Azythromicin versus amoxicillin-clavulanate in the treatment of acute sinusitis in children.

In this prospective study we compared the efficiency of azithromycin and amoxicillin-clavulanate in treatment of acute sinusitis in children. Seventy patients were included in the age between 5 and 15 years. Beside ENT and pediatricians examination, nasal and throat smear on culture and antibiogram is taken from all the patients, as well as, X-ray of paranasal sinuses and laboratory findings, followed by check-up of nasal and throat smear and X-ray of paranasal sinuses. Azithromycin in single daily dose of 10 mg/kg during three days showed same efficiency as amoxicillin-clavulanate given three times per day in dose of 45 mg/kg during ten days.

[Stents in treatment of laryngotracheal stenoses].

Endo larynges tracheal stenoses due to endo luminal or extra luminal processes of benign or malignant character in majority of cases demand the resection of a stenosed part. The use of a stent represents a necessity. In this paper we analyzed 10- year experience of the ENT Clinic of the University Clinical Center Sarajevo regarding the use of stents. 26 patients underwent surgery in a period 1995-2005. Prolonged endo tracheal intubations are encountered as a primary cause of stenoses (73 %). Infection and granulations are noted as a complication in 15,3% of cases. Stent is removed in 16 patients within 3 months and in 7 patients after 12 months.

[Surgical treatment of juvenile nasopharyngeal angifibroma].

Juvenile nasopharyngeal angiofibroma represents non-incapsulated benign tumor. It can lead to serious problems with the extension into nearest structures. Recidivism is described in 20 to 30 % of cases, after surgical or other adjuvant therapy. Three patients with juvenile nasopharyngeal angiofibroma that underwent surgery in a five year period (2001-2005) at the ENT Clinic of the University Clinical Center in Sarajevo are evaluated in this paper. No complications were encountered during surgical procedure and early postoperative period. Recidivism are not noted two years after the surgery. Surgical method of choice in stage IA and IB is combined transnasal-oropharyngeal technique. Other surgical procedures are used for more extensive processes.

[Surgical treatment of thyroid gland diseases].

Thyroid gland surgery is a treatment of choice for morphological changes of the gland, although patients with some functional problems, indicated from endocrynologist, underwent surgery. The goal is to analyze retrospectively patients with thyroid gland diseases according to incidence and surgery. We analyzed surgically treated patients with thyroid gland disease at ENT clinic in Sarajevo in last 5 years. There were 339 patients, 312 (92%) were patients with benign diseases, and 17 (8%) patients with malignant disease. Malignant diseases of thyroid gland were treated in two acts and benign forms in one act. Injury of recurrent nerve was not encountered, neither was postoperative hypoparathyroidism.

Most of the patients were surgically treated with total laryngeoctomy with unilateral or bilateral dissection and thyroidectomy or lobectomy (29%) or total laryngectomy with thyroidectomy or lobectomy (23%), total laryngectomy (22%). The rest of the patients underwent total laryngeoctomy with unilateral or bilateral dissection (16%) chordectomy (4%), supraglottic laryngectomy (3%), hemilaryngectomy (2%) and hemilaryngectomy with dissection (1%) All patients were postoperatively irradiated and chemotherapy was combined with irradiation only in younger patients.
