Efficient and stable gene expression in rabbit intervertebral disc cells transduced with a recombinant baculovirus vector.

STUDY DESIGN: A recombinant baculovirus vector expressing GFP (Ac-CMV-GFP) was tested in rabbit intervertebral disc cells cultured in monolayer in vitro. Direct infection of intervertebral disc cells was then assessed in vivo. OBJECTIVES: To test the efficacy of the baculovirus-mediated marked gene transfer to rabbit intervertebral disc cells, and the expression of the transgene in vitro and in vivo. SUMMARY OF BACKGROUND DATA: The potential application of gene therapy for the treatment of degenerative disc disease has been an area of exciting and active research. Many efforts have been made toward the construction of delivery vectors using mammalian viruses, such as adenovirus, adeno-associated virus, retrovirus, and herpesvirus. However, recently, baculovirus-derived vectors have emerged as possible tools for gene transfer into mammalian cells in vitro and in vivo but, to our knowledge, have never been tested in an intervertebral disc cell. METHODS: The intervertebral disc cells cultured in monolayer were treated with 6 different doses of baculovirus carrying the green fluorescence protein gene (Ac-CMV-GFP). Fluorescence microscopy and flow cytometry were used to analyze transgene expression. The Autographa californica nucleopolyhedrovirus/GFP virus was then injected directly into the intervertebral discs of 8 rabbits at 7, 13, 20, and 28 days after injection. The nucleus pulposus tissues of injected discs were evaluated immediately by fluorescence microscopy for GFP expression. RESULTS: A dose of Ac-CMV-GFP at a multiplicity of infection of 200 achieved the highest transduction ratio (approximately 87% of nucleus pulposus cells) and long-term expression without any toxicity to the cells. In vivo assay showed that Ac-CMV-GFP could also mediate GFP expression in rabbit intervertebral disc cells without inducing any symptoms. The GFP expression level at 7 days after transduction was significantly higher than at 21 and 28 days after treatment. CONCLUSIONS: Baculovirus can transfer exogenous genes into rabbit nucleus pulposus cells at a high efficiency, and safely both in vitro and in vivo. The results suggest that baculoviruses might be useful tools as gene therapy vectors for intervertebral disc diseases.

[Repairing widespread traumatic soft tissue defects in lower limb with free latissimus dorsi muscle-skin flaps].

OBJECTIVE: To explore the results of repairing widespread traumatic soft tissue defects in the heels and adjacent regions with free latissimus dorsi muscle-skin flaps. METHODS: From March 1998 to May 2005, 10 cases of widespread traumatic soft tissue defects in the heels and adjacent regions were repaired with free latissimus dorsi muscle-skin flaps; Of the 10 patients, 9 were male and 1 was female, whose ages ranged from 32 to 60 years, and the disease course was 2 hours to 2 months. The defect was by plough-machine injury in 5 cases, by crush injury in 2 cases, by snake injury in 2 cases, and electricity injury in 1 case. Eight cases of defects involved in the posterior of heel and leg, the defect area ranged from 21 cm x 12 cm to 35 cm x 15 cm; 2 cases had widespread soft tissue defects on heel, ankle, sole and dorsal foot, and the defect area was 27 cmX 14 cm and 30 cm x 21 cm respectively. All cases were accompanied by the exposure of bone; 6 cases by fracture; 4 cases by open infection of ankle joint; and 2 cases by injuries of the posterior tibial vessel and the tibial nerve. The sizes of the dissected flap ranged from 25 cm x 14 cm to 33 cm x 24 cm. The donor sites were covered by large mid-thickness flap. RESULTS: There were no postoperative complication of vascular crisis and infection. Ten flaps survived completely and the wounds healed by first intention. After a follow-up of 3 to 24 months, five cases received two-stage plastic operation because bulky flaps bring some trouble in wearing shoes. In 5 cases of reconstructed sensation, two cases recovered pain and temperature sensation. All cases recovered the abilities to stand and walk without ulcer complication. CONCLUSION: The free latissimus dorsi muscle-skin flap is an ideal flap for repairing widespread traumatic soft tissue defects and infectious wounds with muscle defects and bone exposure in the heel and adjacent regions, because it has such advantages as adequate blood supply, big dermatomic area, and excellent ability to resist infection.

[Operation on upper cervical instability without injury].

OBJECTIVE: To study operative methods of treating upper cervical spine instability without injury. METHODS: Twenty-three cases were treated by internal fixation with autologous bone grafts. Atlantoaxial arthrodesis were performed in 10 cases with interlaminar clamp (5 cases), Atlas cable system (3 cases) and Brooks (2 cases). Occipitocervical fusion were performed in the other 13 cases by using of CD-cervical (3 cases), Cervifix (8 cases) and U-stick fixation (2 cases). RESULTS: All the 23 cases were followed up for 2. 5 years in average (ranged from 6 months to 5 years). Solid arthrodesis was obtained in all 23 cases. Six months after operation, of the 20 cases with pre-operation nervous lesion, improvement was achieved in 16 cases. According to JOA standard and Hirabashi formula, the rate of improvement was 27.1%. CONCLUSION: Posterior fusion is recommended for upper cervical instability.

[Stress changes of upper lumbar facet joint after discectomy and artificial disc replacement].

OBJECTIVE: To explore the influence of discectomy and artificial disc replacement on the upper lumbar facet joint and to provide the evidence for use of artificial disc replacement. METHODS: Under the loads of 200-2000 N axis pressure and 1-10 Nm back-extending/side-bending pressure on the 7 fresh adult corpses, the pressure of L3.4 facet joint was measured in the case of L4.5 disc integrity, discectomy and artificial disc replacement and the statistical analysis was done. RESULTS: Under the same load (axis, back-extending/side bending), there were significant differences (P<0. 01) in the pressure of upper lumbar facet joint both between disc integrity and discectomy and between discectomy and artificial disc replacement, and there was no difference (P>0. 05) between disc integrity and artificial disc replacement. CONCLUSION: The stress of upper lumbar facet joint reduced after discectomy, it implies that simply discectomy may change biomechanics of lumbar and lead to secondary lumbar structure degeneration. The stress of upper lumbar facet joint is close to the normal value after artificial disc replacement, it implies that artificial disc replacement may protect lumbar structure. It provides some biomechanics foundation and evidence for artificial disc replacement.

[Treatment of ischemic necrosis of the femoral head by transplantation of pedicled bone flap with deep iliac circumflex vessel].

OBJECTIVE: To investigate the treatment method of ischemic necrosis of the femoral head by the transplantation of pedicled bone flap with deep iliac circumflex vessel in adult and to assess its curative effect. METHODS: From February 1996 to September 2003, 46 adult patients with ischemic necrosis of the femoral head (58 coxas) were treated by transplantation of pedicled bone flap with deep iliac circumflex vessel. The locations were the left side in 34 coxas and the right side in 24 coxas(stage II in 16 coxas, stage III in 39 coxas and stage IV in 3 coxas). RESULTS: Thirty-seven patients (40 coxas) were followed up 6 months to 10 years after operation. According to the assess of FanQishen, the result was excellent in 19 cases (21 coxas), good in 11 cases (12 coxas), fair in 5 cases (5 coxas) and poor in 2 cases (2 coxas). The postoperative excellent and good rate was 81%. CONCLUSION: Operative treatment of ischemic necrosis of the femoral head in adult by transplantation of bone flap pedicled with deep iliac circumflex vessel has the characteristics of restoration of blood supply of the femoral head, decompression of marrow cavity, elimination of necrotic bone and support of the femoral head. It is an effective and advanced method for treatment of ischemic necrosis of the femoral head in adult.

[Simultaneous bilateral total hip arthroplasty in a single procedure].

OBJECTIVE: To study the effect of simultaneous bilateral total hip arthroplasty in a single procedure. METHODS: From October 1999 to March 2004, 15 patients (30 hips) underwent simultaneous sequential bilateral total hip arthroplasty (THAs) in a single procedure. Of the 15 patients, 11 were male (22 hips) and 4 were female (8 hips). Their ages ranged from 35 to 70 years. Their courses of disease ranged from 1 year to 50 years (4.8 years on average). The Harris scores of the joint function before the operation ranged from 12 to 45 points (27 points on average). Five were done with Smith-Peterson and 10 were done with Moore. RESULTS: The operative time was 3 hours and 25 minutes to 5 hours (4 hours and 10 minutes on average). The volume of blood transfusion during operation was 400 to 2 400 ml (1160 ml on average). All the 15 patients were followed up for 3 to 35 months (18 months on average). The Harris scores of the joint function after the operation ranged from 70 to 100 points (86 points on average). There was significant difference in the scores between before and after operations (P<0.05). There was only 1 death within 1 months of the operation and no serious between complications such as infection, pulmonary embolism, and deep vein plug. All the patients were still ambulant in the community and gained significant pain relief. CONCLUSION: Simultaneous bilateral total hip arthroplasty in a single procedure is a safe and effective method. However, the decision of performing single-stage bilateral total hip arthroplasty should be carefully made and preoperative preparation should be sufficiently made.

[Clinical review of thirty-nine cases of ulnar tunnel syndrome].

OBJECTIVE: To discuss the concept of ulnar tunnel at the wrist, the types, causes, traits of compression, diagnosis, and clinical significance of ulnar tunnel syndrome (UTS). METHODS: Thirty-nine cases diagnosed as having UTS from 1986 were retrospectively reviewed combined with previous relevant literature. RESULTS: Ulnar tunnel included Guyon's canal, piso-hamate tunnel and hypothenar segment. There were 8 types and many causes of UTS. Some patients had compression in more than one zones and might be associated with carpal tunnel syndrome or cubital tunnel syndrome. UTS could be diagnosed through clinical manifestations and electrophysiological examination. CONCLUSION: Defining the concept of ulnar tunnel and the knowledge of the complexity and rarity of UTS can effectively guide diagnosis and treatment.
