Heart block following propofol: a case report.

We report two cases of ventricular arrest with persisting atrial activity in association with propofol anaesthesia. In both cases, anticholinergic agents corrected the arrhythmia. It is recommended that anticholinergic drugs be given routinely when propofol is used in association with vagal stimulants.

The hazardous cuffed endotracheal tube--aspiration and extubation.

A basic defect in the usage of the cuffed endotracheal tube is demonstrated. during anaesthesia a column of fluid can be supported around the tube above the inflated cuff. This fluid may represent secretions, blood or gastric contents which can be aspirated during extubation. A method of investigation was devised and possible measures to prevent postoperative chest complications are suggested.

Coracoid block--a safe and easy technique.

A method of blocking the brachial plexus using an infraclavicular approach is described. Compared with the supraclavicular approach, pulmonary complications do not occur and compared with the axillary approach a higher level of analgesia can be obtained and a potentially septic area is not traversed. However, the level of anaesthesia is at a lower level than that obtained from the supraclavicular approach.
