[A resected case of pulmonary plasma cell granuloma infiltrating the pericardium].

The patient was a 66-year-old man. An abnormal mass on his chest X-ray was pointed out during a regular check-up in November 1991. He was admitted to our hospital with the chief complaint of productive cough in January 1992. With the diagnosis of lung cancer, right middle and lower lobectomy with lymph node resection and partial resection of the pericardium was carried out. The pathological examination revealed pulmonary plasma cell granuloma infiltrating the pericardium. Postoperative course was uneventful. No recurrence was seen 17 months after operation. As pulmonary plasma cell granuloma is histologically benign, this was a very rare case with extra-pulmonary extension. Up to present, only 8 cases including this case has been reported in the literature. We reviewed these 8 cases.

[Reoperation for late pseudoaneurysm in the ascending aorta following the modified Bentall procedure--a case report].

We performed reoperation for annuloaortic ectasia (AAE) in a patient with Marfan's syndrome. The initial operation was the modified Bentall procedure, in which the root of the ascending aorta and the aortic valve were replaced with a composite graft with a prosthetic mechanical valve, and the coronary arteries were reconstructed by direct anastomosis to the right coronary ostium or with an interposing graft to the left coronary ostium. At 1 year and 6 months after the initial operation, a pseudoaneurysm developed in the ascending aorta following the reconstructed coronary ostium on the right side was detached. At reoperation, the hole in the composite graft was closed, but the right coronary artery could not be reconstructed. Her postoperative course was uneventful. In addition, Piehler's method is effective for reconstruction of the coronary arteries, especially for patients with AAE in the Marfan's syndrome.

Cavernous hemangioma in the cavernous sinus.

Cavernous hemangioma, a rare vascular hamartoma, in the cavernous sinus is described in this report. This case showed a slowly progressive course, which first involved the right optic nerve, followed by the left optic nerve, and then the right oculomotor, trochlear, abducens, and trigeminal nerves. Cavernous hemangioma should be suspected in the case of mass in the cavernous sinus with a very slowly progressive course. MRI is very useful to help diagnose this hemangioma.

[A case of isolated congenital left ventricular diverticulum].

Isolated congenital diverticulum of the left ventricle is a rare malformation. A 2-year-old boy was admitted to our hospital for evaluation of heart murmur. The echocardiogram and MRI showed an extra cavity communicating with the left ventricle. The diagnosis was made by LV angiography. He was diagnosed as having congenital isolated diverticulum of the left ventricle without other cardiac anomalies. He received resection of the diverticulum under cardiopulmonary bypass. Histological examination showed a fibrous diverticulum. Postoperative clinical course was uneventful. We can expect good prognosis after resection of the congenital diverticulum of the left ventricle, if necessary under cardiopulmonary bypass.

[A case report of coronary sinus atrial septal defect].

We experienced an uncommon case of coronary sinus atrial septal defect, which was not associated with a left superior vena cava (LSVC). A 29-year-old woman, who experienced fatigue during moderate exercises, was found to have the atrial septal defect located in the lower part of the septum by echocardiography preoperatively. On operation, the coronary sinus ASD was diagnosed. The ostium of coronary sinus was located in the left atrium. This type is classified as partially unroofed coronary sinus without LSVC. Our case, in the absence of the LSVC, was repaired by simple closure of the coronary sinus ASD, so the coronary veins drained into the left atrium. Postoperative course was uneventful.

A case of ectopic opening of vasa deferentia into müllerian duct cyst.

We report very rare case in which the vasa deferentia entered a large Müllerian duct cyst in a 4-month-old infant. The presenting findings were lower abdominal mass and the complaint of urinary retention. A computed tomography scan of the pelvis showed a monolocular cystic mass between the bladder and the rectum. Surgical removal of the cyst was done because the mass increased in size and both hydronephroses and hydroureters were getting worse despite the use of antibiotics. The patient has no urological difficulty after the operation. This case was characterized as follows: the cyst was located on the midline, the vasa entered the cyst directly, the size of the cyst was large, and it was not associated with other malformations of the genitourinary system.

[Recognition of orthodontic patients and their parents about the orthodontic treatment and results--a questionnaire method].

This study was undertaken to investigate the desire and consciousness of orthodontic patients and their parents on the content and effect of orthodontic treatment. The subjects were 362 post-treatment patients and 353 of their parents, who answered the questionnaires. The following conclusions were obtained: 1. 49% of the patients and 16% of the parents of the patients thought of giving up the treatment while the patients were under the orthodontic treatment. The main reasons were the discomfort of orthodontic appliances, long treatment period, and the absence from school. 2. 55% of the patients felt uneasy about a change of the occlusion for the worse after removal of the orthodontic appliances, and 60-70% of the patients and parents had forgotten the necessity of the retainer after active orthodontic treatment. 3. Both patients and parents hoped to finish the orthodontic treatment by the end of junior high school. 4. About 50% of the parents preferred the university hospital and about 45% preferred a private dental office which was convenient for attending as an outpatient. 5. About 70% of the patients and parents were satisfied with occlusion after orthodontic treatment, and 33% of the patients and 61% of the parents were satisfied with the orofacial appearance after treatment. 6. About 9% of the patients hoped to keep secret their history of orthodontic treatment. 7. About 35% of the patients and 60% of the parents would recommend people with malocclusions to receive orthodontic treatment. About 80% of the patients would make their own children receive the orthodontic treatment if needed. 8. About 90-95% of the patients and parents were pleased with orthodontic treatment.

[On eruption of mandibular third molar after extraction of mandibular first or second molar].

In the orthodontic practice, the first or second molars are extracted by reason of a necessity based on the treatment planning. In these cases, it is desirable that the third molar would be taken part in the masticatory function. This study was made to investigate the natural movement of the erupting third molars after the extraction of the first or second molar on the serial lateral cephalometric radiographs and orthopantomographs. Subjects were divided into three groups. The first is the group which the mandibular second molars were extracted. The second is the group which the first molars were extracted bringing about the successful eruption of the third molars. The third is the group which the first molars were extracted with resultant in the failure of the third molar eruption. In all cases, the orthodontic forces were not applied to the third molars. The results were as follows: 1. In the first group, all third molars were successfully erupted and a lot of the spaces after the extraction of second molars were utilized for the eruption of third molars. 2. In the second group, all third molars were successfully erupted and a lot of the spaces after the extraction of first molars were utilized for the mesial movement of the second molars. 3. In the third group, there were various patterns of the impaction of the third molars. It was suggested that the eruption of the third molar was related to the space distal to the second molar but it was not related to the anterio-posterior length of the mandibular body significantly.

[Clinical examination of resected cases of lung cancer with pleural dissemination].

Preoperative examination using chest computed tomography (CT) of cases with pleural dissemination and no pleural effusion revealed small disseminated nodules of the visceral pleura. However, chest CT could not exactly diagnose those of the parietal pleura. The indications of operation, especially pan-pleuropneumonectomy, for cases with pleural dissemination should be limited to the following cases; those in which no pleural effusion and no metastasis to the mediastinal lymphnodes has been clinically proved and in which it is not necessary to perform extensive combined resection other than pan-pleurectomy and partial resection of the diaphragm and/or pericardium. Well differentiated histological types of lung cancer are better indications for surgical treatment.

[Clinical evaluation on the usefulness and safety of norfloxacin in a twice-a-day regimen against upper respiratory tract infections].

Sixty-six patients with tonsillitis or pharyngolaryngitis received oral norfloxacin (NFLX) 200 mg b.i.d. or 300 mg b.i.d. for not less than 3 days in general at School of Medicine, Yokohama City University, and 6 related hospitals. The results are summarized as follows: 1. Ten patients were excluded from the study because 4 patients did not present at the follow-up visits, 4 patients did not follow the protocol and 2 patients had unclear infections. Thus, 56 patients were evaluated. 2. The fifty-six evaluable patients were classified as follows: Twenty patients were with tonsillitis and 36 were with pharyngolaryngitis. Thirty-eight patients were treated with 200 mg b.i.d. while 18 patients received 300 mg b.i.d. Thirty-two patients were male and 24 patients were female. Three patients were inpatients and 53 patients were outpatients. Most of patients were light or moderate cases, and the 26 patients had mild underlying disorders. 3. Clinical improvements were observed in 11 of 15 patients (73.3%) with tonsillitis and 25 of 30 patients (83.3%) with pharyngolaryngitis (80.0% overall) within 7 days administration. 4. Clinical improvements were observed in 15 of all 20 patients (75.0%) with tonsillitis and 30 of all 36 patients (83.3%) with pharyngolaryngitis (80.4% overall). 5. Bacteriologically, "eradication", "decrease" and "replacement" were observed in 6, 2 and 1 patients, respectively. 6. As for adverse effects, nausea and headache were observed in 1 patient and abnormal changes in laboratory findings were noted in 1 patient (slight elevations of GOT and GPT). 7. Oral NFLX 200 mg b.i.d. or 300 mg b.i.d. was as almost equally effective as 200 mg t.i.d. in tonsillitis or pharyngolaryngitis. Thus NFLX 200 mg b.i.d. or 300 mg b.i.d. appears to give sufficient clinical efficiency. 8. When antibacterial activity and serum half-life of NFLX are considered, twice daily administration was confirmed to be sufficiently effective in the clinical application.

Effects of hypothalamic lesion on pancreatic autonomic nerve activity in the rat.

The effects of hypothalamic lesions and intravenous glucose infusion on the efferent activity of vagal and splanchnic nerves to the pancreas were studied in anesthetized rats. Lesions of the ventromedial hypothalamic (VMH), the dorsomedial hypothalamic (DMH) and the paraventricular (PVN) nuclei increased vagal and reduced splanchnic nerve activity. Lesion of the lateral hypothalamic area (LHA) decreased pancreatic vagal nerve activity, and produced either increased or decreased activity of pancreatic splanchnic nerve. Intravenous glucose infusion increased activity of the vagal nerve and reduced that of the splanchnic nerve. These glucose responses were influenced by hypothalamic lesions only slightly or not at all. The findings suggest that hypothalamic modulation of pancreatic hormone secretion involves both the parasympathetic and sympathetic nervous systems, and provide evidence that not only the VMH and the LHA but also the DMH and the PVN are involved in this mechanism.

Production of d-Tagatose from Dulcitol by Arthrobacter globiformis.

A process for the bacterial oxidation of dulcitol to d-tagatose has been developed. The strain Arthrobacter globiformis ST48 used in this fermentation was isolated from soil. The yield of d-tagatose accumulated in the medium from dulcitol was as high as 85%. About 14 g of d-tagatose crystals was isolated from 1 liter of 2% dulcitol medium.

Clinical studies on diverticular disease of the colon.

In 12,505 patients who received barium enemas during the past 14 days, 979 cases (7.8%) of diverticular disease of the colon (DDC) were found. The incidence of DDC increased with age; 2.2% of the patients were below age 29 and 14.3% were over 70 years old. The sex distribution was 584 males and 395 females. 76.1% had diverticula in the right-side colon, 12.9% in the left-side colon, and 11.0% in both sides. All patients below age 29 had the right-side colon type, while 44% of those aged 70 or more did. On the other hand, the left-side colon type and both-sides colon type were found only in patients over 30 years old, and constituted a majority in those of 70 years old or more. The main symptoms and signs were: disturbances in bowel habits 50.7%, abdominal pain 48.1%, abdominal distension and/or discomfort 41.5%, occult blood 25.2% and melena 5.8%. Therefore, DDC is clinically becoming an important disease in recent years in Japan as well as in Western countries.
