[The cerebral angiogram in cases of stroke in youth (author's transl)].

Sixty-seven patients (ages: 11-40 years) suffering from ischemic cerebral circulatory disturbances were investigated clinically and angiographically. In 34 cases, transient ischemic attacks or reversible ischemic neurological defects were diagnosed; 33 patients suffered from completed strokes. In most cases the completed stroke took place without previous transient ischemic attacks. A very good tendency toward recovery was observed in 60 of 67 cases. Stenoses or occlusions were found in only 21.5% of the clinically affected vessel regions. However, 50% of those patients, on whom panangiography was performed, were shown to have stenoses or occlusions. Stenotic vessel wall lesions are also obviously to be expected in clinically nonaffected vessel regions; consequently, in cases of cerebral circulatory disturbances, angiographic investigation of all craniocervical vessels is advisable.

Diagnostic and pathomorphological aspects of glioma multiplicity.

A series of 11 patients with multiple glioma foci is reported with emphasis upon isotope brain scan, angiography, and CT findings; autopsy data is available in 8 cases. In many patients it was necessary to combine the results of several diagnostic techniques in order to demonstrate all the foci proven at autopsy. Thus, the desirability of combining diagnostic techniques in the investigation of glioma patients must be stressed. In spite of this approach, however, multiple metastases and the various types of multiple gliomas are often indistinguishable from each other by current diagnostic techniques.

[Quantitative evaluation of lateral ventricle width by means of CT--values obtained from a normal population (author's transl)].

The possibilities of differentiation between a "normal" and an "enlarged" ventricular system by means of Computerized Tomography are limited. The main problems are the definition of the Normal Population and the fact that a certain methodical error in measurement is not to be avoided. On the other hand, that differentiation is essential for the diagnosis of Ventricular Enlargement, Hydrocephalus internus and Brain Atrophy. The daily practice requires a method, which allows an accurate measurement as well as a quick and simple performance. The evaluation of the width of the lateral ventricles by means of the Ventricle-Index-Quotient (VIQ) and of the Ventricle-Waist-Quotient (VTQ) is likely to meet all these requirements. The results have been obtained from the computer-tomograms of 53 men and 69 women (n = 122), aged between 15 and 80 years. A statistically significant correlation was found between the VIQ- and VTQ-values on the one hand, and the patient's ages on the other. Consequently, whether normal values or the diagnosis of brain atrophy are in question, the patient's age has always to be considered.

[Diagnosis of cerebral atrophy on the basis of the cerebral angiogram (author's transl)].

An increased distance between the brain surface and the skull causes the appearance of a broad avascular band in the carotid angiogram, which -- apart from other criteria -- has been valued as a sign of cerebral atrophy. 54 patients (60 to 79 years old), suffering from cerebrovascular diseases, were examined. Carotid angiography of the afflicted hemispheres were carried out within 4 to 90 days after the stroke. The initially mentioned distance between the skull and the brain surface, represented by the terminal arterial branches, was measured in frontal and lateral projection. The influence of alterations of the wall of extracranial vessels, of the number of attacks, of the severity of the stroke as well as of the patient's age on the distance was statistically evaluated. The results were compared with those of cranial computerized tomography. Only limited correlations between the mentioned parameters and the distance between the vessels and the skull have been found. Thus the distance is only of limited diagnostic value for the verification of senile cerebral atropy on a vascular basis.

The value of CT and EEG in cases of posttraumatic epilepsy.

The results of the clinical neurological investigation were compared with those of electroencephalography (EEG) and computed tomography (CT) in 64 patients suffering from verified posttraumatic epilepsy. Only 18 patients (28%) showed central neurological features with corresponding focal disorders on CT (15 cases) and EEG (11 cases). The combined application of both methods led to positive results in 94% on the part of at least one accessory examination. The clinical neurological investigation as well as the EEG and CT were normal in only 3 cases, although the traumatic etiology of epilepsy was beyond doubt. In addition to the clinical neurological investigation, EEG and CT are most important accessory examinations for the diagnosis and followup studies in cases of posttraumatic epilepsy.

[Cerebral angiography in collagen disease and arteritis of different aetiology (author's transl)].

The cerebral angiograms of 11 patients suffering from collagen disease are presented. Panarteriitis nodosa was diagnosed in 4 cases, Lupus erythematodes in 2 cases. With 5 patients immunovasculitis with cerebral affection was found, which was, however, not to be classified in detail. More or less characteristic features are to be expected in the angiogram; they might harden the suspicion of collagen disease, although they are not likely to prove its diagnosis. An interpretation of the radiological findings should--in addition to the morphology--primarily take into account the distribution type of the vessel wall lesions. Clinically as well as by means of angiography it is difficult to differentiate between collagen disease and cerebral arteriitis of different aetiology; this applies particularly to the alterations in cases of embolic circumscribed encephalitis in sepsis lenta. The diagnostic value of angiography in cases of collagen disease with cerebral affection is discussed, the criteria of cerebral arteriitis of different aetiology are dealth with.

[Indications for neuroradiological investigations using contrast media in patients with psychiatric disorders (author's transl)].

The results are presented of neuroradiological investigations using contrast media in 100 patients hospitalized in a psychiatric department. The existence of neurological disturbances or the psychoorganic syndrome, as well as the results of screening methods such as EEG and brain scan are important factors in the decision for or against the performance of cerebral angiography or pneumoencephalography. The significance of the "brain atrophy syndrome" accompanying psychotic diseases is discussed. In addition, some cases are demonstrated of organic neurological brain diseases with predominantly psychiatric features, where the diagnosis was reached on the basis of only neuroradiological methods using contrast media.

[The significance of transfemoral cerebral angiography in the diagnosis of cerebrovascular diseases (author's transl)].

Comprehensive angiographical investigation is a prerequisite for successful vascular surgery in the region of the head and neck. An examination of the clinically involved region alone is not normally sufficient. The decision to operate can be made only after taking into account the results of cerebral panarteriography, i.e. investigation of the flow areas of both carotid arteries and the vertebral-basilary circulation, together with any comparable method, the advantages and dangers must be flow measurement. Transfemoral cerebral angiography (TCA) is not only the least strenuous method for the patient, butit is also the most effective available, especially as far as assessment of the origins of the four large cranio-cervical vessels is concerned. However, as with any comparable method, the advantages and angers must be weighed up carefully beforehand in each individual case. A definite indication is absolutely necessary. This paper describes the technique, indications and results of investigations in 70 patients suffering from cerebrovascular diseases.

[Cerebral blood flow and social prognosis after cerebrovascular ischaemia (author's transl)].

Cerebral blood flow (CBF) was measured in 180 patients one to eight weeks after sustaining an acute episode of cerebrovascular ischaemia. The patients were divided into four groups in relation to their performance of daily activities at the time of CBF study: score 0 - full previous activities; 1 - slight; 2 - moderate; and 3 - severe disability. There was a statistically highly significant difference between the flow values of these groups. The patients were regrouped according to their rehabilitation status (score 0 to 3 as above, score 4 - dead) after a mean observation period of 34 months. The differences between flow values were even higher now, significant contingency coefficients being found between flow values and final social scores. When the patients were grouped according to changes in performance scores there again were statistically significant differences in flow values between the groups. The relationship between flow values and social re-integration after cerebrovascular ischaemia indicates the value of CBF measurements for prognosticating a patient's rehabilitation ability after stroke.

[Cerebral angiography with the contrast medium vasobrix 32 (ioxitalamate) (author's transl)].

On the basis of angiograms carried out 168 patients, the contrast qualities achieved with the contrast medium, Vasobrix 32 were compared with those of other contrast media used so far. In the case of carotid angiography, Vasobrix 32 allowed better visualisation in the arterial phase than achieved with the contrast media used so far. The superiority of Vasobrix 32 over other substances was even more evident in the venous phase. As regards visualisation of the branches of the small arterial vessels such as the striothalamic branches, the results achieved with Vasobrix 32 were better than with the contrast media used so far and, therefore, a general improvement in visualization can be expected from Vasobrix 32 (Ioxitalamate). Complications with neurological sequelae were not observed, subjective complaints occurring to a slightly lesser extent than in the case of contrast media with a high sodium salt content.

[Computerized tomography in infants with seizures (author's transl)].

A total of 228 children suffering from seizure disorders were investigated by means of cranial computerized tomography (CT). Their age ranged from 3 weeks to 15 years. Structural abnormalities were assessed in 63% of these cases. The greatest incidence of organic abnormalities occurred in children with major epilepsy (83%) and with focal seizures (71%). Clinical data and the course of illness may require additional angiographic and repeated CT investigations.

We evaluated statistically the impact on this distance of alterations in extracranial vessel walls, number of attacks, severity of stroke, and patient's age. The results are compared to those of CAT. Only limited correlations have been found between the above-mentioned parameters and the distance between vessels and skull. Thus the distance is only of limited diagnostic value for the verification of senile cerebral atrophy on a vascular basis.
