[Roentgenological and pathological diagnoses of primary osseous alveolar sarcoma--report of 3 cases].

Three cases of primary osseous alveolar sarcoma are reported. The tumor was of two different types as shown on X-ray films. Osteolytic type: the lesions occurred in the flat and short tubular bones with areas of patchy osteolytic destruction. Mixed type: the lesions, located in the long tubular bones, appeared as irregular compact shadow in the bone marrow without periosteal reaction or soft-tissue swelling besides areas of patchy osteolytic destruction. The pathological diagnosis was made according to the following: (1) tumor cells aggregating to form solid nest-like or alveolar structures; (2) antidiastase PAS positive granules in the tumor cell cytoplasm; (3) specific crystals in the tumor cell cytoplasm.

Pattern of anti-asthmatic drug utilization in Hong Kong compared to other parts of the world.

Inhaled drugs are now considered first line agents for the treatment of asthma, whereas oral beta-agonists, theophylline and its derivatives have been relegated to a secondary role. To determine whether current prescribing in Hong Kong conforms to these recommendations, we have evaluated anti-asthmatic drug utilization in the territory as reflected by 1984-86 drug sales (courtesy Medical and Health Department, Hong Kong). Sales/prescription statistics were also obtained from corresponding agencies of other governments and from published literature. If appropriate, data for comparison was expressed as defined daily doses (DDDs) per 1000 inhabitants per day. In contrast to the pattern of utilization in several developed nations, in Hong Kong (a) oral anti-asthmatic agents (particularly beta agonists) were more commonly utilized than inhaled drugs, and (b) inhaled therapy mainly consisted of non-selective beta-agonists, steroids being very rare. Further education is required to improve anti-asthmatic drug utilization patterns in the community.

Cephalosporin and aminoglycoside utilization in different parts of the world.

Cephalosporin and aminoglycoside utilization in Hong Kong during 1984, 1985 and 1986, were evaluated from wholesale data and compared with corresponding Swedish statistics and with UK data from a survey of non-hospital prescriptions. Details regarding each drug and category were collated, adjusted for population and if appropriate expressed as defined daily doses (DDDs)/1000 inhabitants/day. With respect to cephalosporins: (a) overall sales (especially those of the newer and more expensive parenteral drugs) were increasing; (b) parenteral sales were much larger in Hong Kong hospitals than in the community (up to about 106 versus 16 kg/million inhabitants/year respectively), but in Sweden they were comparable (up to about 38 and 41 kg/million inhabitants/year respectively); (c) non-hospital oral utilization appeared greater in the UK than in Sweden and Hong Kong (up to about 0.7, 0.4 and 0.4 DDDs/1000 inhabitants/day respectively); (d) oral sales to hospitals were greater in Hong Kong than Sweden (up to 0.3 compared to 0.1 DDDs/1000 inhabitants/day respectively) and (e) oral paediatric formulations (liquids) were most popular in Hong Kong, a higher proportion of children in Hong Kong being one possible reason. Non-hospital sales of gentamicin and kanamycin in Hong Kong were much greater than in Sweden (up to about 0.20 and 0.06 vs. 0.01 and 0.00 DDDs/1000 inhabitants/day respectively), whereas UK utilization appeared almost non-existent. Topical neomycin sales in Hong Kong were much more popular than in Sweden (up to about 60 vs. 9 g/million inhabitants/day). These regional differences in antibiotic utilization may be related to respective health care systems (and thus the affordability and availability of drugs) and prescribing preferences (cultural and/or promoted by drug companies), quite apart from possible differences in drug efficacy, drug tolerance and the prevalence and severity of various infections.

[Autoradiographic study on etorphine and phencyclidine specific binding sites in rabbit mesenteric artery].

Autoradiography was used to study the localization of kappa and PCP/sigma receptors in the blood vessels. Slices of rabbit mesenteric arteries were incubated with 0.4 nmol/L [3H]etorphine or/and 10 mumol/L etorphine for 45 min at 25 degrees C, or incubated with 5.2 nmol/L [3H]PCP or/and 20 mumol/L PCP for 60 min at 4 degrees C. Then, slices were covered with emulsion coated coverslip and kept for 8-10 wk at 4 degrees C. The results were as follows: Microscopy Autoradiographic analysis indicated that etorphine and PCP specific binding sites were both located in the outer-layer and the smooth muscle of the artery. However, most of these binding sites were lost in the 6-OHDA pretreated arteries in which the adrenergic nerve endings were destroyed. Microspectrophotometry The absorbance (A) of [3H]etorphine autoradiographic density for total binding (TB) and nonspecific binding (NSB) in control group were 0.416 +/- 0.056 and 0.044 +/- 0.011, respectively (P less than 0.01), and for TB after incubation with 6-OHDA was 0.068 +/- 0.013 which was different from the A value of TB in the control (P less than 0.01). The A value of [3H]PCP autoradiographic density for TB and NSB in the control were 0.546 +/- 0.087 and 0.023 +/- 0.060, respectively (P less than 0.01), and for TB after incubation with 6-OHDA was 0.065 +/- 0.015 which was significantly less than that of TB in the control group (P less than 0.01).(ABSTRACT TRUNCATED AT 250 WORDS)

Worldwide variation in chloramphenicol utilization: should it cause concern?

Per capita sales of chloramphenicol in Hong Kong (which presumably reflect adult and pediatric consumption in the community) are between about 11 to 442 fold greater than in several western countries and Australia. Despite such relatively excessive exposure to a potentially marrow-damaging drug, the certified death rate from aplastic anemia in Hong Kong was only 0.4 per 1000 deaths compared with 1.0 per 1000 in England and Wales. Nor was there any other evidence to indicate that Hong Kong residents suffered an excessive incidence of aplastic anemia. Wherever chloramphenicol use is widespread, prospective investigations should be undertaken in the local population to evaluate the alleged high risks of producing aplastic anemia.

A clinical trial of homoharringtonine in the treatment of advanced breast cancer.

Two combination chemotherapy protocols, CMF (cyclophosphamide, methotrexate, fluorouracil) and CMFH (CMF plus homoharringtonine), were tested in advanced breast cancer patients. The response rate of CMF was 50% in 44 cases, and that of CMFH was 54.16% in 48 cases; the difference was not significant. There was also no difference between the median duration of response and the median survival of the two protocols. It was concluded that the addition of homoharringtonine to the CMF protocol gave no additional benefit.

Questionnaire survey of pruritus and rash in grain elevator workers.

51.4% of 1,954 grain elevator workers surveyed in 1978 and 1979 complained of pruritus following exposure to grain dusts. 95% of 796 grain elevator workers surveyed in 1985 noted that pruritus following exposure to grain dust was of short duration. Exposure to barley dust and oat dust provoked the greatest number of complaints. Those with a past history of infantile eczema were significantly more likely to complain. Younger workers were more likely to complain than older workers. 13.1% of those surveyed in 1985 complained of a rash following exposure, in 35% of whom it lasted more than 1 day. Nasal congestion, ocular and oropharyngeal irritation were also frequent complaints.

Effects of grain dust exposure and smoking on respiratory symptoms and lung function.

In four groups of individually-matched subjects (nonsmoking grain workers, smoking grain workers, nonsmoking community controls, and smoking community controls) we measured pulmonary function variables from the spirogram, from the maximal expiratory flow-volume curve breathing air and helium, and from the single breath nitrogen test as well as symptom prevalences from a questionnaire in order to assess the relative effects of smoking and occupational exposure to grain dust in Saskatchewan country grain elevators. There were similar increased prevalences of respiratory symptoms and reductions in pulmonary function associated with either grain dust exposure or smoking, but the effects of smoking were slightly more pronounced. The combined effects of grain dust and smoking on lung function appeared to be additive except in the least exposed workers (five years or less) where a synergistic effect was observed in tests of peripheral airways dysfunction.

Effects of smoking and occupational exposure on peripheral airway function in young cereal grain workers.

We studied two groups of young male cereal grain elevator workers: 82 lifetime nonsmoking grain workers (2.5 +/- 1.2 yr of dust exposure) who were 22.7 +/- 3.5 yr of age, and 82 smoking grain workers (6.1 +/- 4.1 pack-years of smoking) individually matched to the nonsmoking grain workers for age (22.7 +/- 3.3 yr) and years of work in the grain industry (2.6 +/- 1.3 yr). We also studied the following groups of community control subjects: 82 nonsmoking community control subjects individually matched to the nonsmoking grain workers for age (22.8 +/- 3.7 yr), and 82 smoking community control subjects individually matched to the smoking grain workers for age (22.7 +/0 3.6 yr) and smoking history (6.0 +/- 4.0 pack-years). We measured forced expiratory volume in one second (FEV1), forced vital capacity (FVC), FEV1/FVC %, the maximal mid-expiratory flow rate (FEF25-75), the flow at 50% (Vmax50) and 25% (Vmax25) of a maximal expiratory flow-volume maneuver, the percent increase in the maximal expiratory flow at 50% FVC breathing a mixture of 80% helium-20% oxygen as compared with air (delta Vmax50), and the slope of Phase III of the single breath nitrogen test (delta N2/L). We also assessed the prevalence of respiratory symptoms from responses to a standardized questionnaire. Smoking was associated with an increased prevalence of cough, wheezing, and dyspnea, and a mildly decreased FEV1/FVC %. Two variables were used as indexes of small airway dysfunction, a lower delta Vmax50 and a higher delta N2/L. Only the group of workers who both smoked and were occupationally exposed to grain dust had evidence of small airway dysfunction. The differences in delta N2/L and delta Vmax50 were greater than was expected from an additive model, suggesting that the combined effects of occupational exposure to grain dust and inhalation of cigarette smoke may have a synergistic effect leading to small airway dysfunction in young cereal grain workers.

Between-technician variation in the measurement of spirometry with air and helium.

A study using a two-period cross-over design was conducted to determine the between-technician variation in the measurement of variables derived from spirograms with subjects breathing room air and also breathing a mixture of 80% helium and 20% oxygen (He-O2). Twenty-four subjects were allocated systematically into three 2-period cross-over studies (blocks) of 8 subjects each (4 in each sequence in each block). There were 3 technicians in the study, and the subjects in each block were tested by a pair of technicians. The following variables derived from the tracings were analyzed statistically: forced expiratory volume in one second (FEV1), forced vital capacity (FVC) while breathing room air and also while breathing He-O2, maximal mid-expiratory flow rate (MMRF), FEV1/FVC %, and the per cent decrease in time between 60% VC and 40% VC for the spirograms comparing He-O2 with air (delta t). Analyses were performed to assess the presence of residual effects in the cross-over design, the between-technician difference in the administration of the tests, and in the interpretation of tracings. The results indicated that: (1) significant differences were observed between technicians in the administration of the tests to obtain FVC while the subjects breathed room air and while they breathed He-O2, (2) there were significant differences between the 3 technicians in the interpretation of MMFR, and (3) the between-technician variation in the administration of the tests with respect to delta t and with He-O2 in general were not significantly different from that of the other spirometric variables studied.

Chronic bronchitis and decreased forced expiratory flow rates in lifetime nonsmoking grain workers.

To determine whether clinical symptoms and pulmonary function abnormalities were more common in lifetime nonsmoking grain elevator workers exposed to grain dust than in lifetime nonsmoking community control subjects who were not occupationally exposed to dust, we obtained an occupational health questionnaire, spirometric variables, and flow-volume curves on 90 lifetime nonsmoking grain workers and 90 lifetime nonsmoking community control subjects. For these groups we calculated the prevalence of chronic bronchitis, defined as daily production of phlegm for 3 months/yr for at least 2 yr. The prevalence of chronic bronchitis was higher in nonsmoking workers (23.1%) than in nonsmoking control subjects (3.3%) (p less than 0.01). Nonsmoking grain workers had lower mean values than did nonsmoking control subjects for maximal mid-expiratory flow rate (p less than 0.01) and for maximal expiratory flow at 50% of vital capacity (p less than 0.01). We conclude that exposure to grain dust inlifetime nonsmoking grain workers is associated with an increased prevalence of chronic bronchitis and evidence of airflow obstruction.

The central effect of electro-acupuncture analgesia on visceral pain of rats: a study using the [3H] 2-deoxyglucose method.

This study had the objective to understand the central effect of electro-acupuncture analgesia (EAA) on visceral pain of rats. We used the method of Sokoloff's 2-deoxyglucose (2-DG) auto-radiographic quantitative analysis to observe the changes of local cerebral metabolic rate of glucose (LCMRG) in rats given electrical stimulation of greater splanchnic nerve (GSN) followed by electro-acupuncture. From the results of this study, we found that the LCMRG had a significant difference between EAA group and pain group at some structures, such as the spinal thoracic and lumbar dorsal horns (segments T6-T8, L1-L3), locus coeruleus (lc), nucleus raphe magnus (rm), nucleus reticular gigantocellularis (rgi), periaqueductal gray (PAG) and habenulae lateralis (hl) of thalanum. The results combining reports by other investigators, suggest that these local cerebral structures might be the key nuclei in EAA on visceral pain.

Acupuncture Analgesia↗

C-fos expression in spinal cord and brainstem following noxious stimulation and electroacupuncture plus noxious stimulation.

The present study was to trace the neuronal pathway in the spinal cord and brainstem following noxious stimulation and acupuncture plus noxious stimulation by using immunohistochemical technique to detect the expression of c-fos like protein. The results showed that in both the noxious stimulation group and acupuncture of Zusanli(St.36) and Lanwei(Extra 33) points plus noxious stimulation group a lot of Fos-like immunoreactive (FLI) cell nuclei appeared in the following structures: dorsal horn of lumbar and sacral spinal cord (Laminae I, II, V and X), n. reticularis lateralis, n. reticularis paragigantocellularis lateralis, area A1, n. tractus solitarii, n. raphe magnus, locus coeruleus, n. raphe dorsalis and the periaqueductal grey. In the control group no obvious c-fos expression was shown in the structures mentioned above. It was also shown that in the lumbar and sacral spinal cord the number of FLI nuclei in the acupuncture plus noxious stimulation group was more than that in the noxious stimulation group, but in locus coeruleus and the ventral part of the periaqueductal grey the number of FLI nuclei in the acupuncture plus noxious stimulation group was less than that of the noxious stimulation group. The results imply that acupuncture and noxious stimulation may activate the intrinsic pain modulating system through the spinal dorsal horn, then suppress the c-fos expression induced by noxious stimulation in the locus coeruleus and ventral part of the periaqueductal grey.
