We evaluated 3,066 consecutive women admitted during 1 year to two major hospitals of Kuala Lumpur and the adjacent urban area of Malaysia. Indicators of acute pelvic inflammatory disease were more common among patients with induced abortions. PID was thought to be a major contributor to the higher costs associated with management of patients with induced abortions.
Labor was successfully induced by forewater amniotomy and oral administration of prostaglandin E2 tablets in 91.3% of 153 women with singleton pregnancies. Deliveries occurred within 12 hours for more than 90% of the successfully induced patients. Although not statistically significant, a higher pelvic score appeared to be associated with shorter induction-to-delivery intervals. There were no harmful side effects for either mother or baby; only 14.5% experienced minor side effects, primarily gastrointestinal upsets.

With 25% of its population living in over 148 squatter settlements, with a high incidence of communicable diseases, teenage pregnancies, and psychological and familial stress, the city of Kuala Lumpur has sought ways to improve conditions. This article describes one particularly promising approach: community-based centers integrating three socioeconomic components--preschool education, maternal and child health clinics, and income-generating activities.
