[Severe and malignant arterial hypertension: the optimal treatment].

54 patients were treated with a combination of capoten and diuretics for 40 to 250 days. Some patients had been subjected to the "nitroprusside test" prior to medication. The relation between the clinical state, the perilimbal and eye fundus microcirculation and degree of BP decrease were studied using fluorescent angiography. An adequate (for this group of patients) effect was observed when blood pressure decreased by 25-30% as compared to the initial level. For this capoten, up to 150 mg per day, is sufficient. However, when the BP during the "nitroprusside test" and capoten "overdosage" fell by 35-40% or more as compared to the basal level, undesirable changes were observed. The angioarchitecture deteriorated considerably due to a significant increase in the number of non-perfused vessels and vast ischemic zones as compared to the initial state.

[Fluorescein angiographic studies of the perilimbal conjunctiva and limbus in hypertension].

Seventeen patients with vegetovascular dystonia with hypertensive reactions, 19 patients with essential hypertension at IB-IIA stages, and 26 hypertensive patients at IIB stage of the disease underwent fluorescein-angiographic study of the perilimbal conjunctiva and limb. The advantage of this method as compared to the biomicroscopic one, is that it allows one to obtain objectively documented information on the state of vessels and velocity of blood flow in them. It was established that by the method of fluorescein-angiography of the perilimbal conjunctiva region and limb there may be detected pathologic changes in the microcirculation of the field under study not only at early stages of essential hypertension but also in vegetovascular dystonia patients with hypertensive reactions, when changes on the fundus of the eye were absent.

[Fluorescein angiographic studies of the fundus oculi in the malignant arterial hypertension syndrome].

Fluorescein-angiographic examination of fundus oculi in 33 patients with the syndrome of malignant hypertension made it possible to recognize for the first time changes in the choriocapillary layer, which in 29 patients were displayed by choroidal retention; ischemic zones were seen in 20 patients. It is suggested that the discovery of ischemic zones in the choriocapillary layer may be an early diagnostic criterion of malignant hypertension.
