Bcl-2 expression mediated by Cre/loxP system in olfactory epithelium.

To study the Bcl-2 expression mediated by the Cre/loxP recombination system and its effect on prevention of apoptosis in olfactory epithelium. Adenoviral vectors with cassette for Bcl-2 (AxCALNLBcl-2) and Cre recombinase (AxCANCre) were applied to mouse olfactory epithelium by intranasal instillation. The effect of exogenous Bcl-2 expression on prevention of apoptosis of olfactory receptor neurons was investigated using an apoptosis model induced by bulbectomy. The Bcl-2 product was expressed not only in the olfactory receptor neurons but also in the supporting cells. Although statistical analysis did not show significant difference, the number of apoptotic cells in the infected olfactory epithelium on post-bulbectomy day 2 was lower than that of control and the number of survived mature olfactory receptor neurons in the infected olfactory epithelium on post-bulbectomy day 5 was higher than that of control. Although further studies are required for clinical application, the results of our study suggest that this strategy may be able to deliver exogenous Bcl-2 for the treatment of degeneration of olfactory receptor neurons.

[Off-pump coronary artery bypass via median sternotomy].

Off-pump coronary artery bypass grafting (off-pump CABG: OPCAB) is a useful technique of coronary revascularization in terms of reduction of operative mortality and morbidity. Because the biggest advantage of OPCAB may be that it can prevent perioperative stroke, we selected patients for OPCAB based on the preoperative evaluation of neck and intracranial vessels. We could totally eliminate intraoperative stroke in patients undergoing OPCAB, although some patients with a severe neck vessel disease developed postoperative stroke/transient ischemic attack in an early postoperative period (the 4th approximately 8th postoperative day) mostly due to thrombosis from the diseased vessel. Therefore, high-risk patients with a severe neck vessel disease should be treated with a more aggressive anticoagulation protocol postoperatively. As for the quality of grafting, the number of graft, the rate of complete revascularization, and early graft patency were comparable between OPCAB and CABG with a cardiopulmonary bypass. The long-term results in terms of freedom from cardiac death and cardiac events were also comparable. We conclude that we could achieve less invasiveness in coronary revascularization by using an OPCAB technique without compromising the quality of grafting.

Does off-pump coronary artery bypass grafting beneficially affect renal function?

BACKGROUND: Off-pump coronary artery bypass grafting (CABG) has been reported to beneficially affect renal function, but this remains to be confirmed. The purpose of the present paper was to study the effects of off-pump CABG on renal function and analyse predictors of postoperative renal impairment in patients who received off-pump CABG. METHODS: A total of 451 patients who underwent isolated CABG between January 1999 and August 2003 were retrospectively studied. No patient was receiving dialysis. A total of 300 patients (228 men) underwent off-pump CABG (off-pump group) and 151 patients (104 men) underwent on-pump CABG (on-pump group). Perioperative serum creatinine levels and creatinine ratios (peak postoperative creatinine level/preoperative creatinine level) were compared between the groups. RESULTS: Renal impairment (serum creatinine >1.5 mg/dL) developed postoperatively in 12.7% of the off-pump group and 18.5% of the on-pump group (P = 0.1). The creatinine ratio was significantly lower in the off-pump group (1.2 +/- 0.4) than in the on-pump group (1.4 +/- 0.7, P = 0.003). Logistic regression analysis demonstrated that the strongest predictors of postoperative renal impairment in off-pump CABG were left ventricular dysfunction (odds ratio 10.8) and multivessel grafting (odds ratio 4.3). CONCLUSIONS: Off-pump CABG provides better renal protection than on-pump CABG. However, perioperative renal function should be closely monitored in patients who have left ventricular dysfunction or who undergo multivessel grafting, even when off-pump CABG is performed.

Adenovirus-mediated gene transfer in olfactory epithelium and olfactory bulb: a long-term study.

OBJECTIVES: We sought to study the spatiotemporal gene expression mediated by adenoviral vector in the olfactory pathways. METHODS: The replication-defective adenoviral vector AxCALacZ, which encodes the enzyme Escherichia coli beta-galactosidase, was applied to mouse olfactory epithelium by intranasal instillation. RESULTS: The LacZ gene product, beta-galactosidase, was expressed not only in the olfactory receptor neurons and their axons, but also in the olfactory bulbs. The first evidence of anterograde labeling was observed at postinfection day (PID) 2. At PID 3, beta-galactosidase was strongly expressed in olfactory nerve axons, as well as their terminal glomeruli, in the olfactory bulbs. beta-Galactosidase expression persisted up to PID 90, and there was a significant decrease in the number of labeled neurons at PID 30. CONCLUSIONS: These results suggest possible long-term effects of adenovirus-mediated gene transfer on the olfactory neurons, as well as the olfactory bulbs.

Notch expression in developing olfactory neuroepithelium.

Notch genes encode receptors for signaling pathways that regulate neurogenesis in various tissues. To better understand the roles of Notch genes in olfactory neurogenesis, we studied the expression of Notch family in developing mouse olfactory epithelium. In the earliest stage of olfactory development, Notch1 was observed in the mesenchyme lateral to the olfactory placode. During the developmental stage, Notch1 was mainly observed around the basal membrane, while Notch3 was observed in the lower compartment of the olfactory neuroepithelium. Notch2 was not detected during the entire observation period. As the olfactory neuroepithelium grew mature, both Notch1 and Notch3 gradually disappeared. These results suggest distinct roles of Notch1 and Notch3 in the neurogenesis of the peripheral olfactory system.

Quantitative analysis of expression of NeuroD, GAP43 and receptor tyrosine kinase B in developing mouse olfactory neuroepithelium.

OBJECTIVE: The mammalian olfactory neuroepithelium (OE), which harbors olfactory receptor neurons (ORNs), has the unusual characteristic of continuous neurogenesis throughout its lifetime. This unique feature provides an excellent model for neuronal differentiation. Recently, we found dual-phase expression of NeuroD, a member of the basic helix-loop-helix transcription factor family, in the developing mouse OE, suggesting multiple roles of NeuroD during the development of mammalian ORNs. MATERIAL AND METHODS: In order to better understand the molecular mechanism of the development of ORNs, we performed quantitative analysis of expression of NeuroD, GAP43 and receptor tyrosine kinase B (TrkB), as well as 5-bromo-2'-deoxyuridine (BrdU)-labeled cells, in the developing mouse OE from gestational Day 10 to postnatal Day 28. RESULTS: During the embryonic period, NeuroD expression is mostly confined to the basal compartment. During the neonatal period, NeuroD expression is detected in two compartments: the middle compartment and the basal compartment. GAP43-expressing cells were located between these two NeuroD-positive layers. TrkB-expressing cells were located above the NeuroD-positive layer in the middle compartment. As the mice grew, the numbers of NeuroD-expressing cells and BrdU-labeled cells in the basal compartment significantly decreased, while the number of NeuroD-expressing cells in the middle compartment gradually increased. The number of TrkB-expressing cells dramatically increased. The number of GAP43-expressing cells also gradually increased. However, the relative proportion of GAP43 cells decreased as the OE developed. CONCLUSION: NeuroD is a useful molecular marker for studying olfactory neurogenesis.

Synovial sarcoma of the neck.

BACKGROUND: Synovial sarcoma is a soft tissue sarcoma rarely seen in the head and neck. Due to its rarity and morphologic variations, diagnosis is difficult in most cases. METHOD: A case of synovial sarcoma arising in the upper neck is presented. The detection of the specific chromosomal translocation t(X;18)(p11.2;q11.2) is also described. RESULT: A specific chimeric gene, the SSX-SYT fusion gene, was identified in the formalin-fixed paraffin-embedded surgical specimen using the reverse-transcription polymerase chain reaction (RT-PCR) technique. CONCLUSION: Synovial sarcoma contains a characteristic chromosomal translocation, which serves as a useful diagnostic tool. RT-PCR technique has enabled to detect this specific translocation not only in fresh tissues but also in archival paraffin-embedded specimens.

Median sternotomy and extended left anterior thoracotomy for repair of traumatic aortic transection with ruptured right atrium.

A 22-year-old man presented with traumatic aortic transtion associated with rupture of the right atrium and underwent urgent median sternotomy to repair the right atrium. A T-shaped extended left anterior thoracotomy was performed, and ruptured descending thoracic aorta was repaired under total bypass. A Y-shaped connector was inserted in the arterial catheter to allow cannulation of both ascending aorta and femoral arteries. A 4-cm long Hemoshield graft was used to repair the aortic transection. The patient made a full recovery and was discharged 13 days after the accident.

Hemodiafiltration during cardiac surgery in patients on chronic hemodialysis.

BACKGROUND: We have developed a hemodiafiltration (HDF) protocol used during cardiac surgery to preserve fluid and electrolyte balance and prevent postoperative bleeding in patients on chronic hemodialysis. This retrospective study examined the operative results associated with our new protocol. METHODS: The study included 33 consecutive patients on long-term hemodialysis who underwent cardiac surgery at our hospital between January 2001 and April 2005, including off-pump coronary artery bypass grafting (CABG) in 19 patients. Vascular access was achieved via a 12-French double-lumen catheter inserted into the left femoral vein under general anesthesia, and HDF begun when the operation was started. After completion of cardiopulmonary bypass or, in patients who underwent off-pump CABG, after the distal anastomoses were completed, HDF was continued until target hematocrit between 30% and 35%, central venous pressure between 3 and 5 mmHg, and serum potassium concentration between 3.0 and 3.5 mEq/L were reached. The chest was closed after confirmation of hemostasis. RESULTS: There was no in-hospital death. Three patients were extubated in the operating room. There were no postoperative wound infection, mediastinitis, respiratory tract infection, or hemorrhage. The patients were discharged at a mean of 15.6 days after operation. CONCLUSIONS: These results suggest that intraoperative HDF lowers postoperative morbidity and mortality in chronic dialysis patients. Other advantages include early extubation and ambulation, and a shortened hospitalization.
