The activity of N-acetyl-beta-D-glucosaminidase and tumor necrosis factor-alpha at early stage of diabetic retinopathy development in type 1 diabetes mellitus children.

OBJECTIVE: The assessment of the clinical significance of TNF-alpha, IL-10 and NAG with its A and B isoforms concentrations in children with DM type 1 for the detection of early stages of both diabetic retinopathy and nephropathy. PATIENTS AND METHODS: One hundred and two children with DM type 1 and 35 healthy controls were analyzed. Levels of TNF-alpha, IL10 and total NAG enzyme activity with its A and B isoforms were measured in serum and urine of all participants. RESULTS: Children with diabetic retinopathy had a significantly higher levels of TNFalpha in serum (P=0.01) in comparison to those without retinopathy. The activity of NAG (P=0.002) and its isoform A (P=0.006) and isoform B (P=0.001) were significantly higher in children with diabetic retinopathy in comparison to those without this complication. Conversely, within the group with retinopathy, more children had detectable concentrations of IL10 in serum as compared to those without retinopathy (P=0.01). CONCLUSIONS: These results suggest that NAG activity and TNF-alpha concentration in diabetic retinopathy patients might show a relationship with a degree of renal glomeruli epithelial cells and renal proximal tubules damage.

[Photodynamic therapy with verteporfin in treating the choroidal neovascularisation secondary to multifocal choroiditis].

Apart from AMD and pathologic myopia the posterior uveitis is the most common pathology being the cause of secondary choroidal neovascularisation (CNV). For many years the method of choice in treating those pathologies was the theramal laser photocoagulation with obvious limitations in the cases of subfoveal CNV. The implementation of PDT modality to every day's clinical practice expanded the range of CNV treatment methods. The following case report shows the results of treating the CNV secondary to multifocal choroiditis with PDT. During the 6 months of follow-up period we consider PDT as an effective, safe modality in treating the CNV secondary to inflammation. In the result of single PDT therapy the VA improvement of 4 ETDRS lines and it's stabilization on the level of 20/25 with the decrease of metamorphopsia was gained.

[Vitreoretinal surgery in ocular trauma].

PURPOSE: This retrospective study was undertaken to review the structure of patients that admitted to the hospital during one year, due to the different types of ocular trauma as well as to analyze the rate of the vitreo-retinal procedures being employed to treat those patients. MATERIAL AND METHODS: As many as 1212 consecutive patients (1252 eyes) submitting to the out-patients Department of Ophthalmology of the Medical University Hospital in Gdańsk between the 1st of January and the 30th of December 2004, presenting with eye injury were evaluated. The majority of the patients were males (746) with the mean age of 46.8 years. The females in the number of 466 were 48,5 years old in average. In 40 cases of ocular trauma both eyes were affected. The routine ophthalmologic examination was provided including the anamnesis and collecting the information regarding the circumstances of the injury. The ultrasound (USG) examination, fluorescein angiography (FA) and CT scan was provided when the intraocular foreign body (IOFB), the macular hole or uveal rupture were suspected. THE RESULTS: Most of the patients who admitted to the Clinic were applied the first aid and dismissed. That referred to 1210 of 1252 eyes. In 42 cases patients were hospitalized: 5 eyes received standard treatment while 37 eyes were operated. Twenty five eyes required vitreoretinal surgery - pars plana vitrectomy (PPV) due to the IOFB (6/25), vitreous hemorrhage (2/25), retinal detachment (10/25), dislocation of the lens (4/25) and the uveitis (3/25). CONCLUSIONS: PPV revealed to be the most effective modality used for treating the most severe eye injuries. The analyze included the cases treated in one year time only, as the strategies of treatment tend to change in time as well as the techniques of operation. The time of observation was limited to 12 month additionally in purpose to make comparison easier in the different Polish centers surveys, regarding the treatment of mechanical eye injuries.

[The effect of opsonins on erythrocytes phagocytosis in the vitreous--laboratory research (Part A)].

PURPOSE: We performed experimental investigations in vitro into the opsonization of Rh-positive erythrocytes by IgG anti-RhD antibodies and phagocytosis of erythrocytes in the vitreous body. MATERIAL AND METHODS: The vitreous has been taken from the corpses of young people. Anti-RhD antibodies came from preparations 'Gamma anti-D 150'. 1% Erytrocyte suspension of ccDee phenotype has been applied. Phagocytosis assay was performed in the medium of the vitreous and in Hank's solution. RESULTS: Phagocytosis assay performed in the vitreous and in Hank's solution did not display statistically significant differences. CONCLUSIONS: The investigations in vitro presented here, proved that immunological reactions are really occurring in the vitreous body. They provide a base for the explanation of activity mechanism of anti-RhD antibodies injected intraoculary in cases of hemorrhages into vitreous body.

[The effect of opsonins on erythrocytes' phagocytosis in the vitreous--clinical observations (Part B)].

PURPOSE: We demonstrate clinical material collected during several years, which illustrates the possibilities of immunotherapy in intravitreous hemorrhages. MATERIAL AND METHODS: Immunotherapy was applied in 172 persons, Rh-positive in 174 eyes with intravitreal hemorrhages of a various etiology. The therapy consisted of injecting preparations containing IgG anti RhD antibodies into the vitreous. RESULTS: Visual acuity improvement has been obtained in 77.6% of eyes. CONCLUSIONS: Summing up the effects of immunotherapy of hemorrhages into the vitreous body, it is necessary to stress the simplicity of performance of the procedure, its insignificant invasions and security resulting from the application of natural, human immunoglobulin.

[Serum lipid profile in non-exudative AMD patients treated with locally administrated beta-blockers].

Reduced perfusion in the choroidal vessels has been proposed as the vascular pathogenesis for AMD. As no specific therapy is currently available, the authors decided to evaluate the possible benefits of topically administrated betaxolol in non-exudative AMD patients. 114 randomly selected patients with NE AMD was divided into 2 groups: group I (64 patients) and group II (50 patients). The subjects in group I have been treated with topical 0.5% betaxolol, while in the group II with artificial tears twice daily for one year. None of the patients had received prior topical or systemic treatment lowering IOP. The subjects in both groups had follow-up for 1 year, during which lipoproteins' profile sampling was performed twice at baseline and after 52 weeks. Both, baseline and outcome measures included: complete ophthalmologic examination. The changes in serum lipoproteins levels varied, but the statistical analyze did not show the significant differences.

[Carnitine level in human lens and density of cataract].

PURPOSE: To evaluate whether the level of carnitine in human lens is affected by development of cataract. MATERIAL AND METHODS: The study covers 52 cases of cataract lenses extracted by ECCE. The nucleus of each lens was analyzed according to the level of total and free carnitine. The amount of cataract was determined by the microscopic and ophthalmoscopic examination. Cataracts were divided into 3 groups depending on the advanced degree of cataract. RESULTS AND CONCLUSIONS: It was possible to find statistically significant difference in mean carnitine level of total and free carnitine between lenses with mature cataract and lenses with partial cataract. This level was lower approximately by 30% in the lenses with dense opacification. This could be another proof for protective role of carnitine in development of human cataract and could trigger more researchon the subject of its supplementation in patients in older age.

[Epidemiology of hypertensive retinopathy in young patients after coarctation of the aorta repair].

PURPOSE: Coarctation of the aorta is a cause of childhood hypertension. In some patients hypertension persists after coarctation repair. We evaluated hypertensive retinopathy in patients after coarctation repair, factors correlated with retinopathy, specificity of retinal findings. MATERIAL AND METHODS: 54 patients (mean age of surgery 6 years, mean follow-up time 10 years), were studied. Hypertension was present in all subjects prior to operation. We used ambulatory blood pressure monitoring (ABPM) to follow blood pressure evaluation after repair. Direct ophtalmoscopy was performed in all subjects. RESULTS: Subjects were divided into two groups according to current blood pressure values: group I--normotensive--34 pts, group II--hypertensive--20 pts. Hypertensive angiopathy (I-II K-W) was found in 30% patients (9% in group I and 65% in group II). Subjects with angiopathy were mean 7 years older and had higher blood pressure values (24h mean systolic and load), than those without retinal changes. Patients with and without angiopathy did not differ significantly in preoperative blood pressure values. 81% subjects with angiopathy were hypertensive and 86% patients without retinal changes were normotensive. The most common fundus pathologies were arteriolar tortuosity (35%) and narrowing (28%). CONCLUSIONS: 1. There were no signs of persistent angiopathy in most normotensive patients, even though all of them suffered from hypertension in childhood. 2. In our study hypertensive angiopathy corresponded with present blood pressure values and patient's age. 3. All abnormalities were mild (I-II degree K-W), which also caused some diagnostic difficulties to distinguish between normal and pathological subjects.

[Orbital tumors in the material of Department of Ophthalmology of Medical University of Gdańsk in years 1991-2002].

PURPOSE: Aim of the study was to determine the types and frequency of orbital tumors and to assess the method of treatment relevant to patient, orbital and tumor data. MATERIAL AND METHODS: Current classification of orbital tumors is presented. The retrospective analysis of 127 patients with orbital tumor treated in the Ophthalmology Clinic in Gdańsk in years 1991-2002 was carried out. The diagnosis was made with orbital biopsy in available cases, the clinical findings, imaging studies and histopatological examination. RESULTS: There were 91 cases of primary tumors, 33 of secondary lesions and 3 metastatic neoplasms. The surgical treatment was involved in 87 cases. 14 of them required the synergic action with radiotherapy and 4 with chemotherapy. CONCLUSIONS: The age of the studied group is an important factor affecting relative incidence of orbital tumors. With increasing age, malignant orbital tumors become more common. Orbital lesion may manifest the systemic neoplastic disease.

[Carbofiltratio in neovascular glaucoma].

PURPOSE: We present our own antiglaucoma procedure using glassy carbon fibre. MATERIAL AND METHODS: Our procedure belongs to filtration type of antyglaucoma operations, so we call it carbofiltratio. 32 carbofiltratio procedures were performed. All our patients had neovascular glaucoma. Each patient complained of intensive pain, because of increasing intraocular pressure. RESULTS: The follow-up time was 3 month to 2 years. All patients were periodically tested for intraocular pressure, pain and condition of surgery spot. In most of our cases we achieved to reduce pain and intraocular pressure without hypotensive drugs. 34.4% of our procedure was insufficient. We had to make enucleation or reoperate. All these procedures were performed before 60 days after first operation.

[Vitrectomy with daunorubicin].

PURPOSE: The vitreoretinal re-proliferations (PVR) are the most eminent cause of retinal surgery misfortunes. Daunorubicin has been widely discussed as the antimetabolic drug for PVR treatment for recent 20 years. The aim of the study was to evaluate the usefulness of daunorubicin, applied intravitreally during vitrectomy, as an antiproliferative factor stopping the vitreoretinal re-proliferations in eyes with rhegamatogenous retinal detachment complicated with PVR. MATERIAL AND METHODS: Vitrectomy, with daunorubicin infusion (1 microg/ml), was performed in 6 eyes with rhegamatogenous retinal detachment and PVR. In 6 other cases of retinal detachment, the gentamycine (20 mg/ml) solution was used as a vitrectomy infusion. During the 12 months follow-up, the risk of PVR relapse was evaluated. RESULTS: There was no signs of PVR relapse in the eyes treated with daunorubicin during the 12 months follow-up. As in the group of patients that had no daunorubicin infusion the PVR re-proliferation occurred in 2 of 6 operated eyes. CONCLUSIONS: 12 months follow-up showed that Daunorubicine could be a useful adjuvant drug reducing the rate of re-proliferations in the patients with rhegamatogenous retinal detachment.

[Fixation in patients with anisometropic amblyopia without strabismus].

PURPOSE: To determine the type of fixation in patients with anisometropic amblyopia without strabismus and find correlation between amount of anisometropia and eccentricity of fixation. MATERIAL AND METHODS: 100 patients with anisometropic amblyopia without strabismus were examined. The following data was obtained: visual acuity, type of fixation, refraction error after cycloplegia. Amount of anisometropia was calculated as the difference of spherical equivalents. RESULTS: Only 44% of patients demonstrated central fixation. The amount of amblyopia corresponded to eccentricity of fixation. For most cases (anisometropia up to 6D of SE) the amount of anisometropia did not correlate with eccentricity of fixation. CONCLUSIONS: Simply difference of refraction must not be the only factor affecting development of anisometropic amblyopia.

[Iodine treatment in vitreous degeneration].

PURPOSE: We present our result of topical administration of iodine in group of patients with corpus vitreous degeneration. MATERIAL AND METHODS: Corpus vitreous degeneration was studied in 360 patients (660 eyes), 184 male and 176 female. Treatment last form 6 to 12 month. We administrate iodine 3-4 times a day as an eye drops. The untreated control group consist of 110 person (204 eyes) 80 male and 50 female with similar corpus vitreous degeneration. RESULTS: The results are discussed in relation to our own vitreous degeneration scale. We come to a conclusion that topical administration of iodine reduce corpus vitreous degeneration.

[Ocular and orbital complications of paranasal sinusitis].

PURPOSE: The task of the research was epidemiological and clinical analyses of those patients. MATERIAL AND METHODS: In the period of 1976-200 147 patients with complications of perinasal sinusitis were treated. As numerous classifications of paranasal sinusitis complications exist, the analysis of the material was based on the modified classification proposed by the authors. The frequency of the complications was presented in 5-years periods. RESULTS: There were 133 patients treated due to ocular and orbital complications. In our material there were 79 patients (59.4%) up to 15 years of age and 54 patients (40.6%) older than 15 years. We noted 47 females (35.3%) and 86 males (64.7%). In evaluated 5-years periods the number of patients treated balanced between 17 and 37. In our material dominated orbital periostitis (palpebral edema)--103 cases (77.4%). Subperiosteal abscessus of the orbit was noted in 21 cases (15.8%), inflammation of the orbital tissue--in 5 cases (3.8%), orbital abscessus in 2 cases (1.5%), other rare complications--in 2 cases (1.5%). CONCLUSIONS: Generally the number of patients with ocular and orbital complications of paranasal sinusitis was changing in 5-years periods, but we did not notice any permanent tendency for increase or decrease. In the last 5 years we noted the increase in the number of complications, which was connected with the increase of the number of patients treated for orbital periostitis. In 2 patients we found rare orbital and ocular complications, that were not included in existing classifications. These were: orbital subperiosteal haematoma and episcleritis.

[Microbiological evaluation of conjunctival sac in Sjögren syndrome patients].

PURPOSE: To evaluate bacterial flora of conjunctival sac in patients with Sjogren syndrome and compare it with the flora of healthy individuals. MATERIAL AND METHODS: Bacteriological test was performed on the swabs from the conjunctival sac of 15 patients with primary Sjögren syndrome (PS), 15 patients with secondary Sjögren syndrome (SS) and 30 healthy individuals, chosen at random. RESULTS: Most of the swabs, in every group, were aseptic. The bacteria found relatively often was Staphylococcus epidermidis or described as the Staphylococcus sp. coagulasonegative. Bacterias: Staphylococcus aureus, Corynebacterium or Enterobacteriaceae were found rarely. CONCLUSIONS: There is no significant difference between bacterial flora of conjunctival sac of patients with PS and SS, when compared to healthy individuals.

[Sandhoff's and Tay-Sachs disease--based on our own cases].

The authors described two infant with Sandhoff's and Tay-Sachs disease. Tay-Sachs disease is well-known inherited disease leading to an accumulation of gangliosides in the brain and retina. Sandhoffs disease (GM2 gangliosidosis type 0) was diagnosed in an infant, in whom a progressive neurological disorder and cherry-red foveal spots were developed. In addition, to the general clinical examination, indirect ophthalnoscopy, blood white cells enzymatic examination are used to make definitive diagnosis. In this cases, the early eye fundus examination allowed us to make the proper diagnosis. The fundus change is characterized by the "cherry-red spot" in the central area.

[Vascular flow in patients with retinal detachment in the color Doppler's ultrasonography].

UNLABELLED: For many years now Doppler's ultrasonography has been broadly applied in ophthalmology. Recently it has been communicated, that there is a possibility of assessing the disorders of retinal circulation in the process of retinal detachment. PURPOSE: Our research had to establish, whether after the scleral buckling treatments executed due to the retinal detachment, there is a change in the blood flow in the vessels of the eyeballs. MATERIAL AND METHODS: The observations were conducted in 30 eyes of 15 people, who were diagnosed with opening-related retinal detachment. The colour Doppler's ultrasonography was performed by using ATL Ultramark 9HDI apparatus with a linear sound of the frequency range from 7.5 to 10 MHz. The anatomic structures of the eye socket in the transverse and arrow plane were assessed. The flow spectrum was measured according to the PD method (pulsating Doppler) in the central artery of the retina at the distance of approximately 2-4 mm from the optic disc and in the ophthalmic artery. The analysis of the spectrum allowed one to determine the values of the pulsation indexes-PI, and resistance indexes-RI, in the ophthalmic artery and in the central artery of the retina of the healthy eye, and of the eye with detached retina 2-3 days before and in 5-7 days after the operative procedure. RESULTS: After radial and circumferential buckling treatments, the conducted research found no increased resistance of the flow in the arteries, namely: the central artery of the retina and the ophthalmic artery. The condition for the maintenance of the correct parameters of the blood flow was the intraocular pressure. At the same time, in the eyes with detached retina, the decrease in the resistance of the flow in the ophthalmic artery was observed, which was being brought back to normal after the operative treatment.

[History of teaching of medicine in Gdańsk].

The work has aimed at presenting the history of medicine in Gdańsk comprising the period from the second half on the 16th century up to the present. The most remote stage of the history was connected with the foundation and development of the Academic Gymnasium of Gdańsk (1558) where medicine, general medical sciences and anatomy were taught. Out of those subjects it was the latter that could boast of great achievements owing to famous anatomists lecturing there. It was already in 1613 that the first human dissection was performed in Gdańsk, the dissection being also the first in Poland and in Northern Europe. It was here that in 1722 was published the famous anatomical atlas of Jan Adam Kulmus - 'Tabulae anatomicae' - translated for seven languages, including Japanese. Quite a number of graduates of the Academic Gymnasium, having later completed their higher medical education abroad, were coming back to Gdańsk and carrying out researches, obtaining excellent results. Among them were: Ludwik von Hammen, Izrael Conradt, Placotomus and others. The Gdańsk physicians - Jan Schmiedt (Fabrytius) and Ernest Scheffer were the authors of the first Polish pharmacopoeia (1665). The period from the 18th century up till now has been characterized by great advances in both medical service and hospital management, their crowning achievement being foundation of the Medical Academy in Gdańsk, the first, after the war, independent medical university in Poland.
