[VAD regimen for multiple myeloma--the effectiveness as first line therapy].

VAD regimen, combining vincristine, doxorubicin, and dexamethasone was given to 6 previously untreated and 4 treated patients with multiple myeloma. The response rate was obtained in 67% of previously untreated patients and in 50% of previously treated patients. In responders, a rapid improvement of clinical symptoms accompanied with a significant decrease in M-protein was observed. Patients with response to VAD therapy were received melphalan and prednisolone as maintenance. Response duration ranged between 4 months and 38 months. The infectious episodes were occurred in 36.8% patients but none of these was serious. The present results suggest that VAD regimen is useful as primary treatment for multiple myeloma when rapid control of disease is necessary.

[Clinical experience with chemotherapy using sulbactam/cefoperazone for severe infections accompanying malignant hematological disorders].

Clinical and bacteriological efficacies of sulbactam/cefoperazone (SBT/CPZ) were studied in 44 patients with serious infections associated with hematological malignancy. 1. SBT/CPZ was clinically effective in 33 cases (76.7%). Excellent effects were obtained in 23 cases, good effects in 10 cases and fairly good effects in 7 cases. Clinical effectiveness of SBT/CPZ was not dependent on neutrophil number in peripheral blood. 2. Bacteriologically SBT/CPZ was effective against all of the isolated organisms from 21 cases. 3. Adverse reactions were not significant except one case with eruption, 2 cases with abnormalities in hepatic function tests and 3 cases with abnormalities in renal function tests.

Radioimmunoassay for erythrocyte acidic GSH S-transferase.

We have developed a RIA for erythrocyte acid glutathione S-transferase (GST), which is immunologically identical to major GSTs from other blood cell components, and measured its serum concentrations in various hematological disorders. In some patients with paroxysmal nocturnal hemoglobinuria, chronic myelomonocytic leukemia, chronic myelocytic leukemia, polycythemia vera and myelofibrosis, the concentrations were high. Very high levels were found in 2 of 3 patients with acute lymphocytic leukemia, while acute myelocytic leukemia exhibited a modest increment. No or little increase was seen in aplastic anemia and myelodysplastic syndrome except chronic myelomonocytic leukemia. It is suggested that the measurement of serum acidic GST may be of use as a clinical marker of increased destruction and/or overproduction of blood cells.

[Clinical experience of chemotherapy with cefmetazole for severe infections accompanying malignant hematological disorders].

We made an attempt to treat with cefmetazole (CMZ) 25 patients who developed severe infectious diseases while suffering with granulocytopenia associated with the treatment of malignant hematological disorders. 1. Determination of bacteriological efficacy While 20 strains were isolated and identified from 15 patients, no significant bacteria were detected in 9 patients. Isolates obtained were: 5 strains of Enterococcus faecalis, 3 strains of Haemophilus influenzae, 2 strains of Staphylococcus epidermidis, 2 strains of Klebsiella oxytoca, 2 strains of Staphylococcus aureus, and 1 strain each of Neisseria sp., Pseudomonas maltophilia, Enterobacter sp., alpha-Streptococcus, beta-Streptococcus and Gram-positive cocci. Causative organisms were eradicated or markedly in 7 of the 15 patients from whom bacteria were isolated. Clinical findings, including fever, revealed that none of the patients, in whom bacteriological efficacy was determined to be poor, exhibited sufficient clinical response. E. faecalis was isolated from 4 of 6 patients bacteriologically determined to have no response. 2. As for 23 patients, who were found to be evaluable among the 25 patients, 8 (34.8%), 4 (17.4%), 4 (17.4%), and 7 (30.4%) demonstrated excellent, good, fair and poor responses, respectively, showing a 69.6% efficacy rate which indicates a sum of percentages of patients with excellent, good and fair responses. 3. While an efficacy rate of 100% was obtained for 3 patients with number of peripheral neutrophils less than 500/mm3 before the beginning of CMZ administration, only an efficacy rate of 66.7% was obtained for 15 patients with neutrophils more than 500/mm3.(ABSTRACT TRUNCATED AT 250 WORDS)

[A case of acute mixed leukemia with expression of J5 and GPIIb/IIIa complex by ABC method].

Identification of the lineage and maturation stage of the leukemic cells is essential for the diagnosis, treatment and followup of acute leukemia. Staining of blood smear by avidin-biotin-peroxidase complex method, which requires no special equipments, enables to observe both morphology and surface antigens at the same time. Simultaneous expression of a B-lymphocyte antigen J5 and a platelet antigen GP IIb/IIIa complex on the same blasts was demonstrated by this technique in the present case.

[Relationship between clinical efficiency of cefoperazone and the value of area under the time concentration curve in infectious diseases].

Clinical efficiency and side effects of cefoperazone (CPZ) against 40 infectious diseases in the field of internal medicine were studied. In addition, relationship between clinical efficiency and the value of area under the time concentration curve (AUC) was evaluated. It was possible to evaluate clinical responses in 36 cases of 40 infections; Responses were excellent in 15, good in 16, fair in 3 and poor in 2. An overall efficacy rate was 86.1%. A high dose of CPZ (2 g X 2/day) produced a higher efficacy rate than a low dose of the drug (1 g X 2/day) [95.8% (23/24 cases) vs. 66.7% (8/12 cases)] but the difference was not statistically significant. It was possible to evaluate bacteriological efficacy in 32 cases. Bacteria were eradicated in 29 (90.6%) and decreased in 3. The value of AUC in the high dose group was significantly higher than that in the low dose group. Values of AUC in cases of excellent or good clinical responses tended to be higher than values in cases of fair or poor responses. Side effects were noted in 4 cases, but they were not serious and improvements were observed without withdrawal of the drug except 1 case.

[A clinical study on cefoperazone in serious infections associated with hematological malignancy].

Clinical and bacteriological efficiencies of cefoperazone (CPZ) in 19 serious infections associated with hematological malignancy were studied. CPZ was clinically effective in 13 cases (68.4%). Excellent effects were obtained in 3 cases (15.8%), good effects in 7 cases (36.8%) and fairly good effects in 3 cases (15.8%). The rate of clinical effectiveness by CPZ was not dependent on neutrophils number in peripheral blood. Bacteriological effectiveness to isolated organisms in 15 cases was obtained in 9 cases (60%). Clinically significant serum concentrations of CPZ persisted over 6 hours after injection (1 g/l hour, d.i.v.). In comparative study of MIC against clinically isolated P. aeruginosa, CPZ was superior to other cephalosporins. Adverse reactions were not significant except for 3 cases with abnormalities of hepatic function.

Suppression of nocturnal pineal N-acetyltransferase activity and melatonin content by inverted magnetic fields and induced eddy currents.

Recent reports suggest that the apparent magnetosensitivity of the rat pineal gland is a result of induced electric currents (eddy currents) in the animals caused by the transients of activation and deactivation of artificially applied magnetic fields (MF). To test this, young adult male rats were exposed at night when pineal melatonin production is high to situations that caused either the induction of eddy currents or eddy currents plus an intermittently inverted MF. Only those animals exposed to both the inverted MF and induced eddy currents demonstrated alterations in nocturnal pineal indoleamine metabolism. These results demonstrate that the response of the rat pineal gland to an inversion of the MF may be a consequence of at least two interacting factors, i.e., the inverted MF and the induced electric currents.

[The characteristics of melatonin biosynthesis in the epiphysis of relatively wild and domesticated silver foxes Vulpes fulvus].

In earlier experiments, it had been demonstrated that at night melatonin concentration in the pineals of foxes Vulpes fulvus which were subjected to a prolonged selection for domestic behaviour, was higher than in undomesticated ones. A more detailed study of melatonin biosynthesis showed that concentration of melatonin precursor, namely serotonin, as well as the activity of N-acetyltransferase and hydroxyindole-O-methyltransferase participating in melatonin synthesis, together with the content of dopamine and noradrenaline which regulate this process, are practically identical in the pineal gland of both groups of animals. Possible causes of differences in melatonin concentration in the pineal tissue of domesticated and undomesticated foxes are discussed.

[The melatonin content of the tissues of relatively wild and domesticated silver foxes Vulpes fulvus].

Pineal, plasma and retina melatonin content and its diurnal dynamics have been studied in adult relatively wild and domesticated female silver foxes. Diurnal cyclicity of melatonin concentration in these tissues was observed with a minimum during the day time, maximum in the middle of the night and intermediate level in the evening. Circadian melatonin dynamics was pronounced in non-domesticated as much as in domesticated females. However, since the pineal mass in domesticated animals is significantly smaller than in wild ones, the quantity of the hormone per 1 mg of the pineal tissue at night was much higher in the former. At the same time, plasma concentration of melatonin in both groups was the same. A possible cause of higher melatonin concentration in the pineal organ of domesticated foxes is discussed.
