Oral brivudin vs. intravenous acyclovir in the treatment of herpes zoster in immunocompromised patients: a randomized double-blind trial.

The efficacy of oral brivudin vs. intravenous acyclovir was compared in a randomized multicentered study under double-blind conditions using the double-dummy technique. Forty-eight patients with a herpes zoster rash less than 72 hours in duration were entered in the study. Brivudin was given as one 125-mg tablet every 6 hours. Acyclovir was infused over 1 hour at a dose of 10 mg/kg every 8 hours. Treatment was continued for 5 days. There was no significant difference between the treatment groups when analyzed in terms of new lesion formation, increase in the area of rash within the primary dermatome, cutaneous dissemination, and affection of mucous membranes or visceral organs. Both treatment regimes were also equally effective in the time to full crusting of lesions. Oral brivudin and intravenous acyclovir were well tolerated by most patients. There was no need to interrupt the treatment in any case. As effective as intravenous acyclovir in the treatment of herpes zoster, oral brivudin offers the potential for outpatient treatment of herpes zoster in immunocompromised patients.

[5-(2-bromovinyl)-2'-deoxyuridine therapy of herpes zoster diseases in patients with malignant primary diseases].

BVDU [(E)-5-(2-bromovinyl)-2'-deoxyuridine] has proved effective in the treatment of varicella-zoster virus diseases in patients with malignancies. In 13 out of 20 patients a prompt cessation of new vesicle formation accompanied by rapid resolution of fever, crusting and complete epithelialisation of cutaneous lesions were noted. Only in few cases a prolonged recovery from the severe zoster occurred. When starting the treatment later than 48 hours after the onset of initial rash the dissemination of epithelial lesions could not be prevented. BVDU was well tolerated. Laboratory abnormalities were observed only in some cases and did not result in interruption of treatment.

[Diagnostic value of the differential lymphatic blood picture in leukemic non-Hodgkin's lymphomas].

In 84 leukemic non-Hodgkin-lymphomas the diagnostic evidence of peripheral blood smears in Pappenheim-staining and diverse cytochemical reactions (PAS, alpha-naphthyl acetate esterase, acid phosphatase, acid esterase, beta-glucuronidase) was analysed using a homogeneous cell grid (lymphatic differential blood picture). In all entities the small lymphocyte proved to be the most frequent cell-form. The other lymphomas of low malignity (intermediate malignity) can clearly be demarcated from the CLL by the more intense polymorphism of the blood smears and in the majority can be classified entity-related by the differential blood picture. For the diagnostic evidence of the differential blood picture proved significant that entity-typical cell-forms (e.g. lymphoplasmacytoid lymphocytes, centrocytes) following the small (mature) lymphocyte as cell-type of second frequency are flooded out into the peripheral blood in non-Hodgkin-lymphomas. Highly malignant lymphomas could clearly be demarcated from low-malignant ones by their higher proportion of blasts in the differential blood picture. Of the cytochemical reactions only the acid phosphatase, with definitely focal-perinuclear reaction, an importance as T-cell marker is ascribed.

[Detection of Epstein-Barr virus nucleic acid sequences in non-Hodgkin's lymphoma].

Tumor tissue specimens of 25 patients with various entities of Non-Hodgkin's lymphomas were examined for the presence of Epstein-Barr virus (EBV) nucleic acids and EBV nuclear antigens (EBNA) by in situ hybridization and anticomplement immunofluorescence. In tumor cells of five patients EBV nucleic acid and EBNA were demonstrated. The histopathologic examinations revealed in these cases one Burkitt's lymphoma, two centroblastic lymphomas and each one lymphoplasmocytic and lymphocytic lymphoma.

Demonstration of Epstein-Barr virus in malignant non-Hodgkin's lymphomas.

Lymph nodes or tumor biopsies of 60 persons suspected of having a malignant lymphoma were examined for the presence of Epstein-Barr virus nuclear antigen (EBNA) by anticomplement immunofluorescence. In 8 cases the tissue specimens were also assayed for Epstein-Barr virus (EBV) DNA by nucleic acid hybridization. Serum samples of patients and controls were tested for EBV-related antibodies. The histological tests in 37 cases showed a malignant non-Hodgkin lymphoma, and in 23 cases a reactive lymphadenopathy. A Burkitt lymphoma of a European boy and a polymorphic centroblastoma contained EBNA and approximately 27 or 30 genome equivalents EBV DNA per cell, respectively. EBNA was also demonstrated in about 20% of the cells of a lymph node from a patient with recurrent reactive lymphadenopathy.

[Comparative study of the efficacy of the DBVCy protocol (daunoblastin, bleomycin, vincristine, cytostasan) and the ABVD protocol in advanced Hodgkin's lymphoma. A prospective inter-clinic study within the framework of the Hauptforschungsrichtung Geschwulsterkrankungen].

In MOPP-resistant patients with Hodgkin-lymphoma stage III B and IV the well known ABVD-protocol was compared with the DBVCy-protocol (daunorubicin, bleomycin, vincristine, cytostasan) in a randomized prospective trial. 73 patients were evaluable, 35 received ABVD, 38 DBVCy. 4 out of 35 (11%) in the ABVD-group and 9 out of 38(24%) in the DBVCy-group reached complete remission. The median duration of the remission for DBVCy was 4,5 months, for ABVD 3,4 months (no significance). Median survivals (61 months in DBVCy and 37 months in ABVD) did not show significant differences; but the DBVCy-scheme seems to be better tolerabel. With the given evaluation can not be proved a difference between the two groups, but it is not likely, that one of the regimen is very much better or very much worse as the other.

[Immunodiagnosis of chronic lymphatic leukemia and leukemic non-Hodgkin's lymphoma with the monoclonal antibodies BL-Ig-L/1, BL-DR/1 and BL-T2].

3 self made monoclonal antibodies are tested with the indirect immunofluorescence method for the immunologic diagnostic of the circulating lymphocytes from 24 chronic lymphatic leukaemia (CLL) and 14 leukaemic Non-Hodgkin lymphoma (NHL) patients. In comparison, investigations were done with various specific rabbit antibody F(ab)2 fragments and antisera. The monoclonal antibody BL-Ig-L/1, which is directed to the human Ig L-chains, marked the neoplastic lymphocytes from 12 of 24 CLL and from 9 of 14 leukaemic NHL as membrane Ig+. The monoclonal antibody BL-DR/1, which reacts with a HLA-DR, binds to the blood lymphocytes of 23 CLL and 11 NHL patients. By this BL-DR/1 is superior to BL-Ig-L/1 for the immunologic diagnostic of the non-T-cell neoplasia. The with normal peripheral T-cells reactive monoclonal antibody BL-T2 reacts with malignant B lymphocytes on an equal scale as BL-DR/1. It is not qualified for the differentiation of malignant blood lymphocytes.

[Results of polychemotherapy (a VMCP/VCAP program) in multiple myeloma].

62 patients with multiple myeloma were primarily treated with the combination vincristin + melphalan + cyclophosphamide + prednisolone (VMCP). In therapy failures and relapses, respectively, vincristin + cyclophosphamide + adriamycin + prednisolone (VCAP) were administered. Under primary treatment with VMCP in 28 patients (45%) remissions were obtained, in 22 patients (36%) incomplete remissions and 12 patients (19%) were estimated as therapy failures. The secondary application of VCAP increased the total number of the remissions to 32 (52%). Incomplete remissions and therapy failures more frequently appeared in the clinical stage III and in undifferentiated myelomas. The median survival time of the total group was 46 months. With more than 60 months patients with remission showed significantly larger median survival times than those ones with incomplete remission (14.5 months) and therapy failures (6 months), respectively.

[Prognostic relevancy of histomorphological bone marrow findings in chronic lymphocytic leukemia (CLL)].

Punctures of the pelvic crest made in 45 patients with CLL (25 men, 20 women, mean age 58.7 years) were classified according to the type of lymphatic bone-marrow infiltrations which can be identified histomorphologically. Four different types were distinguished: 1. interstitial type (lymphatic infiltration of bone-marrow tracts, with lipocytes being present) - 11 cases 2. nodular type (nodular infiltration, free bone-marrow tracts) - 5 cases 3. mixed type (combination of interstitial and nodular type) - 11 cases 4. diffuse type (original haematopoiesis and fat cells have been displaced, type A: purely diffuse type; type B: diffuse + nodular) - 18 cases The statistical analysis of survival curves reveals significant differences in the probability for survival to be met with in types with morphologically limited bone-marrow infiltration in the sense of a proceeding deterioration of prognosis in the following order: interstitial, nodular, mixed, and diffuse type. In addition, a statistically significant correlation existing between the non-diffuse infiltration types and clinical early stages (RAI 0 to II) on the one hand, and the diffuse type and clinical late stages (RAI III and IV) on the other hand, could be identified. In the course of CLL the different types of lymphatic bone-marrow infiltration apparently reflect different degrees of lymphocyte accumulation or proliferation respectively. Because of their significance the histomorphological bone-marrow findings should be included in the therapy planning for CLL.

[Blast crisis in chronic myeloid leukemia].

Although the worst prognosis among acute forms of leukemia is persistently attributed to the blast crisis of chronic myeloid leukemia, a therapeutic nihilism seems to us to be unreasonable. Even in the developing stage of a blast crisis we consider an aggressive chemotherapy to be justified particularly in younger patients. In this connection, value and necessity of supporting of therapy are especially emphasized. Remissions can be achieved in about one third of patients during the blast crisis of CML. As in other forms of tumour therapy, the initial status, age, and secondary diseases have to be taken into account and a careful, individual therapy planning has to be made. For the future there is the hope that the increasing possibilities of differentiating blasts will allow more systematic therapeutic conclusions to be made. According to the present knowledge the greatest hopes for achieving long-term remissions or even healings can, in our opinion, be expected by bone-marrow transplantation.

Antibodies against herpesviruses in patients with Hodgkin's disease.

The antibody titres to Epstein-Barr virus (EBV), cytomegalovirus (CMV), herpes simplex virus (HSV) and varicella-zoster virus (VZV) were determined in 57 adult patients with Hodgkin's disease and a group of age- and sex-matched controls. In the group of patients, the geometric mean antibody titres to EBV capsid antigen, CMV early and late antigens, and also VZV, were significantly higher than in the healthy controls. Antibodies to early antigens of EBV and CMV were found more commonly in patients than in controls. The raised antibody titres to herpesviruses were taken to be the result of the disturbed cellular immunity in Hodgkin's disease.

[Long-term study in congenital refractory anemia and secondary siderosis].

It is reported on the course of the disease in a female patient with congenital therapy-refractory anaemia, in whom a transfusion siderosis of high degree with functional disturbances of the myocardium and the liver, with endocrine disturbances (primary hypothyroidism and normogonadotropic hypogonadism) and with small growth became clinically manifest. A reduction of the iron overload by a continuous desferrioxamine therapy in form of long-term subcutaneous infusions over two years could be proved. Possibilities and problems of treatment and conclusions relevant to practice are described.

[Contribution to the true non-African Burkitt's lymphoma (author's transl)].

It is reported on a 6-year-old Middle European boy suffering from a nasopharynx tumor with involvement of a submandibular lymph node. The tumor was classified by morphologic, serologic and virologic data as a true or Epstein-Barr virus-associated Burkitt's lymphoma, resp., which has been, up to now, seldom seen outside Africa. The patient died 3 months after diagnosis. Autopsy has shown tumor involvement of nasopharynx and soft tissues in the left neck region, several lymph nodes, kidneys, liver, spleen, and bone marrow. Cytologic, light and electron microscopic data are reported and the structural change of the tumor is referred to.
