Perspectives in palliative care.

OBJECTIVES: To review the evolution of palliative care in the United States and to define the terms palliative, supportive, and end-of-life care. DATA SOURCES: Published articles, textbooks, and reports. CONCLUSION: The health care demands that will arise from our aging society will force change in the field of palliative care. IMPLICATIONS FOR NURSING PRACTICE: Nurses providing care to patients who are living and dying from advanced cancer should apply palliative interventions from diagnosis until death.

Applying evidence to palliative care.

OBJECTIVES: To discuss the evidence-based practice process for palliative care, from defining a clinical problem to implementation and evaluation of an evidence-based practice change. DATA SOURCES: Professional experience, literature, practice guidelines, and web sites related to evidence-based practice and palliative care. CONCLUSION: Nurses caring for patients needing palliative care need evidence from multiple sources. Using a systematic process to find and appraise evidence followed by strategically planning for implementation and evaluation of evidence-based practice changes optimizes the likelihood of enhanced patient care. IMPLICATIONS FOR NURSING PRACTICE: The integration of evidence-based practices into palliative care nursing will empower nurses to promote optimal patient outcomes and decrease practice variability.

The palliative care advanced practice nurse.

The emerging role of the advanced practice nurse (APN) as a palliative care provider is gaining increased recognition in the United States. The APN's ability to perform comprehensive physical evaluations, order and interpret diagnostics, and prescribe appropriate medications while receiving reimbursement allow this provider to become a valuable and important discipline in the patient's plan of care. APNs consistently demonstrate an ongoing commitment to the care of the frail, poor, culturally diverse, nursing home, and rural populations-areas in which palliative care is often unavailable. This paper provides an overview of the APN's role and the ability to integrate palliative interventions into traditional health care settings. It highlights reimbursement issues and describes a case example to demonstrate how the APN can provide continuity, coordination, and reimbursement of services for the patient living with and dying from advanced illness.

Why there is no cookbook approach to palliative care: implications of the P450 enzyme system.

A plethora of literature describes the impact of the P450 enzyme system, but this information is limited regarding its relevancy to nursing practice. However, oncology nurses providing palliative symptom management must have a working knowledge of the P450 enzyme system to recognize the variability that exists among individual medication reactions or why a "cookbook approach" to symptom management is not always effective and appropriate. This article describes the variations associated with medication metabolism with reference to ethnic differences. Having a basic understanding of the P450 enzyme system and, more specifically, the CYP2D6 influence on the metabolism of common medications used in palliative symptom management can help to prevent medication toxicity or underdosing, which interferes with patients' quality of life.
