Intracranial administration of transforming growth factor-beta3 increases fat oxidation in rats.

The effects of intracranial transforming growth factor (TGF)-beta3 on spontaneous motor activity and energy metabolism were examined in rats. After injection of TGF-beta3 into the cisterna magna of the rat, spontaneous motor activity decreased significantly for 1 h. The intracranial injection of TGF-beta3 produced an immediate decrease in respiratory exchange ratio (RER). No significant changes were observed in energy expenditure. TGF-beta3 induced a significant increase in total fat oxidation and a decrease in total carbohydrate oxidation. Furthermore, the serum substrates associated with fat metabolism were significantly altered in rats injected with TGF-beta3. Both lipoprotein lipase activity in skeletal muscle and the concentration of serum ketone bodies increased, suggesting that the increase in fat oxidation caused by TGF-beta3 may have occurred in the liver and muscle. Intracranial injection of TGF-beta3 appeared to evoke a switch in the energy substrates accessed in energy expenditure. These results suggest that the release of TGF-beta3 in the brain by exercise is a signal for regulating energy consumption.

Short term locum tenens for rural practice: a trial of a Japanese medical school.

This study investigated locum tenens dispatch to rural practices organised by a medical school. Locum tenens has not been studied previously in rural general practice, and little has been reported about locum dispatch from an academic institution. Since 1990, Jichi Medical School (JMS) has sent faculty members as locum tenens to rural practices where its graduates have been working. The details of all locum dispatches between 1990 and 1996 were reviewed and analyzed. Locum dispatches were performed 486 times for a total of 1233 days from April 1990 to March 1996. Locum relief was supplied to 15 group practices and 42 solo practices which are located throughout most of Japan. Most locum reliefs (98.1%) were short and less than one week. There was a wide variety of reasons for requesting a locum, ranging from official to personal. The three main reasons were practice support (30.5%), continuing medical education (13.8%) and physician absence (13.8%). These reasons were different between solo and group practices. Solo practices had a more diverse range of inquiries than group practices. In group practices, practice support amounted to over half of the inquiry reasons. Rural practices face a wide range of circumstances which necessitate locum dispatch. Organised locum dispatch by a medical school can provide practical support of rural practices through locum tenens dispatch.

Tuning of Spin Density Wave Strengths in Quasi-One-Dimensional Halogen-Bridged Ni(III) Complexes with Strong Electron Correlations, [Ni(III)(chxn)(2)X]Y(2).

A series of quasi-one-dimensional halogen-bridged Ni(III) complexes, [Ni(chxn)(2)X]Y(2) (chxn = 1R,2R-diaminocyclohexane; X = Cl, Br, and mixed halides; Y = Cl, Br, mixed halides, NO(3), BF(4), and ClO(4)) have been synthesized in order to investigate the effect of the bridging halogens and counteranions on their crystal, electronic structures, and moreover the spin density wave strengths. In the crystal structures, the [Ni(chxn)(2)] moieties are symmetrically bridged by halogen ions, forming linear-chain Ni(III)-X-Ni(III) structures. The hydrogen bonds between the aminohydrogens of chxn and the counteranions are constructed not only along the chains but also over the chains, forming the two-dimensional hydrogen-bond networks. While the Ni(III)-X-Ni(III) distances or b axes are almost constant in the compounds with the same bridging halogens, the c axes which correspond to the interchain distances in the directions of the interchain hydrogen bonds are remarkably lengthened with the increase of the ionic radius of the counterions; X < NO(3) < BF(4) < ClO(4). These compounds show the very strong antiferromagnetic interactions among spins on Ni 3d(z)2 orbitals through the superexchange mechanisms via the bridging halogen ions. Judging from the results of X-ray photoelectron spectra (XPS), Auger spectra, and single-crystal reflectance spectra, these Ni compounds are not Mott-insulators but charge-transfer-insulators. Their electronic structures or the spin density wave strengths are found to be tuned by the combinations of the counteranions and the bridging halogens.

Venous thromboembolism in earthquake victims.

Sudden death from pulmonary thromboembolism following an earthquake has not been reported to date. This is a published case report of a 48-year-old woman who died suddenly of a pulmonary embolism on October 28, 2004. She lived in the Chuetu District in Japan, where a big epicentral earthquake occurred on October 23. She evacuated her house with her family and spent 4 of 5 nights in a private car. Until November 1, 8 persons who had been living out of their cars after evacuation from their houses in the District were treated for pulmonary thromboembolism. The immobility that comes with living in a car should be recognized as a risk factor for venous thromboembolism. In addition, potential preventive measures should be recommended, including provisions for emergency shelters, access to other temporary housing, public education, and the use of compression stockings.
