indicated ischemic myocardial dysfunction.

[The action of streptodecase on the size of the focal lesion, the hemostatic system, blood rheology and microcirculation in relation to the time of the start of the treatment of patients with an acute myocardial infarct].

Fifty patients with myocardial infarction (MI) were given streptodecase intravenously (the main group). Depending on the time elapsed from the appearance of the disease symptoms and the onset of streptodecase injection, the main group was distributed into 2 subgroups (less than or equal to 4 h and 4 to 8 h). The size of myocardial injury was judged from the activity CPK and ECG mapping in 35 thoracic leads. The Size of MI in the main group was significantly less than in the control group. At the same time the 2 subgroups of the patients demonstrated no significant difference in myocardial injury. This is accounted for by the fact that the majority of the patients (75%) were given the thrombolytic between the 120th and 360th minute since the disease onset. In view of the fact that streptodecase was discovered to exert a lasting beneficial effect (10 to 14 days) on hemostasis, hemorheology and microcirculation, it is suggested that the drug be used at later times of MI to prevent and treat complications consequent on microcirculatory disorders.

[Relation between left-ventricular insufficiency and necrotic mass in myocardial infarction, determined by analysis of the QRS complex].

A noninvasive method, based on the analysis of QRS parameters from 10 of 12 standard ECG leads, was used to assess the size of necrosis in patients with acute myocardial infarction. A close correlation between anterior left-ventricular myocardial infarction weight and congestive left-ventricular insufficiency has been demonstrated. No such correlation was demonstrated for inferior myocardial infarction.

[Effect of 24-hour intravenous administration of nitroglycerin on the status of the focus of a lesion in patients with an acute myocardial infarct].

Nitroglycerin was drip-infused for 24 hours to 35 patients with acute myocardial infarction. Thirty-three patients were taken as controls. Intergroup comparisons were made of the patterns of precordial cartograms from 35 leads and serial measurements of plasma MB CPK activity. The treatment was shown to reduce significantly the activity of MB CPK and total ST elevation, the effect being particularly pronounced with the drug administered within the first 6 hours of the attack. It was also shown that 24-hour intravenous nitroglycerin infusion produced a significant decrease in the end diastolic pressure of the pulmonary artery that occurred by the 15th minute and persisted till the end of the administration.

[Computer analysis of the ECG during physical exercise].

Treadmill tests were performed in one hundred men below 60, with computerized assessment of their ECG data. Reduced R wave amplitude from lead V5 at maximum exercise was only noted in subjects showing no electrocardiographic evidence of ischemia. Increased R wave from lead V5 was regarded as cardiac dysfunction in response to exercise, which, in the presence of ischemic ECG changes, preceded the development of pathologic ST depression. A negative T wave from leads III and aVF on resting ECG did not correlate with abnormal exercise test results. Ventricular arrhythmias were more common in subjects with non-ischemic ECG changes during exercise.

[Effectiveness of lidocaine use in the initial period of acute myocardial infarct].

The efficiency of variant regimens of lidocaine management of acute myocardial infarction is compared. The need for continuous infusion of the drug throughout the first day of the attack at the rate of 1 mg per minute, and the reduction in the maximum heart rate as well as the incidence, rate and duration of the ventricular tachycardia runs are demonstrated.

[Pulmonary artery pressure and blood gas composition in the acute period of myocardial infarct].

The mean pressure in the pulmonary artery and the gaseous composition of arterial, capillary, venous and mixed venous blood were studied in 40 patients with acute myocardial infarction. It was found that increased pressure in the pulmonary artery is congruous with low oxygen and carbon dioxide tension in arterial and capillary blood. Oxygen tension in venous blood was low while carbon dioxide tension was increased. Since increased pressure in the pulmonary artery was predominantly found in patients with cardiac insufficiency, the authors suggest using the data obtained in the comprehensive appraisal of the condition of hemodynamics and the efficacy of treatment.

[Anti-angina effectiveness of sustak and obzidan in the acute period of myocardial infarct].

Sustak and obsidan were given separately and in combination to 99 patients with acute myocardial infarction and frequent attacks of angina pectoris. With the use of these agents the attacks of angina pectoris were very rare. The course of the disease and the prognosis were more favourable in the group of patients treated with anti-anginal agents than in the control group (100 patients).

[Prognosis of the immediate outcome of myocardial infarct according to data of acid-base and blood oxygen balance].

In 261 patients with acute myocardial infarction an immediate prognosis of the disease was made by using quantitative values of the acid-base and blood oxygen balance, mathematically processed with the help of the analytical method of notion-shaping. The possibility of prognosing in priniciple on the ground of all the factors studied, the blood oxygen tension and the hemoglobin saturation with oxygen being the most informative, was ascertained.
