Gangliocytic paraganglioma of the duodenum: an immunocytochemical study.

Two cases of duodenal paragangioliomas that were studied using light microscopy and immunocytochemical methods are reported. The tumors showed immunoreactivity to somatostatin, cholecystokinin, calcitonin, gastrin, vasoactive intestinal polypeptide, serotonin, met-enkephalin, leu-enkephalin, and substance P. The similarities of duodenal paragangioliomas to other neoplasms occurring in similar anatomic locations are outlined.

Trichosporon hepatitis.

A 33-yr-old Puerto Rican women was hospitalized for chemotherapy and multiple antibiotic treatment for relapse of acute myelomonocytic leukemia. While she was already receiving amphotericin for suspected Aspergillus infection, she developed hepatomegaly and abnormal liver enzymes with high serum bilirubin. The blood cultures were negative. Percutaneous liver biopsy revealed granulomatous fungal hepatitis identified by cultures as Trichosporon cutaneum. In spite of the continued administration of amphotericin, with the addition of 5-fluorocytosine, Trichosporon was later cultured from her blood, and she succumbed to fungemia and polymicrobial sepsis.

Human cryptosporidiosis in the acquired immune deficiency syndrome.

We studied two cases of cryptosporidiosis in patients with acquired immune deficiency syndrome. In one patient, the gallbladder was affected in addition to gastric and intestinal involvements. This is the first human case in which Cryptosporidium has been found in the biliary tract. It is important that pathologists become aware of this protozoan infection since the number of patients with it is likely to increase in the near future.

Neuroendocrine carcinoma of the skin in a young adult.

Neuroendocrine (Merkel cell) carcinoma of the skin is usually a tumor found in elderly individuals. Microscopically, the tumor is a small cell carcinoma that involves primarily the dermis. The dominant ultrastructural features are the presence of short cell processes containing small, dense-core, membrane-bound granules. The tumor is often locally aggressive, has the potential to metastasize to lymph nodes, and eventually causes death. The authors report a case of such a neoplasm that affected a 24-year-old man with congenital ectodermal dysplasia. It was associated with a concurrent basal cell carcinoma.

Immunoperoxidase localization of factor VIII in angiosarcomas.

Factor VIII-related antigen was studied in 28 cases of angiosarcoma from different sites by means of the immunoperoxidase technique. Positive immunostaining was observed in 23 cases. Five cases composed preponderantly of solid areas, with little vasoformative activity, showed negative immunostaining. This technique is of value in diagnosing tumors of endothelial origin.

Nodular regenerative hyperplasia of the liver: report of two cases and review of the literature.

We review the clinical and pathologic features of 39 cases of nodular regenerative hyperplasia of the liver and include two new cases. This is an uncommon but distinct pathological entity characterized by regenerative hepatocytic nodules without fibrosis. Portal hypertension that often leads to hemorrhage from esophageal varices is the most important clinical manifestation. Characteristically, manifestations of hepatic failure are lacking, but a number of patients have elevated alkaline phosphatase of hepatic origin. Felty's syndrome is the entity most often associated with nodular regenerative hyperplasia. Anemia, leukopenia, and thrombocytopenia are frequent hematologic abnormalities secondary to hypersplenism. The etiology and pathogenesis of nodular regenerative hyperplasia of the liver are unknown.
