[Neureprotective drugs efficiency in patients with chronic tension headaches].

Authors examined 126 patients with chronic tension headaches. Patients were divided in two groups: general and control. The control group received conventional therapy (tizanidin, fluoxetin, vinpocetin, manual acupressure active points of m. trapezius and temporo-mastoideus part of the head); in the group besides standard medicines was used mexidol. Before and after the treatment clinical neurophisiological examination was done, including electroneuromyography. Increasing evidence suggests that combination of basic medications with mexidol is the most effective method for treating TH.

[Correction of hyperlipidemia with Allicor].

The hypocholesterolemic action of long-acting garlic powder tablets Allicor was investigated in a double-blind placebo-controlled study in men with mild hypercholesterolemia. Group 1 received allicor in a daily dose 600 mg, group 2 received placebo. The results show that allicor lowers total cholesterol, LDLP cholesterol, raises HDLP cholesterol and therefore can be recommended for correction of lipid content in patients with moderate hyperlipidemia.

Heterotopic nociceptive EMG-reactions in M. masseter.

Peculiarities of EMG-reactions of m. masseter to noxious homo- and heterotopic stimulation were studied on healthy volunteers. Homotopic noxious electrical stimulation of n. mentalis elicited several successive responses in m. masseter consisting of R-component, early exteroceptive suppression period, early excitation period, late exteroceptive suppression period, and late excitation period. Heterotopic noxious stimulation (forehead, ear lobe, index finger) induced only the late exteroceptive suppression period and late excitation period. It is concluded that the excitatory components have a reflex nature, and that the late exteroceptive suppression period is formed under the effect of central cerebral structures, which mediate their influence via the spinal-corticospinal return loop.

[Non-invasive topical ECG diagnosis of Wolff-Parkinson-White syndrome].

The paper deals with the procedures and techniques of the non-invasive topical ECG diagnosis of the Wolff-Parkinson-White syndrome, which were based on the regression analysis. Based on the analysis of ECG data and the results of intraoperative cardiac mapping in 64 patients operated on for the Wolff-Parkinson-White syndrome, a linear regression model was developed, which precisely localizes the site of the Kent bundle from the measurements of delta-wave amplitude and polarity in 12 standard leads. The site of the Kent bundle was determined by 30 zones of the atrioventricular sulcus. The model represented a linear statistical dependence of the zone number of the atrioventricular sulcus on the delta-wave amplitude. Its parameters were evaluated from the data obtained in the training group of 47 patients and the accuracy of the diagnosis was assessed from the data obtained in 17 controls. The testing of the model showed that the Kent bundle may be localized at an accuracy (0.79 +/- 0.16), on an average, no greater than 1 zone. The estimation error was no more than 1 zone in 88% of the cases and 2 zones in 12%. The proposed model may be widely used in in- and outpatient care, as its application requires no cost-effective equipment and computer facilities.

[Reduced cell sensitivity to glucocorticoid hormones in hypercholesterolemia].

The number of 3H-dexamethasone binding sites in lymphocytes of subjects with hypercholesterolemia (HCS) was found to be decreased as compared to the receptor level in normolipidemic patients (N). In HCS-lymphocytes, the dexamethasone-induced inhibition of 3H-thymidine and 14C-acetate incorporation was less pronounced (by 20% and 22%, respectively) than in control cells, which is suggestive of the decreased sensitivity of HCS-lymphocytes to the hormone. An addition of 5-25% HCS blood sera to human skin fibroblast cultures caused a 10-50% decrease in the number of 3H-dexamethasone binding sites and diminished the Kd values 2-3 times. Lipid-depleted HCS-sera had no effect on the glucocorticoid reception in fibroblasts, whereas very low (VLDL) and low (LDL) density lipoproteins inhibited the 3H-dexamethasone binding to the cells. The most potent inhibiting effect was exerted by VLDL (both N-VLDL and HCS-VLDL). HCS-VLDL were more effective than N-LDL. HCS-HDL and N-HDL did not effect the 3H-dexamethasone binding to fibroblasts. In cells preincubated with VLDL dexamethasone inhibited the incorporation of 3H-thymidine and 14C-acetate less intensively (by 27% and 20%, respectively) than in control fibroblasts. The experimental results are suggestive of a decreased sensitivity of peripheral HCS-cells to glucocorticoids, which may shed some light on the mechanism of hypercholesterolemia realization into coronary heart disease and atherosclerosis.

[Hypoalphacholesterolemia: dynamics of the subfractional spectrum of high density lipoproteins during their interaction with fibroblasts and hepatoma Hep-G2 cell line].

Using gradient gel electrophoresis, the dynamics of subfractional spectrum of high density lipoproteins (HDL) according to the particle size was studied during HDL interaction with hepatoma Hep-G2 cells and human skin fibroblasts. It was found that incubation of sera obtained from normolipidemic donors with cholesterol-loaded fibroblasts results in a decrease of the proportion of all small-sized particles of the HDL3 subclass, i.e., HDL3a, HDL3b and HDL3c as well as in an increase in the proportion of large-sized particles of the HDL2 subclass (HDL2a and HDL2b) due to cholesterol acceptance by HDL. In contrast, incubation of the same sera with hepatoma Hep-G2 cells causes a decrease in the proportion of HDL2b and a release of smaller cholesterol-deficient HDL3a particles. The dynamics of subfractional spectrum of HDL in hypoalphacholesterolemic sera is somewhat different, i.e., incubation with fibroblasts results in a decrease of the proportion of HDL3b and HDL3c; that of HDL2a is increased. The HDL2b fraction is unchanged. After incubation of the same sera with hepatoma Hep-G2 cells, the proportion of HDL2b does not fall as in the case of normolipidemic sera, but shows a marked increase. It is concluded that hypoalphacholesterolemia is characterized not only by a low HDL level in the plasma, but also by the formation of HDL2b-deficient particles which less effectively interact with liver cells.

Interconversions of HDL subfractions in normolipidemic and hypoalphacholesterolemic sera incubated with fibroblasts and hepatoma cell line Hep-G2.

Gradient gel electrophoresis was used to study the changes in the HDL subfractional spectrum according to the particle size during their interaction with hepatoma Hep-G2 cells and human skin fibroblasts. Incubation of normolipidemic sera with Ch-loaded fibroblasts resulted in a drop in small particle (HDL3a' HDL3b, HDL3c) and, a rise in large particle (HDL2b, HDL2a) proportions. In contrast, incubation of the same sera with hepatoma cells caused a decrease in HDL2b proportion and a HDL3a release. Incubation of hypoalphacholesterolemic sera with fibroblasts showed a decrease in HDL3b, HDL3c particles and an increase only in HDL2a. The proportion of HDL2b rose considerably when the sera were incubated with hepatoma cells. It is concluded that not only low HDL sera levels, but also the formation of defective HDL2b particles which interact less effectively with liver cells, are characteristic of hypoalphacholesterolemia.

New functions for the components of glucocorticoid receptor apparatus?

By ammonium sulphate precipitation, phosphocellulose and DEAE-Sephacel chromatography and Sephadex G-75 gel filtration, a factor was separated from rat liver cytosol which was shown to suppress the inhibitory effect of the steroid deoxycorticosterone (DOC) on the specific [3H]glucocorticoid binding to cytoplasmic receptors. By SDS-polyacrylamide gel electrophoresis (SDS-PAGE) of factor-containing fractions its Mr was suggested to be about 40000. The possible role of this factor in the regulation of glucocorticoid receptor apparatus function is discussed.

[Accumulation by preparations of rat heart sarcolemma of protein-bound forms of glucocorticoids].

The accumulation of free and protein-bound forms of [3H]corticosterone and [3H]dexamethasone by preparations of heart sarcolemma from adult male Wistar rats was studied. At physiological concentrations of glucocorticoids the sarcolemma preparations showed a considerably greater accumulation of the cytosol-bound form of glucocorticoids as compared to the free form. On the contrary, the formation of [3H]corticosterone-transcortine complex decreased the steroid uptake by the membranes under these conditions. The [3H]steroid-albumin complexes obtained by incubation of very high concentrations of [3H]glucocorticoids with pure albumin were retained by the membranes better than the free form of the hormones. The differences in the accumulation by membranes of steroid--cytosol and steroid--albumin complexes during temperature rise from 0 degrees to 20 degrees C were found. The increase in the uptake of steroid--cytosol complexes was evidently caused by thermal "activation" of the complexes, while the temperature influence on the steroid--albumin complexes accumulation by sarcolemma preparations was directly due to the uptake process. An addition of molybdate (blocker of steroid--receptor complexes activation) at a concentration of 100 mM completely eliminated the thermal effect on the steroid--cytosol complexes accumulation.

[Activation of glucocorticoid cytoreceptors in rat heart].

Accumulation of 3H-dexamethasone by the nuclei in a cell-free system was studied. The increase in temperature from 0 degrees to 20 degrees C and treatment of cytosol by KCl (0.4 M) or theophylline (10mM) significantly increased the absorption the bound hormone by the nuclear fraction. Activation of the steroid-protein complex induced by temperature and addition of theophylline did not change its size. The increase in the ionic strength decreased the Stokes radius from 53 A down to 39.5 A and the sedimentation coefficient value from 7S down to 4S. It is concluded that the heart tissue cytoplasm contains a glucocorticoid receptor.

[Clinical significance of histological types of prostatic cancer].

Biopsy material obtained from 310 patients with prostatic cancer (PC) was used in the study of the incidence rate of different PC histological types, of tumor cell immunophenotype and differentiation by Glison. The results of the study have shown that definition of the tumor histological type and tumor immunophenotype facilitates identification of tumors with the most malignant potential. Therefore, this definition should be included into the diagnostic complex in the study of PC. Choice of treatment policy should not be based only on differentiation of the tumor process by Glison. The decision should be made only after establishment of histogenesis and immunophenotype of tumor cells. The presence of neuroendocrine differentiation is an unfavourable prognostic factor.

[Kidney carcinosarcoma].

A kidney tumour in a 36 year old female is described. The tumor was soft, of gray-brown color, size 12 x 11 cm. Histological, immunohistochemical and ultrastructural findings have demonstrated the signs of biphasic epithelial and mesenchymal cell differentiation. These data allowed the authors to define this tumor as a renal carcinosarcoma. Histogenesis of this tumor and differential diagnosis with other malignant renal tumors are discussed.
