Action of rHuEpo on mechanical membrane properties of red blood cells in children with end-stage renal disease.

By means of a micropipette aspiration technique mechanical membrane properties of RBC in ten uraemic children undergoing haemodialysis were compared to those of nine healthy children. In contrast to the healthy reference group (mu = 4.01 +/- 0.71 microN/m), the mean apparent elastic shear modulus of RBC membrane mu as a parameter of static deformability was significantly increased to 9.05 +/- 1.61 microN/m in children with chronic renal failure measured in the pre-study period. At the 7th week of the correction period of rHuEpo therapy, the haematocrit of treated patients is enhanced to a mean of 0.30 +/- 0.03, whereas mean mu is decreased to 5.41 +/- 0.63 microN/m. Within the maintenance period of rHuEpo treatment (30th week), the parameter mu did not show significant differences to the reference value 3.78 +/- 0.31 microN/m. Additionally, a significant increase in red cell mean corpuscular volume was obtained during rHuEpo therapy. Improved deformability of uraemic RBC induced by the rHuEpo therapy can only be explained by the assumption that in the course of this therapy an increasing number of cells with normalised viscoelastic properties have been formed by stimulated erythropoiesis.

Nucleotide and glutathione status in erythrocytes of children undergoing chronic hemodialysis under erythropoietin treatment.

Recombinant human erythropoietin (rhEPO) used for the treatment of children with renal anemia is an effective stimulus of erythroid proliferation and differentiation. The increases of reticulocytes and of the hematocrit values give the possibility to renounce further blood transfusions in dialysis patients. There are no beneficial in vivo effects of rhEPO on the glutathione status and on the nucleotide contents of red blood cells. Therefore, indices for stabilizing effects of rhEPO on the circulating red blood cells cannot be due to glutathione and nucleotide metabolism.

Evaluation of indicators of erythropoiesis stimulated by recombinant human erythropoietin in renal anemia.

Recombinant human erythropoietin (rh-EPO) was administered to 11 anemic children with end-stage renal disease who were undergoing hemodialysis. Hematocrit (Hct), absolute reticulocyte count (retics), creatine concentration of red cells and erythrocyte density test (EDT) were chosen as indicators of rh-EPO effect on erythropoiesis. In the present report the first 9 weeks shall be presented in which all patients received an identical dosage of rh-EPO per kg body weight. The pre-EPO values of retics, creatine and EDT varied considerably. In 10 patients the Hct increased to 0.30 after 9 weeks of rh-EPO treatment. Retics and creatine rose in all cases above the normal range, rates of increase and maximal values differed. Retics, creatine and EDT respond faster to rh-EPO than Hct. Retics and creatine seem to be equally good indicators of rh-EPO stimulated erythropoiesis. The EDT showed an increase only up to the 4th week.

[Effect of human uremic serum on the survival of fetal rat hepatocytes].

Children suffering from chronic renal insufficiency frequently show a liver cell damage. It is not clarified, whether this damage is caused immediately by chronic renal insufficiency. In the model of fetal rat hepatocytes, which were isolated by trypsin digestion, we have investigated the influence of normal and uremic serum on the lactate dehydrogenase release and the staining with trypan blue, respectively. Fetal rat hepatocytes under incubation conditions seems to be a useful model in the investigation of the influence of uremic metabolism on the liver cell. In our liver cell model the uremic serum did not influence the membrane permeability of hepatocytes for macromolecules. However, the viability of hepatocytes was impaired by 23% compared with normal serum. The vitality-reducing factor of the uremic serum is not dialysable.

[The incidence of kidney diseases in a pediatric kidney dispensary].

400 children (141 boys and 259 girls) in a renal disease care system were analyzed regarding their most frequent diagnoses of renal diseases and their age and sex distribution. Furthermore, the familiary situation, the time of observation, the number of consultations and the frequency of morphological changes of the kidney and the urinary tract were examined. 72% of the children suffered from urinary tract infection and in 10% a glomerulopathy and in 5.5% an urolithiasis were found. The symptoms of enuresis occurred in 22.2% of the patients. Infants were the most frequent age group (18%), 70% of which were boys. We found no differences in the incidence of renal diseases in comparison with other reports.

[Oral galactose loading in characterizing the elimination and metabolizing performance of the liver in chronic renal failure].

For characterising the elimination and metabolisation performance of the liver in chronic renal insufficiency the galactose load test was carried out. 9 children with healthy liver and kidneys, 35 children with chronic renal insufficiency (15 were conservatively treated = group 1, 20 were in the chronic haemodialysis programme = group 2) as well as 5 children after kidney transplantation were examined. In group 1 a normal blood galactose concentration was present. Patients of the 2nd group showed increased concentrations of galactose in the blood which might refer to a decreased redox potential in the liver caused by the uraemic intoxication. In the group of patients who underwent a kidney transplantation in the patients with azathioprine therapy a disturbed use of galactose was present. Children with cyclosporin-A had a normal galactose concentration.

[Subvesical urinary track obstruction in children as the cause of chronic renal failure].

The infantile stenosis of the vesical cervix is a rare disease, etiology and pathogenesis of which are controversial. If it is not treated it leads to uraemia. On the basis of a clinical material consisting of 8 patients clinic and course of the disease are discussed. In the children with stenosis of the vesical cervix observed by us disturbances of the miction (bilateral mictions, thin urinary stream), relapsing abdominal pain, enuresis, minor growth and haematuria were in the first place. We got clearness on the diagnosis by judging the mode of miction, by means of miction cystourethrography and by urethrocystoscopy. Even in stenosis of the vesical cervix with advanced renal insufficiency also in childhood an operation should be performed.
