Prognostic significance of parafollicular small lymphocytes in follicular lymphoma: clinicopathological studies of 82 cases of primary nodal origin.

Pretreatment biopsy material of 82 cases of follicular lymphoma of lymph node origin was reviewed. According to the amount of small lymphocytes in the parafollicular lymphoid tissue (PSL), tumors were classified: PSL++, abundant lymphocytes with prominent postcapillary venules; PSL+, a number of lymphocytes readily recognizable, postcapillary venules present but not prominent; PSL+/-, lymphocytes barely appreciable or absent. Five-year actuarial survival rates were 82.7% in PSL++, 42.3% in PSL+, and 0% in PSL+/-. Ten-year actuarial survival rates were 51.5% and 4.7% in PSL++ and +, respectively. The PSL++ pattern was often associated with localized disease. Patients surviving more than 5 years with disseminated disease had either PSL++ or +. Survival was better correlated with PSL patterns than histologic tumor types or clinical stages. Transition of the pattern from nodular to diffuse occurred in 17 of 26 cases observed and was concomitant with the progressive loss of parafollicular (or interfollicular) components, namely small lymphocytes and lymphocyte-associated vasculature. Classification of follicular lymphoma based on parafollicular small lymphocytes (PSL) offers a new set of prognostic criteria and a guide to improved clinical management.

A combined treatment approach to management of hepatic metastasis.

Forty-eight patients with liver metastases were treated at Rhode ISland Hospital in a nonrandomized sequential manner between January 1972 and June 1977. Eight received 5 FUDR hepatic artery infusion, 14 hepatic irradiation, and 25 were planned for combined intra-arterial chemotherapy plus total hepatic irradiation. Those patients who successfully completed induction treatments had a median survival in the radiation only group of 140 days, in the intra-arterial chemotherapy group 270 days, and in the combined group 376 days. Hepatic radiation when combined with chemotherapy was well tolerated. Primary tumor site, disease duration, and degree of abnormality of liver function had no relationship to the response to treatment. The pretreatment performance level of the patient as determined by the Karnofsky Performance Index gave the best indication for potential response to combined therapy. Based on the results of this treatment and the reports of other series, it appears that the combination of intra-arterial 5 FUDR plus hepatic irradiation may offer prolonged and worthwhile palliation to appropriately chosen patients.

Preoperative simultaneously administered cis-platinum plus radiation therapy for advanced squamous cell carcinoma of the head and neck.

Synchronously administered cis-platinum (cis-DDP) and radiation therapy have been used to treat unresectable squamous cell carcinomas of the head and neck. The purpose of this study was to evaluate the efficacy and tolerance of preoperative adjuvant cis-DDP plus radiation therapy in operable stage III and IV head and neck cancers. Radiation therapy (4,500 rad) was delivered in 180-rad daily fractions. Cis-DDP (20 mg/M2) was given before radiotherapy on days 1-4 and 21-24. Eighteen patients began therapy; 16 completed the combined regimen. Toxicity included stomatitis and WBC below 2,500/mm3. One patient died from therapy of a cerebrovascular accident. Sixteen patients (89%) achieved a complete or partial response to therapy. Complete responses were observed in 13 of 18 primary tumors (72%), and in all three patients with cervical lymphadenopathy. Complete responses were noted for lesions of the nasopharynx, oral cavity, pharynx, hypopharynx, and larynx, for all histologic grades of squamous cell carcinoma. Twelve patients underwent curative surgery. Site-related morbidity occurred in two patients (15%) and a third patient developed postoperative pneumonia. Five of 10 resected primary tumors with preoperative complete responses were pathologically negative for tumor. Concurrent bolus cis-DDP and radiation therapy are well-tolerated and result in impressive tumor reduction. Morbidity after subsequent curative surgery is low, and histologic complete responses are frequent.(ABSTRACT TRUNCATED AT 250 WORDS)
