Effect of extracellular fluid volume contraction and expansion on plasma atrial natriuretic peptide (ANP) concentration in patients with acromegaly.

Atrial natriuretic peptide (ANP) is a hormone release into the circulation by atrial cardiocytes (Gutkowska et al. 1984). Extracellular fluid volume expansion acts as a powerful stimulus for ANP secretion and results in the augmentation of its plasma concentration (Lang et al. 1985). Patients with active acromegaly demonstrate the increased extracellular fluid volume (Falkheden et al. 1964), while a successful treatment of the disease results in the disappearance of hypervolemia (Strauch et al. 1977). We have recently demonstrated that in patients with active acromegaly the increased total body plasma volumes are accompanied by the elevated plasma ANP concentrations, whereas, in the successfully treated patients, both: total plasma volumes and plasma ANP levels do not differ significantly from these in healthy subjects (Czekalski et al. 1988b).

[Effect of furosemide on the plasma level of atrial natriuretic peptide (ANP) in healthy persons and in patients with uncomplicated primary arterial hypertension].

The study included 15 healthy individuals aged 37.3 +/- 7.7 years and 27 patients with the primary uncomplicated blood hypertension (stages I and II according to WHO classification) of the comparable age, untreated and given a diet containing 100-120 nM Na+ daily. Plasma ANP concentrations were measured prior to and after 30, 60, and 90 minutes following 40 mg furosemide intravenously. An increase in 1-minute urine output and 1-minute Na+ excretion in the urine were determined during 90 minutes following furosemide administration. A significant decrease in ANP plasma levels was noted in all examined individuals following furosemide administration in all time intervals comparing with baseline values. An increase in 1-minute urine output and 1-minute sodium excretion with the urine significantly correlated with plasma ANP decrease during 90 minutes following furosemide administration. The obtained results suggest that furosemide inhibits ANP secretion in the patients with uncomplicated primary hypertension similarly to healthy individuals.

[Churg-Strauss syndrome: two cases histories].

Two cases of Churg-Strauss syndrome with severe polyneuropathy have been diagnosed according to clinical and histopathologic criteria. Perforation of small intestine, successfully treated with surgery, was observed in one of two cases. The resected part of gut presented specific histologic picture of Churg-Strauss syndrome.

[Comparison of the effect of short-term treatment with nifedipine and verapamil on blood pressure, plasma atrial natriuretic peptide (ANP) and cyclic guanosine monophosphate (cGMP) in patients with primary arterial hypertension I degree according to WHO].

The aim of the study was the comparison of the effect of seven day treatment with nifedipine (2 x 10 mg daily) and subsequently with verapamil (3 x 40 mg daily) on blood pressure, heart rate, plasma ANP, cGMP, renin activity (PRA), aldosterone (ALDO) concentrations in patients with primary hypertension. The material consisted of 12 untreated patients with primary arterial hypertension Io WHO. These results suggest that short-term treatment with nifedipine and subsequently with verapamil in patients with primary hypertension Io WHO not influence on plasma ANP, cGMP, PRA and ALDO in spite of blood pressure reduction and the changes in heart rate. It seems that ANP did not participate in hypotensive action of nifedipine and verapamil. No augmentation of urinary sodium excretion was found after short-term treatment with nifedipine or verapamil.
