A prospective randomised trial of different doses of intravitreal triamcinolone for diabetic macular oedema.

OBJECTIVE: To compare the safety and efficacy of different doses of intravitreal triamcinolone (ivTA) in treating clinically significant diabetic macular oedema (CSMO). METHODS: 63 eyes of 63 patients with CSMO and central foveal thickness (CFT) of > or =250 microm on optical coherence tomography were randomised to receive 4 mg (n = 23), 6 mg (n = 20) or 8 mg (n = 20) ivTA. Patients were followed up for 6 months, and changes in best-corrected visual acuity (BCVA), optical coherence tomography CFT, standardised change in macular thickness (SCMT), and side effects such as intraocular pressure and cataractogenesis were compared between the three groups. RESULTS: After ivTA injection, improvements of BCVA and CFT occurred in all groups. The mean BCVA improvement at 6 months was significantly higher for the 8 mg group compared with the 4 mg group, with 9.9 and 3.1 improvement in letters on the Early Treatment of Diabetic Retinopathy Study chart, respectively (p = 0.047). The mean SCMT at 6 months for the 4, 6 and 8 mg groups was 28.7%, 42.3% and 60.5%, respectively (p = 0.06). The proportion of eyes with SCMT > or =75% at 6 months was higher in the 8 mg group, but the difference failed to reach significance (p = 0.06). Ocular hypertensive responses (>21 mm Hg) occurred in 39%, 30% and 55% of eyes in the 4, 6, and 8 mg groups, respectively (p = 0.27). CONCLUSIONS: Higher doses of ivTA may prolong the duration of visual benefit in diabetic CSMO and seemed to result in more sustained reduction in macular oedema. Further studies are warranted to investigate the optimum dose of ivTA in treating diabetic CSMO.

Use of intravitreal triamcinolone acetonide injection in unilateral idiopathic juxtafoveal telangiectasis.

Idiopathic juxtafoveal telangiectasis (IJT) is one of the primary retinal telangiectasia that can result in visual impairment attributed to resultant cystoid macular oedema and rarely, choroidal neovascularization. The authors herein report successful treatment using intravitreal triamcinolone acetonide in a case of unilateral IJT that did not respond to prior laser photocoagulation. Serial optical coherence tomography was used to monitor the effectiveness of intravitreal triamcinolone acetonide in the treatment of IJT.

Photodynamic therapy with verteporfin for symptomatic polypoidal choroidal vasculopathy: one-year results of a prospective case series.

OBJECTIVE: To determine the efficacy of photodynamic therapy (PDT) with verteporfin as a treatment for symptomatic polypoidal choroidal vasculopathy (PCV). DESIGN: Prospective consecutive, 2-centered, noncomparative interventional case series. PARTICIPANTS: Twenty-one Asian patients with 22 eyes presenting with serosanguinous maculopathy due to PCV and an initial best-corrected visual acuity (BCVA) of 20/40 or worse were recruited prospectively. All patients had angiographic leakage seen on fluorescein angiograms (FAs) and features of PCV seen with indocyanine green (ICG) angiography. METHODS: Intravenous infusion of verteporfin at a dose of 6 mg/m(2) of body surface area over 10 minutes was administered. Five minutes after the completion of infusion, a 689-nm laser was applied for 83 seconds, with a light dose of 50 J/cm(2). The laser spot size was chosen to cover the polyps and the surrounding abnormally dilated choroidal vessels shown on ICG angiography plus an extra 1000-microm margin. Photodynamic therapy retreatment was performed if leakage from the polyps was found on both repeat FAs and ICG angiography at regular 3-month follow-up intervals. MAIN OUTCOME MEASURES: The proportion of eyes with stable or improved vision at a 1-year follow-up. Secondary outcome measures included change in mean BCVA and the changes in clinical and angiographic features in FAs and ICG angiography. The total number of PDT sessions and any complications were also recorded. RESULTS: Stable or improved vision was achieved in 21 (95%) of the 22 eyes at the 1-year follow-up. Ten (45%) eyes had a moderate gain in vision (improved by > or =3 lines), whereas 1 (5%) eye suffered a moderate visual loss (decrease by > or =3 lines). The mean BCVA improved from a logarithm of the minimum angle of resolution(logMAR) of 0.73 to 0.60, an equivalent of 1.3 lines of improvement. The change in logMAR BCVA at 12 months was statistically significant (Wilcoxon signed-ranks test, P = 0.009). Complete absence of leakage in FAs and total regression of the polyps in ICG angiography were observed in 20 (91%) and 21 (95%) eyes, respectively. Severe loss of vision due to massive subretinal hemorrhage occurred in 1 eye; otherwise, there were no other serious treatment-related adverse events. CONCLUSIONS: The 1-year results of PDT in treating PCV of the macular type with serosanguinous presentations are encouraging. Further studies with longer follow-up and randomized controlled trials are warranted to assess the long-term safety and efficacy of PDT relative to observation or other treatment modalities.

Intravitreal triamcinolone for diabetic macular oedema in Chinese patients: six-month prospective longitudinal pilot study.

AIM: To evaluate the safety and efficacy of intravitreal triamcinolone (IVTA) in Chinese patients with diabetic clinical significant macular oedema (CSMO). METHODS: Eighteen eyes of 17 consecutive patients with CSMO were prospectively recruited and treated with a 4 mg injection of IVTA. Best-corrected visual acuity (BCVA) on the ETDRS chart and central macular thickness (CMT) on optical coherence tomography were measured at baseline weeks 1, 2, 3, and months 1, 2, 3, 4, 5 and 6. Side-effects were monitored. RESULTS: All patients completed 6 months of follow up. The mean baseline BCVA and CMT were 1.20 +/- 0.31 logMAR units and 552 +/- 179 microm, respectively. Improvements in CMT and BCVA were observed as early as at 1 and 2 weeks, respectively (P < 0.05). Mean BCVA peaked at 2 months (0.97 +/- 0.38 logMAR units) while mean CMT was maximally reduced at 3 months (326 +/- 145 microm). Improvements in BCVA and CMT were less afterwards but still statistically significant at 6 months; the final mean BCVA and CMT were 0.99 +/- 0.36 logMAR units and 427 +/- 145 microm, respectively. A total of 5/18 (28%) eyes developed a transient increase in intraocular pressure. Cataract progression was noted in 2/12 (17%) of the phakic eyes. CONCLUSIONS: Intravitreal triamcinolone appeared generally safe and effective in Chinese patients with CSMO. Although the improvements in BCVA and CMT were transient, there were residual benefits at 6 months. Due to the transient nature of IVTA, re-treatment seems necessary but the optimal timing and dosage will require further investigations.

Photodynamic therapy with verteporfin in laser-induced choroidal neovascularization.

PURPOSE: To report a case of laser-induced choroidal neovascularization (CNV) treated with verteporfin photodynamic therapy. DESIGN: Interventional case report. METHODS: A patient developed CNV in the macular area of the left eye 2 years following grid laser for macular edema secondary to branch retinal vein occlusion. RESULTS: The CNV was controlled with one session of photodynamic therapy. Lesion decreased in size, contracting to a smaller subretinal fibrotic scar. Vision improved from 20/200 to 20/100, and there was no recurrence at 2-year follow-up. No complications were encountered. CONCLUSION: Laser-induced CNV can be safely and effectively treated by photodynamic therapy.

Photodynamic therapy with verteporfin for subfoveal idiopathic choroidal neovascularization: one-year results from a prospective case series.

OBJECTIVE: To study the safety and efficacy of photodynamic therapy (PDT) with verteporfin in treating subfoveal choroidal neovascularization (CNV) of idiopathic cause. DESIGN: A prospective, noncomparative, consecutive, open-label, two-center, interventional case series. PARTICIPANTS: Patients aged 50 years or younger with the diagnosis of idiopathic CNV by exclusion of other related causes together with the CNV extending under the geometric center of the foveal avascular zone. METHODS: Seventeen eyes from 17 patients received PDT with verteporfin according to a standardized protocol. Retreatment was considered if fluorescein leakage from the CNV was shown in fluorescein angiography at every 3-month follow-up. The visual, clinical, and angiographic responses were observed. Complications from treatment were monitored and documented. MAIN OUTCOMES MEASURES: The changes in the best-corrected visual acuity (BCVA) at the 1-year follow-up. The proportion of patients with improved (gained 2 lines or more), stable, and dropped (lost 2 lines or more) vision and the mean number of treatments required during the 1-year interval were measured. Complications were monitored and tabulated. RESULTS: Seventeen eyes with complete 1-year follow-up were analyzed. The mean logarithm of the minimum angle of resolution BCVA improved from 0.64 to 0.41 (Wilcoxon signed-ranks test, P = 0.007). Sixteen eyes (94%) had stable or improved vision. Only one patient (6%) had moderate loss of vision. The mean number of PDT treatment was 1.8 per eye over the 1-year period. Patients with smaller lesion size or active vascular CNV on presentation were more likely to have a final BCVA of 20/100 or better (Fisher's exact test, P = 0.006 and P = 0.015, respectively). No serious local or systemic complications were encountered. CONCLUSIONS: PDT seems to be a promising treatment strategy in achieving a stable or improved vision for subfoveal idiopathic CNV. The treatment was well tolerated. Further studies and longer follow-up are warranted to assess the long-term safety and efficacy of PDT compared with observation or other treatment modalities.

Pars plana vitrectomy in the management of retained intravitreal lens fragments after cataract surgery.

PURPOSE: To assess the outcome of patients who underwent pars plana vitrectomy for retained lens fragments after cataract surgery. METHODS: A retrospective study of all consecutive cases with pars plana vitrectomy performed for retained lens fragment was conducted. Twenty-seven eyes of 27 patients were included in the study. RESULTS: Twenty-four (89.9%) eyes received phacoemulsification. Pars plana vitrectomy was performed at the same sitting, or ranged from day 1-70 after cataract surgery. The mean follow up was 31.1 months. Final visual acuity of 6/12 or better was achieved in 15 (55.6%) patients. After excluding patients with pre-existing eye diseases, 68.4% of patients had visual acuity 6/12 or better. Complications after pars plana vitrectomy included glaucoma (22.2%), retinal detachment (11.1%) and surgically induced necrotizing scleritis (3.7%). CONCLUSION: Ocular complications with poor visual outcome can occur after removal of intravitreal retained lens fragments complicating cataract surgery.

Photodynamic therapy in macular diseases of asian populations: when East meets West.

PURPOSE: We reviewed the indications, safety, and efficacy of photodynamic therapy with verteporfin in various macular diseases and vasculopathies, which are common in Asian populations, and compared the outcomes of photodynamic therapy in Asian patients with the outcomes in Caucasian patients. METHODS: Relevant clinical and laboratory original articles, case reports, and review articles that have been published in the literature between January 1999 and October 2004 were searched in Medline. The potential differences in the response to photodynamic therapy between Asian and Caucasian patients were evaluated. Articles in foreign languages with English abstracts were included. RESULTS: Macular diseases commonly seen in Asian populations, including choroidal neovascularization (CNV) of age-related macular degeneration, secondary to pathologic myopia or from an idiopathic cause, and choroidal vasculopathies such as central serous chorioretinopathy and polypoidal choroidal vasculopathy were included in the review. The results were tabulated and the differences with Caucasian populations were compared and highlighted. CONCLUSION: Photodynamic therapy has been found to be an effective and noninvasive treatment for various subfoveal CNV and choroidal vasculopathies of the macula. Diverse behavior in different ethnic groups is observed.
