Antileprosy protective vaccination of rhesus monkeys with BCG or BCG plus heat-killed Mycobacterium leprae: immunologic observations.

Groups of rhesus monkeys were vaccinated and boosted with Mycobacterium bovis bacillus Calmette Guerin (BCG) or BCG plus low-dose (LD) or high-dose (HD) heat-killed M. leprae (HKML), or were unvaccinated. Prior to and following vaccination-boosting and subsequent M. leprae (ML) challenge, these and unvaccinated, unchallenged control monkeys were observed longitudinally for approximately 3 years. Vaccination with BCG plus HKML initially stimulated significant in vitro blood mononuclear cell blastogenic responses to lepromin, which returned to baseline post-boosting and post-live-ML-challenge, minimally reappearing significantly 2 years post-ML-challenge. Vaccination with BCG failed to stimulated positive blastogenic responses to lepromin before ML-challenge but small, marginally positive, intermittent responses were seen post-ML-challenge. Compared to the unvaccinated ML-challenged group, significant increases in the numbers of blood CD4+ and CD8+ T-cell subsets and an increased CD4+:CD8+ ratio were observed in both BCG plus HKML-vaccinated, ML-challenged groups, but not in the BCG-only-vaccinated, ML-challenged group. CD4+CD29+ and CD4+CD45RA+ subset numbers increased significantly over time in only the BCG plus LD HKML-vaccinated, ML-challenged group. Compared to unvaccinated, ML-challenged groups, vaccination with BCG or BCG plus HKML followed by ML-challenge produced lower IgM:IgG antiphenolic glycolipid-I (PGL-I) serum antibody ratios and protected rhesus monkeys from clinical leprosy, consistent with prior observations that low IgM:IgG anti-PGL-I responses correlated with resistance to and protection from leprosy.

Successful molecular dynamics simulation of the zinc-bound farnesyltransferase using the cationic dummy atom approach.

Farnesyltransferase (FT) inhibitors can suppress tumor cell proliferation without substantially interfering with normal cell growth, thus holding promise for cancer treatment. A structure-based approach to the design of improved FT inhibitors relies on knowledge of the conformational flexibility of the zinc-containing active site of FT. Although several X-ray structures of FT have been reported, detailed information regarding the active site conformational flexibility of the enzyme is still not available. Molecular dynamics (MD) simulations of FT can offer the requisite information, but have not been applied due to a lack of effective methods for simulating the four-ligand coordination of zinc in proteins. Here, we report in detail the problems that occurred in the conventional MD simulations of the zinc-bound FT and a solution to these problems by employing a simple method that uses cationic dummy atoms to impose orientational requirement for zinc ligands. A successful 1.0 ns (1.0 fs time step) MD simulation of zinc-bound FT suggests that nine conserved residues (Asn127alpha, Gln162alpha, Asn165alpha, Gln195alpha, His248beta, Lys294beta, Leu295beta, Lys353beta, and Ser357beta) in the active site of mammalian FT are relatively mobile. Some of these residues might be involved in the ligand-induced active site conformational rearrangement upon binding and deserve attention in screening and design of improved FT inhibitors for cancer chemotherapy.

[In vitro dissolution study on the hydroxyapatite biocoatings].

This paper has studied the dissolution behavior of the plasma sprayed and electrochemically prepared hydroxyapatite(HA) biocoatings in the simulated body fluid. XRD and SEM show that the plasma sprayed coating and post-heat treated HA coatings are more soluble, and CaO and alpha-TCP in the coatings gradually disappear and CaCO3 forms during the immersion. The composition of the electrocrystallized and alkaline-treated HA coatings is basically unchanged, and the coatings are more stable in the simulated body fluid.

[Changes of maternal and umbilical serum nitric oxide in patients with the intrauterine growth retardation].

OBJECTIVE: To determine whether the levels of maternal and umbilical serum nitric oxide (NO) were changed in cases of intrauterine growth retardation (IUGR). METHODS: Fifteen pregnant women with fetuses of IUGR (IUGR group) and 25 normal pregnant women in late trimester (normal control group) were studied. Maternal and fetal umbilical venous blood were collected from all the cases. Serum NO2-/NO3-, the end products of NO, were measured with the Griess reaction after reduction with nitrate reductase. RESULTS: Mean NO2-/NO3- values in maternal serum were (74.22 +/- 28.99) mumol/L in IUGR group and (56.71 +/- 22.81) mumol/L in the control. Umbilical serum NO2-/NO3- values were (39.73 +/- 24.65) mumol/L in IUGR group and (21.49 +/- 6.69) mumol/L in the control. Compared with the control, maternal as well as umbilical serum NO2-/NO3- in IUGR group were significantly higher (P < 0.05 and P < 0.001, respectively). The total nitrite levels in fetal circulation were lower than in maternal circulation in both groups (P < 0.05). CONCLUSIONS: Changes of NO in the pregnant women and their fetuses were due to the result of secondary complementary response in the patients with IUGR. The metabolic disorder of nitric oxide in maternal and fetal circulation may play an important role in pathogenesis of IUGR.

[Study on women with abnormal uterine bleeding treated by hysteroscopic electric resection].

OBJECTIVE: To evaluate the efficacy, prognosis and influences of hysteroscopic electrosurgery in treating abnormal uterine bleeding (AUB). METHODS: Eighty two women with AUB were treated by hysteroscopic electric resection. Among them, 48 cases had submucous myomas, 29 cases endometrial polyps, 4 cases dysfunctional uterine bleeding, and 1 case had submucous myoma and endometrial polyps. RESULTS: The mean time of follow-up was 25.5 months. The effective rate of menstrual flow ameliorated was in 91.5% of the patients. Yet, the percentage of the effective rate was decreased gradually with time. The improved effective rates of dysmenorrhea, midcycle pain, premenstrual symptoms and perimenstrual symptoms were 83.3%, 60.0%, 60.0% and 62.7% respectively. The effective rate of total symptoms improved in endometrial thickness with < or = 5 mm was 91.3%, and that in endometrial thickness with > 5 mm was 63.6%. There is a significant difference between in endometrial thickness with < 5 mm and in endometrial thickness with < 5 mm (P < 0.05). The effective rate of total symptoms improved with drugs suppressing endometrium used in perioperation was 93.8%, and that with no drug used was 72.0%. There is also a significant difference between drug used and no drug used groups (P < 0.05). CONCLUSIONS: Hysteroscopic electric resection in treating AUB improves not only menstrual flow but also perimenstrual symptoms. The effectiveness and prognosis of this operation may be related to endometrial thickness and medical treatment.

Antileprosy protective vaccination of sooty mangabey monkeys with BCG or BCG plus heat-killed Mycobacterium leprae: immunologic observations.

Groups of sooty mangabey monkeys (SMM) were vaccinated and boosted with Mycobacterium bovis bacillus Calmette-Guerin (BCG), or BCG + low-dose (LD) or high-dose (HD) heat-killed M. leprae (HKML), or were unvaccinated. Prior to and following vaccination-boosting and subsequent M. leprae (ML) challenge, these and unvaccinated, unchallenged control monkeys were immunologically observed longitudinally for approximately 3 years. SMM [multibacillary (MB) leprosy-prone as a species] were not protected clinically by BCG or BCG + HKML, although the disease progress was slowed by vaccination with BCG alone. The longitudinal immune response profiles to BCG or BCG + HKML in SMM showed that: 1) vaccination with BCG or BCG + HKML initially stimulated significant in vitro blood mononuclear cell blastogenic responses to ML antigens, which returned to baseline post-boosting and post-live ML challenge; 2) BCG + LD HKML-vaccinated groups gave the largest blastongenic response (SI = 23) followed by the BCG + HD HKML group (SI = 14.5) and by the BCG-only vaccinated group (SI = 3.6); 3) significantly diminished numbers of blood CD4+ (helper) and CD4+CD29+ (helper-inducer) T-cell subsets were observed longitudinally in all ML-challenged groups compared to controls regardless of whether they had been vaccinated or not; 4) CD8+ (suppressor) T-cell numbers remained longitudinally constant, on average, in all ML-challenged groups (vaccinated or not) compared to controls; 5) there was a significant decrease in the CD4+:CD8+ ratio over time in all ML-challenged groups (vaccinated or not); 6) vaccination with BCG or BCG + LD or HD HKML resulted in significantly increased numbers of CD4+CD45RA+ (suppressor-inducer) T cells longitudinally compared to the unvaccinated, ML-challenged control group; and 7) over time, vaccination with BCG + HKML followed by live ML-challenge produced higher IGM:IgG antiphenolic glycolipid-I (PGL-I) serum antibody response ratios than BCG-only vaccinated, ML-challenged monkeys or unvaccinated, ML-challenged SMM, consistent with prior observations that IgG anti-PGL-I responses correlate with resistance to and protection from clinical leprosy and IgM anti-PGL-I responses correlate with increased susceptibility.

[Relationship between factor v Leiden mutation and Chinese Budd-Chiari syndrome and its clinical significance].

OBJECTIVE: To investigate the relationship between factor v Leiden (FvL) mutation and Chinese sporadic Budd-Chiari syndrome (BCS), familial BCS, and to explore the significance of FvL mutation in the etiology of BCS. METHODS: Twenty-five patients with sporadic BCS, 6 patients with familial BCS (from A and B families), 39 both A and B family members, and 31 healthy persons were detected for FvL mutation with PCR. Meantime, two family members were explored for the related etiology of BCS. RESULTS: Factor V Leiden mutation was detected in 4 of 6 patients with familial BCS and in 2 family members. AIII(7,11,15) and BII(10), AII(2) and BIII(5) were found FvL mutation, and mutation was heterzygous. FvL mutation in the two degrees was compatible with Mendel hereditery law. The frequency of FvL mutation in 31 BCS and 31 healthy persons showed no statistical significance: but the frequency of FvL mutation between the familial BCS and healthy persons showed statistical significance. Ten persons in A family had varicose vein of the low extremeties, which was compatible with autosomal dominant inheritance. CONCLUSION: FvL mutation is related to Chinese familial BCS, but is not related to Chinese sporadic BCS. FvL mutation may be a underlying pathogenicity of familial BCS. Varicose vein of the low extremeties may be one of the pathogenicity of familial BCS.

[Middle-aged and elderly patients with hydronephrosis induced by ureteric obstruction: etiology and diagnosis].

OBJECTIVE: To improve the etiologic diagnosis of middle-aged and elderly patients with ureteric obstruction. METHODS: The etiology and diagnosis were retrospectively studied in 141 middle-aged and elderly patients with hydronephrosis in recent 10 years. RESULT: 59 patients had ureterolithiasis, 51 ureteric cancer, 11 ureteric tuberculosis, 9 UPJ stricture, 6 uretero-cyst and 5 ureteric polyp. CONCLUSIONS: Ureterolithiasis and ureteric cancer contribute greatly to ureteric obstruction in middle-aged and elderly patients. Etiologically, B ultrasonography is suggested to be used as the first choice in diagnosis and follow-up of the middle-aged and elderly patients with hydronephrosis, especially those accompanied with hematuria.

[A report of two Chinese familial Budd-Chiari syndrome].

OBJECTIVE: To investigate the etiology of two Chinese familial Budd-Chiari syndrome (BCS). METHOD: Four patients with familial BCS (from A and B families), and the other 41 family members were detected by angiography, ultrasound Dopler, etiology analysis and Factor V Leiden (FvL) mutation analysis. RESULT: Four BCS patients were proved by angiography, 2 by ultra sound Dopler in family A. Ten members in family A were varicosis in low extremeties. FvL mutation was detected in 4 of 6 patients and 2 normal family members. A II(2), A III(7, 11, 15,) B II(10) and B III(5) had FvL mutation. The FvL mutations were compatible with Mendel hereditary law. CONCLUSION: FvL mutation may be one of main risk factors and varicosis in low extremeties may be another risk factors for familial BCS.

[Effects of Nilestriol on reproductive system in ovariectomized rats].

The purpose of this study was to determine the effects of Nilestriol on reproductive system. Female SD rats, aged 4 months, were randomly divided into three groups. The OVX group's rats were subjected to bilateral ovariectomy, the SHAM group's rats received sham surgery; the Nilestriol(CEE3) group's rats were subjected to bilateral ovariectomy and treated with CEE3(0.15 mg/100 g BW, once a week). A computerized image analyzer was used to evaluate the endometrial pathological changes 10 weeks later. The weight of uterus was measured. The results showed that in OVX group, atrophic uteri; and atrophied endometria were noted. The weight of uterus was increased in CEE3 group, compared with that in OVX group (642.20 +/- 283.60 mg vs. 297.00 +/- 106.90 mg, P < 0.05). The volume fraction of endometrial glands was also significantly elevated in CEE3 group, compared with that in OVX group (1.84% vs 0.58%). The endometria in CEE3 group were at an early proliferative stage. These suggest that Nilestriol has a slight stimulating effect on the uteri of ovariectomized rats, including their endometria and uterine smooth muscles. Nilestriol is suited to be used in combination with progestogen.

[Influence of the administrators of family planning on the spread of norplant in countryside of Sichuan].

The purpose of the study was to understand the family planning administrators' knowledge of Norplant and their fluence on the spread of Norplant in countryside of Sichuan. 156 administratoras were surveyed. It was found that most of the administrators worked hard on education, propagation, organization and management of using Norplant, and they recognized the contraceptive efficacy of Norplant. However, the educational level of the administrators was generally low. They were short of the training on Norplant related knowledge(training rate was only 36.5%). The degree of the users' satisfaction and the administrators' knowledge on Norplant varied between areas. The result indicated that the basic quality of the administrators and their knowledge of Norplant were influences on the spread of Norplant in the countryside of Sichuan. Soit is very important to improve the basic quality and knowledge of the administrators of family planning.

[Evaluation of modified maze procedure by Doppler tissue imager].

Since 1998 the cardiac surgeons of WCUMS have performed modified maze procedure for treating the patients with rheumatic mitral valve disease accompanied with chronic atrial fibrillation (Af). In this study, echocardiography, especially Doppler Tissue Imager (DTI), was used to evaluate atria contraction and function of such patients before and after the operation. 14 rheumatic mitral disease patients, aged 27-60 years, were chosen; among them, 11 had mitral stenosis, 3 mitral regurgitation. The control group consisted of 16 cases, aged 24-67 years, with normal echocardiogram. Before and half-1 year after the operation, left atrium, left ventricle, right atrium and ventricle were measured, and cardiac function and posterior wall motion of left atrium were measured. Velocity of mitral valve annual and tricuspid valve annual was measured in DTI. The results showed: There was no age difference between the two groups. The motion of posterior wall of left atrium was 4.18 +/- 0.37 mm in the operation group, 4.09 +/- 0.29 mm in the control group (P > 0.05). No difference in velocity of mitral/tricuspid valve annual between the two groups was observed; 3 peaks of mitral valve annual were S 0.11 +/- 0.02, e 0.11 +/- 0.01, a 0.09 +/- 0.01 (m/s) in operation group, and S 0.08 +/- 0.00, e 0.10 +/- 0.01, a 0.08 +/- 0.01 (m/s) in control group, (P > 0.05, respectively); 3 peaks of tricuspid valve annual were S 0.10 +/- 0.01 vs 0.11 +/- 0.08, e 0.11 +/- 0.08 vs 0.10 +/- 0.01, a 0.10 +/- 0.10 vs 0.09 +/- 0.01 (m/s), (P > 0.05, respectively). The diameter of left atrium after operation in cases of mitral stenosis decreased significantly, compared with that before operation (P < 0.01). The diameter of right atrium after operation decreased significantly, compared with that before operation (P < 0.01). There was no significant change in right ventricle/left ventricle/EF and FS. The diameter of left atrium after operation in cases of mitral regurgitation decreased significantly, compared with that before operation (P < 0.01). The diameter of left ventricle after operation decreased significantly, compared with that before operation (P < 0.05). No significant change in right ventricle/right atrium/EF and FS was noted. The authors conclude the DTI, as a new diagnostic method in echocardiography, has high sensitivity and is less influenced when used to observe motion of atrium-ventricle annual influenced. It can avoid the error caused by deviation of blood displacement. In the rheumatic mitral valve disease patients who had mitral valve replacement and modified maze procedure, atrial contraction respores, the risk of thromboembolism decreases and prognosis improves. So the operation is worth doing.

Polymorphism of angiotensin-converting enzyme gene and genetic susceptibility to asthma with familial aggregation.

OBJECTIVE: Angiotensin-converting enzyme (ACE) plays a key role in the metabolism of angiotensin II (AT II) and inactivation of bradykinins and tachykinins, which are potent bronchialconstrictors and mediators of inflammation asthma, and ACE is heavily expressed in the lungs. An insertion-deletion (D/I) polymorphism of ACE gene has been shown to be associated with levels of ACE. We investigate whether the polymorphism of ACE gene is associated with asthma and bronchial responsiveness. METHODS: A case-control study was carried out in 50 asthmatics, 7 families with at least 2 asthmatic individuals, and 50 healthy subjects. The insertion/deletion (I/D) polymorphism of ACE gene was amplified by polymerase chain reaction (PCR). Methacholine brocho-provocation and pulmonary function tests were performed in all asthmatics. RESULTS. There was an higher gene frequency of DD genotype of ACE gene in asthmatic subjects and families individuals compared with healthy subjects (46%, 53% vs 16%, P<0.05; odd ratio 4.98). An- higher prevalence of DD genotype of ACE was in patients with bronchial hyperresposiveness (BHR) (67% vs 33%, P<0.05; odd ratio 3.8). Accordingly, the mean values of FEV1% and FEV1/FVC were higher in asthmatics carrying non-DD alleles than patients with DD genotype (73.78% vs 56.56%, P<0.05, 79.19% vs 69.29%, P<0.05, respectively). CONCLUSION: These results suggested that DD allele of ACE genotype was significantly involved in genetic susceptibility to asthma. DD genotype of ACE might be a risk factor for the degree of airway obstruction, it could also be implicated in pathogenesis of bronchial hyperresponsiveness.

[The summarization of clinical experience of difficult tracheal intubation].

OBJECTIVE: To summarize our experiences and lessons of difficult tracheal intubation for clinical anesthesia reference. METHODS: We had done a retrospective analysis of clinical data on difficult tracheal intubation in 2,825 patients undergoing elective plastic surgery with anesthesia. The main causes of difficult tracheal intubation were the limitations of neck extension (n = 1,169), mouth opening (n = 889), both neck extension and mouth opening (n = 698), and micromaxillary deformity (n = 69). By the Cormack's classification, all the patients had the laryngeal exposure of grade II or more. The tracheal intubations were done under neuroleptanalgesia combined with topical spray of local anesthetic in 439 patients, intravenous anesthesia of sedative drugs and nondepolarizing relaxants of subnormal doses in 629 subjects, and total intravenous or inhaled anesthesia in 1,757 cases, respectively. RESULTS: The difficult tracheal intubations were completed using blind nasal intubations in 142 patients, blind oral intubations with direct laryngoscope in 2,377 patients, oral intubations with fiberoptic stylet rigid laryngoscope in 186 patients, and oral or nasal intubations with flexible fiberoptic bronchoscope in 72 patients. The incidence of successful intubation was 99.7%. The common complication of intubation was airway trauma and its incidence was 19.3% in all the patients. Anesthetic techniques could affect significantly the intubation time and the incidences of complications in the patients with difficult intubation. CONCLUSIONS: By the improvement of anesthetic methods and common intubation techniques, the intubation time and the incidence of complications in the patients with difficult intubations were reduced.

[High temperature Raman spectroscopic study of the structure of sodium disilicate crystal, glass and its melt].

The structure of Na2Si2O5 from room temperature up to 1,773 K are studied by high temperature Raman spectroscopy using copper vapor pulse laser and integral time-resolved detection technique without any black-body radiation effect on spectral record. Back-scattering optical configuration is coupled with confocal collection of Raman signal of macro-sample in the high temperature shaft tube furnace. Results show that temperature-dependent Raman spectra can clearly indicate phase transition during melting. Relative densities of various kinds of SiO4(n-4) (n, bridging-oxygen number binding to one tetrahedron former Si) tetrahedrons can be qualitatively and quantitatively resolved by Gaussian spectral deconvolution. Obviously high temperature Raman spectroscopy provides an useful tool for the micro-structure research of materials under high temperature.

[A theoretical study on vibrational spectra of S7 rings].

The geometries of S7 rings with the chair-type and boat-type conformations have been optimized using SCF-HF and hybrid density functional methods. (B3LYP) with the split valence 3-21 + G(d) and 6-31 + G(d) basis sets. Vibrational spectra of S7 rings have been predicted by using the same two methods: B3LYP/6-31 + G(d) and HF/3-21 + G(d) over the geometries optimized above, respectively. The symmetries of normal vibrational modes have been assigned. We also make comparisons between the present predictions and the previous experimental data.

[Staining of complete denture: a preliminary clinical study]

OBJECTIVE:To study the relation between staining of complete denture and drinking tea, coffee and smoking, and to evaluate the result of denture cleaning agent on removing stain of complete denture.METHODS:A survey of 176 patients with complete dentures were carried out, which included the history of drinking tea, coffee and smoking, the use of denture-cleaning agent and the times of daily use. Statistical analysis was performed to determine the relationship between staining of denture and the above factors. RESULTS:The results showed (1)The longer the complete denture was weared, the more serious the staining. (2)Drinking tea and smoking were the main causes for denture staining. (3)Daily use of cleaning agent can effictively remove stain of denture. CONCLUSION:Change of life habit(drinking less tea and stop of smoking) and daily use of cleaning agent can reduce denture staining.
