Serum endotoxin, tumor necrosis factor, and interleukin-6 response to total hip arthroplasty.

The purpose of this study was to determine if endotoxin, tumor necrosis factor (TNF), and interleukin 6 (IL-6) increased in patients during or after total hip arthroplasty. Endotoxin is absorbed from the gut lumen and stimulates macrophages to make TNF. TNF is one of the stimuli for the increased production of IL-6. All three entities cause fever. If an increase takes place in any of the three, it could contribute to the commonly observed postop fever in total hip patients. Five serum samples were obtained from 12 patients: preoperatively; after the acetabular component was seated intraoperatively; in the recovery room; day 1 postoperatively; and day 3 postoperatively. IL-6 and TNF were measured using monoclonal antibody tests and endotoxin was measured with a chromogenic limulus amoebocyte lysate assay. A statistically significant increase in all three parameters was demonstrated. Endotoxin peaked intraoperatively or in the recovery room, TNF tended to peak in the recovery room, and the peak in IL-6 was more variable. Ten patients had a concomitant postoperative fever.

New extraction procedure and high-performance liquid chromatographic method for analyzing polyethylene glycol-400 in urine.

We describe a new, highly efficient method for extracting polyethylene glycol-400 from urine and for its analysis by isocratic reversed-phase high-performance liquid chromatography. This method is an improvement over previously published methods in that it does not require the use of ion-exchange resins and lyophilization prior to extraction, nor does it require the separation and analysis of the individual polymers of polyethylene glycol. The procedure described in this report entails extraction with a salt-solvent combination of ammonium sulfate and dichloromethane and analysis by reversed-phase high-performance liquid chromatography. The lower limit of detection was approximately 0.25 g/l with a 2-ml urine sample. Analytical recoveries of polyethylene glycol-400 added to urine at 2.5 and 5.0 g/l averaged 97 and 96%, respectively (n = 10). Within- and between-day coefficients of variation were less than 5% at 2.5 and 5.0 g/l. Studies of various urine samples from patients receiving polyethylene glycol-400 revealed no interferences from other urine substances.

Blastocystis hominis infection: signs and symptoms in patients at Wilford Hall Medical Center.

Blastocystis hominis (B. hominis) is a protozoan that may inhabit the human gastrointestinal tract. In our study we reviewed the signs and symptoms of patients at Wilford Hall with stool specimens positive for B. hominis. These patients fell into four groups, HIV-positive adults, foreign nationals, children, and adults not known to be HIV positive. B. hominis caused an acute self-limited diarrheal illness, or chronic gastroenteritis with nausea, abdominal pain, and mild diarrhea. Metronidazole effectively relieved the symptoms and cleared the organism in some but not all patients.

Carbon monoxide: its role in photochemical smog formation.

The photochemistry of trace amounts of isobutene and oxides of nitrogen in an atmosphere of air was studied both in the presence and in the absence of small amounts of carbon monoxide. Carbon monoxide accelerates the reaction as measured by nitric oxide oxidation or ozone formation. This finding has relevance to photochemical smog formation.
