[Effect of the neuroleptic fluphenazine dihydrochloride on the cardiovascular system of polytraumatized patients with an alcoholic anamnesis].

Haemodynamic effects in 10 multi-injured patients were registered after a bolus injection of 20 mg fluphenazin-dihydrochloride (Dapotum acutum; Fa. Heyden). Favourable results were seen in pre- and afterload. Parameters such as TTI, TTIB, CE and TI, characterising myocardial oxygen consumption, were also influenced positively. A reduction of mean arterial pressure can be avoided by administering slowly increasing volume and, occasionally, catecholamines. By use of modern monitoring in ICV, thus drug seems to be free from risk.

[Surgery for stomach cancer in patients over 70].

Among a total of 1038 operations for early and advanced gastric cancer, 257 patients were older than 70 years. The resection rate in this group was 64.2%, 95 subtotal and 70 total gastrectomies were performed. Postoperative complications were caused primarily by concomitant respiratory and cardiac diseases, whereas the overall operative mortality amounted to 16.9%. Depending on the stage of tumour development a 5-year survival rate of 17.7% could be obtained. This rate increased to 27.2% after subtotal or total gastrectomies. The choice of surgical procedures for gastric cancer does not show any principal differences between young and old persons; thus, the use of individual and liberal criteria for total gastrectomy can be justified even in the aged.

[Artificial respiration of multi-injured patients with He-O2 and N2-O2 mixtures. II. Hemodynamics].

Multi-injured patients (n = 18) requiring ventilatory support were alternatively ventilated with either He-O2 or N2-O2 mixtures (FiO2 = 0.3). Haemodynamic effects in favour of He-O2 were especially seen in cardiac output in arising PEEP. It was the result of a reduced intrathoracic pressure when administering He-O2 mixtures for controlled respiration. These results must be seen in combination with the advantages of the ventilatory effects.

[Technical innovation on a cava catheter].

A method is described for fixing a central-vein-catheter, during insertion when the envelope is withdrawn, so as to render the procedure more sterile.

[Experimental studies with a new Giessen model high frequency jet ventilator].

In some critically ill patients respiratory can be maintained by high frequency jet ventilation. Gas exchange and transport by this method cannot be explained exactly up to date. Therefore it is impossible to compare this method with classical IPPV. Statistics and nomograms must be prepared for common clinical use.

[Hemodynamic effects of mexiletine and lidocaine in coronary surgery patients].

In two groups of 10 coronary bypass patients during the postoperative phase a comparison was carried out of the haemodynamic effects of mexiletine and lidocaine in order to evaluate mexiletine's applicability as an antiarrhythmic in intensive care patients. With mexiletine cardiac effort (CE) fell by 6%, total peripheral resistance (TPR) fell by 11%, while CO increased by 8%. With lidocaine CE and TPR remained unchanged, CO dropped by 4%. Mexiletine seems to be applicable in intensive care without hazards to circulation.

[Respiratory depression after epidural opiate analgesia with buprenorphine hydrochloride? A clinical case report].

In a case of epidural application of buprenorphinehydrochloride a reduction of arterial pO2 was seen 4-6 hours after the injection. This cannot be explained as a respiratory depression due to buprenorphine, because immediately after a repeat epidural application of the drug the pO2 rose again. Reduced pO2 a long time after buprenorphine is a a result of hypoventilation caused by increased pain.

[Respiration of multi-injured patients with He-O2 and N2-O2 mixtures. I. Ventilatory effects and pulmonary gas exchange].

Polytraumatic patients (n = 19), requiring for ventilatory support [8, 23, 24], were alternatively normoventilated (PaCO2 approximately equal to 40 mm Hg) with either He-O2 or N2-O2 (FIO2 = 0,3) using positive endexspiratory pressures of 0, 5, 10 and 15 cm H2O. The results demonstrated that the tidal volume decreased by 17% during the ventilation with He-O2 as compared with N2-O2. At the same time the inspiratory resistance and the inspiratory peak-pressure decreased by 20%. These results can be explained by the physical properties of helium, since helium guarantees a laminar flow during respiration in contrast to N2 even in obstructive airways. In conclusion it can be said that polytrauma patients have a inhomogeneous distribution of ventilation starting at the day of accident, that leads to a rise in deadspace ventilation. Because of having lower respiration pressures by using He-O2 mixtures, pulmonary barotrauma could be reduced.

[Comparative haemodynamic studies with mexiletine and lidocaine in patients with coronary heart disease after aorto-coronary bypass (author's transl)].

In two patient groups undergoing coronary surgery (n = 10) hemodynamic side effects of mexiletine and lidocaine were investigated. Changes of clinical importance were seen in cardiac-output, stroke volume, total peripheral resistance, triple- and cardiac index. There are significant differences 10, 15, and 30 min after application (p less than 0.05; p less than 0.025) in cardiac-index.

[Alloplastic tracheal prosthesis with biocarbone. A study of animal experiments (author's transl)].

Animal experiments have shown bicarbone to be a suitable material for alloplastic tracheal replacement. So far survival rates have been limited by connective tissue stenoses and by loosening prostheses. Although the transplants are not lined by bronchial mucosa, the movement of secretions is not seriously impaired due to the smooth surface. It is still a matter of speculation as to whether modifying the suture technique with a more elastic transistion from the trachea to the prosthesis would produce better results.

[Advantages of a new thoracic drainage unit with double sealing].

The paper deals with the application of a new 'Four-Compartment Drainage System' with double-sealing in 20 patients having lung parenchyma resections, explorative thoracotomies, and pneumothorax. This drainage system has an advantage over conventional intercostal drainage in as much as it prevents tension pneumothorax in leakage of the lung and suction failure. The closed system avoids bacterial contamination of the pleural space.
