[Diagnostic and prognostic significance of characteristics of serum interferon antiviral activity in chronic and acute diffuse liver diseases].

The interferon (IFN-alpha) level and its antiviral activity were investigated in 236 patients, suffered from the chronic diffuse liver disease (CDLD), including 121 patient with chronic hepatitis (CH), 115 patients with hepatocirrhosis (HC) and 26 patients with acute viral hepatitis (AVH). This permitted to elaborate the pathogenetic and prognostic value of these indices in CH, AVH and HC. The analysis of the variations of IFN-alpha antiviral activity in patients with CDLD in dependence on the etiology, activity of the pathologic process, CH stages by CHild-Pugh graduation, the presence of the portal hypertension and its complications was carried out for the first time. The antiviral activity in the patients suffered from HC with progression of the disease, aggravation of the portal hypertension, development of the decompensation along with persisted elevated IFN-alpha concentration decreased. The main mechanisms modulating IFN-alpha antiviral activity in peripheral blood of the patients suffered from CDLD have been determined. The use of the suggested indices in the combination with other methods of the examining significantly extends the possibilities of the diagnostics, permits to evaluate the reserve possibilities of the organism and to detect the strategy of the immunomodulating treatment. The determination of the IFN-alpha antiviral activity in serum in dynamics is recommended for the control of the therapy of the patients with CDLD and AVH.

[Cytokines in the immunorehabilitation of infectious patients].

For the first time in Russia the authors have defined an infectious process of Astrakhan rickettsiosis hemorrhagic fever attended by hyper- and hypoproduction of interferon g and its aftereffects. Reaferon and gammaferon therapy have proved to be very effective in fast normalisation of the immunology index. Enthalferon pills greatly reduced the morbidity rate in servicemen in rotaviral infections.

[Endogenous regulatory peptides, interferons and cytokines as adaptogens in exposure of the body to adverse environmental factors].

The authors summarize data on the efficacy of various prefabricated recombinant interferons against virus, bacterial, and chlamydial infections, and in unfavorable environments. The spaceflight factors were shown to have a negative influence on natural killers in the human body providing the immunological resistance and defence not only from foreign microorganisms but also the body cells transformed because of a virus infection or malignancy. Entalferon awakes a particular interest for its convenience in extreme conditions, space flight included. It is still an open issue whether the interferon sensitivity of natural killers is compromised by space flight. This trend of investigation seems to be of paramount importance and promise in the context of enhancing the antiviral, antimicrobic and antineoplastic immunity of cosmonauts.

[Study of antiinfectious potential of recombinant tumor necrosis factor-alpha].

Tumor necrosis factor-alpha (TNF) was shown to have an antiinfectious effect on murine and rat models of viral, bacterial and parasitic infections. The effective antiinfectious dose of TNF was determined to be 10(2)-10(3) U/kg body weight. TNF, interferon, and antibiotics were demonstrated to potentiate the antibacterial effect of each. Clinical trials of TNF in the treatment of slowly progressive, latent, and chronic infections and in the emergency prevention of acute infections are advisable.

[Interferon efficacy in treating patients with Astrakhan fever].

Astrakhan fever is a new endemic rickettsiosis belonging to the group of tick-borne spotted fevers. Adjuvant use of interferons in basic treatment of Astrakhan fever elevates its efficacy. The maximal contribution of interferons can be reached at administration of alpha-2 and gamma-interferons in daily doses 15,000-25,000 and 1500-2500 IU/kg, respectively. The response is higher at early administration of the medication. Interferons induce side effects early in the course: intensification of hyperthermia and pains.

[Interactions of interferon with other immunomodulators in the regulation of human natural killer cell activity].

The cytotoxic activity of natural killer (NK) cells isolated from peripheral blood of 20 healthy donors and 34 patients with multiple sclerosis (MS) against labelled with H3-uridine target cells K-562 before and after their 1 hr treatment with reaferon (RF), T-activin (TA), myelopid (MP), opioid preparation dalargin (DL) as well as with combinations of TA, MP and DL with RF was studied in 14 hrs cytotoxic test. It has been shown that combination of RF with TA, MP and DL changed the regulatory action of these peptides on NK cell activity in healthy donors in vitro. The same combination of the preparations in patients with MS caused another changes in regulation of NK activity by them because NK cells in MS patients had had initially changed sensitivity to action of these regulatory polypeptides.

[Use of alpha interferon preparations in acute lymphoblastic leukemia in children during remission].

The Soviet alpha-interferon drugs (human leukocytic interferon, reaferon) were used in the program of the treatment of children with acute lymphoblastic leukemia (during remission). The treatment was found to be more effective with an increase of its duration, relapse-free course of the disease (69.4-82.3%) and survival (70.1-93.1%) of patients with an unfavourable disease prognosis (over 5 years).

[The use of melamine-formaldehyde latex in the automated analysis of immunocompetent cells].

The optimization of the loading of monodispersed melamine-formaldehyde latex with fluorochrome has permitted its use as indicator material, equally suitable both for the visual evaluation of the reaction of phagocytosis and for the automated evaluation of this reaction by means of a scanning fluorescent microscope. The study has shown the possibility of using fluorescent melamine formaldehyde latex for the evaluation of the phagocytosis of mouse peritoneal macrophages and human blood cells, as well as the analysis of the surface receptors of immunocompetent cells.

[An experimental study of human recombinant gamma interferon with a proteolytically reduced C-end area of the protein].

The work presents the results of experimental study of gamma interferon obtained by gene engineering techniques on the basis of Escherichia coli producer strains. The study has revealed that gamma interferon, whose molecular weight is 15 KD, due to intracellular proteolytic degradation shows the absence of some amino acids at the C-end of protein and is electrophoretically homogeneous, while its antiviral, antiproliferative and immunomodulating effects are less pronounced than those of gamma interferon with a molecular weight of 18 KD.

[The results of clinical trials and the prospects of using the Soviet preparation of human recombinant interferon alpha-2 (reaferon) in medical practice].

The results of the clinical trials of human recombinant interferon alpha-2 (reaferon) make it possible to come to the conclusion that the preparation is well-tolerated and produces a pronounced therapeutic effect in a number of viral and oncological diseases. The Pharmacological Committee of the USSR has recommended reaferon for use in acute hepatitis B, hairy cell leukemia, renal cancer at stage IV, disseminated sclerosis, ocular herpes. The use of reaferon has been found to be promising in the treatment of papillomatosis of the larynx, Kaposi's sarcoma, mycosis fungoides, chronic myeloleukemia.

[Recombinant human alfa2-interferon (reaferon) in ointment form in the treatment of various diseases of the skin and mucosa].

Clinical trials of the gene engineered human alpha 2-interferon as ointment were performed. The ointment was used in the treatment of patients with relapsing herpes of various localization, relapsing aphthous stomatitis and pemphigus vulgaris. It was shown that the ointment was harmless and its therapeutic effect was favourable. The preparation is recommended for introduction into the practice of public health.

[Effect of recombinant human alpha-interferon (reaferon) on macrophage functional activity in mice].

The influence of human recombinant alpha 2-interferon (reaferon) on the parameters of the phagocytic activity of mouse peritoneal macrophages (migration, spreading, adhesion and absorption of corpuscular antigen) has been studied. Reaferon in doses of 5-5 X 10(2) I. U./ml has been found to produce a stimulating effect on all parameters under study. The data obtained in this study suggest that a stimulating effect on the functional activity of macrophages is the same for recombinant (alpha 2) interferon and natural alpha-interferon.

[The immunoregulatory properties of reaferon].

The influence of human recombinant alpha-interferon (reaferon) on cell-mediated and humoral immune response has been studied. Experimental facts on the blast transformation of lymphocytes, humoral immune response and the reaction of delayed hypersensitivity are presented. The study has shown that reaferon possesses the main immunoregulatory properties, characteristic of natural human leukocytic alpha-interferon. Manifestation of these properties depends on the dose of preparation and the time of its use.

[Determination of the antiproliferative activity of human recombinant interferon (reaferon) on diploid human cells].

Dependence of the antiproliferative activity of reaferon in cultures of human diploid cells on the drug dose and duration of its action on the cells was studied by counting viable cells in the Goriaev chamber. No relationship was detected. However, such dependence was clearly evident with using tumor cells. With counting of mitoses it was shown that in doses of 10(4) and 10(3) IU reaferon significantly inhibited the cell mitotic activity when the mitotic index in the control of the strain M-19 cells exceeded 25%. When the mitotic index was lower than 25% reaferon in all the doses had no inhibitory effect on the cells. On the contrary it was noted that there was even a certain tendency to stimulation of the mitotic activity. Therefore, the line of the human diploid fibroblast cells (strain M-19) may be used for assay of reaferon antiproliferative activity in case of high mitotic activity in its monolayer (the mitotic index higher than 25%). It was also demonstrated that the dose of reaferon should be not lower than 1000 IU per 1 ml of the medium.
