A systematic review on ankle injury and ankle sprain in sports.

This article systematically reviews epidemiological studies on sports injury from 1977 to 2005 in which ankle injury was included. A total of 227 studies reporting injury pattern in 70 sports from 38 countries were included. A total of 201,600 patients were included, with 32,509 ankle injuries. Ankle injury information was available from 14,098 patients, with 11 847 ankle sprains. Results show that the ankle was the most common injured body site in 24 of 70 included sports, especially in aeroball, wall climbing, indoor volleyball, mountaineering, netball and field events in track and field. Ankle sprain was the major ankle injury in 33 of 43 sports, especially in Australian football, field hockey, handball, orienteering, scooter and squash. In sports injuries throughout the countries studied, the ankle was the second most common injured body site after the knee, and ankle sprain was the most common type of ankle injury. The incidence of ankle injury and ankle sprain was high in court games and team sports, such as rugby, soccer, volleyball, handball and basketball. This systematic review provides a summary of the epidemiology of ankle injury in sports.

Trabecular bone status in ultradistal tibia under habitual gait loading: a pQCT study in postmenopausal women.

This study investigated regional volumetric trabecular bone mineral density (tBMD) and bone area at the ultradistal tibia in Chinese women using peripheral quantitative computed tomography. Fifty-six postmenopausal women aged 47-62 yr participated in BMD measurements at baseline and 22 of them were followed at both 1-yr and 3-yr follow-up scans. Regional baseline tBMD, rate of annual bone loss, and trabecular bone area were determined. Baseline measurements showed that the tBMD of both the posterior (252.9+/-63.4 mg/cm(3)) and medial (226.6+/-68.9 mg/cm(3)) regions was significantly higher than that of the anterior (126.3+/-61.9 mg/cm(3)) and lateral regions (149.8+/-50.6 mg/cm(3)), respectively (p<0.001). Both the 1-yr and 3-yr follow-up measurements showed that there was significant physiological annual tBMD loss on an average of 1.61%, at the four regions. Inter-slice regional tBMD and trabecular bone area measurements demonstrated a significant linear decrease from the distal to proximal aspects (p<0.001). Findings suggest that dynamic compressive loading during the heel strike and the body weight vector shifting toward the medial aspect during the stance phase in a normal gait might account for the regional tBMD differences. Increased tBMD and bone area toward the distal tibial endplate may adapt to withstand the axial impact loading. However, the low-impact weight-bearing nature of a normal gait may not be osteogenic to prevent regional bone loss. An exercise program specific to the women at risk should be contemplated.

Virtual reality techniques. Application to anatomic visualization and orthopaedics training.

Surgical training systems using virtual reality simulation techniques offer a cost-effective alternative to traditional training methods. In this sense, techniques for interactive visualization and virtual reality surgery have been one of the very important research areas. We describe various techniques we have used in developing a virtual reality system for anatomic visualization and training arthroscopic knee surgeons. Virtual models used in our systems are constructed from the Visible Human Project and Chinese Visible Human data sets. We present our various developments in segmentation, personal-computer-based real-time volume visualization, soft tissue deformation with topological change in real-time using finite element analysis, and soft tissue cutting with tactile feedback.

TGF-beta1 reverses the effects of matrix anchorage on the gene expression of decorin and procollagen type I in tendon fibroblasts.

Transforming growth factor-beta1 is known for its effect on the production of extracellular matrix in tendons. Elevated levels of transforming growth factor-beta1 have been reported in tendon adhesion and tendinosis, which suggests that transforming growth factor-beta1 plays an important role in matrix disturbances. Tendon adhesion involves excessive collagen deposition, whereas tendinosis is associated with increased proteoglycan deposition. It seems that other factors also may affect matrix deposition and modulate the effects of transforming growth factor-beta1. We assessed whether matrix anchorage to Type I collagen or fibronectin could change the gene expression of matrix proteins in tendon fibroblasts, and studied whether the effects of transforming growth factor-beta1 were altered by matrix anchorage. Human patellar tendon fibroblast cultures were prepared in different cell anchorages, and the cellular responses to transforming growth factor-beta1 were measured as gene expression of procollagen Type I, Type III, decorin, and biglycan by real-time reverse transcriptase-polymerase chain reaction. Fibronectin anchorage significantly increased the messenger ribonucleic acid level of decorin, and the messenger ribonucleic acid level of procollagen Type I was decreased by matrix anchorage to either fibronectin or Type I collagen. Transforming growth factor-beta1 increased the messenger ribonucleic acid level of procollagen Type I in Type I collagen-coated plates, but it suppressed the messenger ribonucleic acid level of decorin in fibronectin-coated plates. These findings suggest that interaction of matrix anchorage and transforming growth factor-beta1 is an important determinant of matrix deposition in healing tendons and the development of matrix disturbances in tendons.

A virtual-reality training system for knee arthroscopic surgery.

Surgical training systems based on virtual-reality (VR) simulation techniques offer a cost-effective and efficient alternative to traditional training methods. This paper describes a VR system for training arthroscopic knee surgery. Virtual models used in this system are constructed from the Visual Human Project dataset. Our system simulates soft tissue deformation with topological change in real-time using finite-element analysis. To offer realistic tactile feedback, we build a tailor-made force feedback hardware.

The effects of verbascoside on plasma lipid peroxidation level and erythrocyte membrane fluidity during immobilization in rabbits: a time course study.

We studied the changes in the level of plasma lipid peroxidation indicated by malondialdehyde (MDA) level and erythrocyte membrane fluidity expressed by the value of order parameter (S) during single hindlimb immobilization of 21 days using thiobarbituric acid - reactive substances method and spin label electron spin resonance method, respectively. The impacts of verbascoside that has been proved its antioxidative activity on the measured parameters were examined. 11 New Zealand white rabbits were immobilized and divided into two groups. The rabbits in the verbascoside group were administrated with 0.8 mg/kg of verbascoside twice a day orally throughout the immobilization. The rabbits in the placebo group were treated with normal saline. In placebo group, the results showed that the level of MDA significantly increased on day 3, peaked on day 7, and was still significantly higher on day 14 of immobilization, compared with the value measured before immobilization. The value of S reached the highest on day 7 and subsequently lowered but still higher on day 14 than those measured before immobilization. Compared with placebo group, there were lower MDA level (P < 0.05, 0.001, and 0.05 for days 3, 7, and 14, respectively) and higher erythrocyte membrane fluidity (P < 0.05, 0.001, and 0.001 for days 3, 7, and 14, respectively) in verbascoside group. The data indicated that immobilization caused temporal changes of increase in plasma lipid peroxidation and decrease in erythrocyte membrane fluidity. Verbascoside might have the effects to moderate oxidative stress and erythrocyte membrane fluidity during immobilization.

The roles of bone morphogenetic protein (BMP) 12 in stimulating the proliferation and matrix production of human patellar tendon fibroblasts.

Recombinant human (rh) bone morphogenetic protein 12 (BMP12) is proved to induce the formation of tendon and ligament tissues in animal experiments. But the roles of BMP12 on tissue regeneration in human tendons remain unexplored. In the present study, healthy human patellar tendon samples were collected for histological examination and preparation of tendon fibroblast culture. Immunohistochemical staining showed that BMP12 was detected on healthy patellar tendon samples, only located on active tenoblasts and perivascular mesenchymal cells but not in interstitial tenocytes. The expression of PCNA and procollagen type I also exhibited a similar distribution. It indicates that BMP12 may be involved in matrix remodeling process in adult tissues. In vitro studies showed that rhBMP12 could increase proliferation of tendon fibroblasts and increase the gene expression of procollagen type I and type III, but decrease the gene expression of decorin in tendon fibroblasts culture. Our findings suggest that BMP12 may play a role in early phases of tissue regeneration in tendons.
