[Usefulness of isotope diagnosis, ultrasound neck investigation and thyroglobulin assays in quantification of patients operated on for differentiated thyroid cancer and prepared for radioiodine treatment].

UNLABELLED: The aim of the study was to assess the efficacy of surgical treatment of patients operated on differentiated thyroid carcinoma, using Tg serum assays and 131-I whole body scintigraphy. We investigated 208 patients aged from 15 to 78 yr. (mean 43.52, S.S. 16.37) including 182 females and 26 males. 183 cases of papillary cancer and 25 cases of follicular cancer were confirmed by pathologist. All the patients were investigated 6-8 weeks after thyreoidectomy. The following procedures were performed in all the patients: 1. ultrasound imaging. 2. 131 iodine uptake test, 24 hours after administration. 3. 131-I whole body scintigraphy (dose per patient 3 mCi, device Varicam). 4. TSH and Tg serum assay using FIA method. Moreover, all the tissue fragments obtained during surgery underwent histopathological assessment. RESULTS: The mean volume of the post-surgery thyroid tissue remains was 2.91 cc varying from 0 to 40. Volume was counted with Gutekunt's formula. Oncologically suspected lymph nodes were found in 8 cases. The mean TSH serum level was 79.31 (SD 59.59). The mean Tg serum level was 51.73 (SD 179). The mean value of an iodine uptake test was 6.96% (SD 6.69). Whole body scintigraphy discovered solitary thyroid site iodine uptake areas in 199 patients (95.7%). Additional uptake areas were observed in 4.3% 2 (0.9%) cases in cervical lymph nodes, 4 (1.9%) cases in lungs, 3 (1.5%) in bones. The correlation between investigations both laboratory and isotopic, and clinical state was observed in 207 cases (99.5%). The presence of high Tg serum level despite of absence of extracervical iodine uptake areas was observed in one case. We presume that the situation can be caused by the presence of multiple lung metastases not having possibility of iodine uptake. Ultrasound imaging, 131-I scintigraphy and Tg serum assays are essential methods in diagnosing and treatment process in the patients with differentiated thyroid carcinoma.

[Analysis of serum testosterone, estradiol, dehydroepiandrosterone sulfate and gonadotropin concentration and lipid parameters in 50- and 60-year-old men from urban population].

As aging proceeds many different changes occur in endocrine system, particularly in the sexual system. In this paper were presented results of studies on 142 healthy men living in Poznań aged 50 and 60 (group I consisted of 69 persons and group II consisted of 72 persons). The following studies were performed: BMI, serum lipids parameters and hormones estimation including gonadotropins, free and total testosterone, estradiol and dehydroepiandrosterone sulfate (DHEAS). A lower mean concentration of DHEAS, free and total testosterone and a higher frequency of increased gonadotropins concentrations were found in men aged 60 than aged 50. Moreover, positive correlation between BMI and serum total cholesterol and LDL level in the studied group was observed. To conclude, we can say that in men over 50 years of age hormones examination should be performed and in case of insufficient hormones level adequate replacement should be instituted.

[Is hyperthyroidism often present in patients with thyroid differentiated carcinoma?].

The coexistence of thyroid cancer and hyperthyroidism is confirmed by many authors. It appears that the frequency of both disease can be greater as a result of lack of proper and penetrating biopsy diagnosis and of qualification to surgery treatment patients with hyperfunctional goiter and Graves disease, especially coexisting with nodes. The aim of our study was estimation of occurrence of hyperthyroidism in the patients with thyroid differentiated cancer. We examined group of 217 patients with diagnosed thyroid differentiated cancer, 20 patients (9.1%) of them were earlier hyperthyroidism diagnosed. 17 of them were hyperfunctional nodular goiter diagnosed and three as Graves disease, confirmed by presence of anti-TSH receptor antibodies (TRAK). Before thyroidectomy ultrasonography showed nodular goiter in 17 patients and hypoechogenic goiter with nodules in 3 patients. After thyroidectomy at the hyperthyroid patients in 16 papillary thyroid cancer and in 4 follicular thyroid cancers were diagnosed. The frequency of coexistence of cancer and hyperthyroidism in our material amounted for 9.1%. The results of our observation do not diverge from facts given in world literature and is point out the need for precise analysis of patients with hyperthyroidism and of proper qualification to surgery treatments changes suspected to be malignant process.

[Usefulness of serum calcitonin, CEA and AFP assays in the early detection of medullary thyroid carcinoma relapse].

The medullary thyroid carcinoma (MTC) occurs in the two major forms, the sporadic medullary carcinoma (SMTC) and hereditary medullary carcinoma, to which belong familial form (FMTC) and the element of Multiple Endocrine Neoplasia Syndrome MEN 2A and 2B. The method-of-choice of treatment is total thyroidectomy with following radiotherapy in selected cases. Among the follow-up methods, there are two used the most frequently: the pentagastrin and omeprazole stimulation test of calcitonin (CT), and CEA antigen assay. The aim of this study is to evaluate usefulness of plasma CT, CEA and AFP assay in the early detection of relapse or metastasis of MTC. 18 patients (14 females and 4 males) were investigated. The following procedures were performed in all the patients: plasma CT assays in the basal conditions and after stimulation tests, CEA and AFP. The results were analysed according to TNM staging and neck USG results. Our conclusion is that calcitonin stimulation tests, and CEA assays are useful methods to estimate the presence of relapse or metastases in the patients after surgical treatment due to MTC. Assays of plasma AFP concentration are not useful in a follow-up of patients operated on MTC.

[Thyroid gland and pregnancy--physiology and pathology].

Changes in thyroid function and structure during pregnancy, including abnormalities in thyroid ultrasonography. Levels of thyroid stimulating hormone (TSH), free thyroxine (FT4), total thyroxine (TT4), total triiodothyronine (TT3), reverse triiodothyronine (rT3), thyroxin binding globulin (TBG) and thyroglobulin (Tg) as well as the changes in metabolism have been presented. Difficulties in the diagnostics of hyperthyroidism and hypothyroidism in pregnant women have been described. In addition modern ideas about the treatment of thyroid dysfunction in this period of woman life have been presented. Furthermore thyroid physiology and pathology in fetus and newborn have been described.

[Olfaction in congenital hypothyroidism].

The aim of the study is to present results of the olfactory testing in congenital hypothyroidism in the group of 20 subjects. The etiology of hypothyroidism included: Pendred's syndrome in 2 cases, hypoplasia in 2 cases, athyreosis in 5 cases and ectopy of thyroid gland in 11 cases. Olfactory testing involved olfactometry by Elsberg-Levy modified by Pruszewicz for 4 odorants: coffee, lemon, mint and anise; smell fatigability test by Pruszewicz et al. for lemon. Level of plasma TSH was determined in each subject. Age of the subjects, at which hormonal replacement therapy was started and regularity of the treatment were taken into consideration. Obtained results: impaired odorant identification and shortened smell fatigability period prove a significant impact of thyroid status on central nervous system development most probably already prior to birth and on cognition processes.
