Hemoperfusion effect on oxygen-carrying function of preserved donor blood and erythrocytic mass.

The purpose of the study was to explore the preserved (7-21 days) donor blood and erythrocytic mass, following perfusion through hemosorbent SKN-D. After the sorption the oxygen-transport function of erythrocytes was found to improve considerably (oxyhemoglobin dissociation curve was shifted to the right on average by 4-4.2 mm Hg). The energy supply became better (ATP level was increased by 38-59%). In view of the above, long-preserved blood and erythrocytic mass following their perfusion through SKN-D hemosorbent can be considered a sufficiently valid transfusion medium.

[Characteristics of the changes in the concentration of 2,3-diphosphoglycerate in heart surgery patients].

Changes in 2,3-DPG level have been studied in cardiosurgical patients. It has been shown that 2,3-DPG concentration increase, as a moderately fast mechanism in a common model of tissue pO2 regulation, is observed in almost half of patients with acquired heart valve defects. An increase in 2,3-DPG level and P50 value leading to a decrease in hemoglobin affinity to oxygen has been found in one third of patients. It has been established that 2,3-DPG level and P50 value are, as a rule, normal in patients with mitral stenosis, acyanotic congenital heart valve defects and ischemic heart disease. In a number of patients with mitral stenosis an increased Hb concentration, as a slow mechanism of hypoxia compensation in the model of tissue pO2 regulation, is a prevailing mechanism of adaptation to hypoxia.

[Determination of the degree of saturation of hemoglobin with oxygen in blood microsamples].

The method of polarographic determination of blood O2 content (CO2) and oxygen capacity (OC) is described. The method for the calculation of Hb O2 saturation (SO2) by CO2 to OC ratio was compared to direct SO2 measurement on OSM-2 radiometer. The measured and calculated SO2 values were identical and correlated well (r-1.0), the accuracy of calculation independent of the baseline blood pO2 and Hb concentration. P50 and pO2 values, with SO2-50%, calculated by Hill's formula using the calculated and measured SO2 values were identical. Polarographic method of CO2 calculation does not require large blood volumes, is realized using simple devices and microanalysers, possesses adequate accuracy and reproducibility of the results.

[Different methods of determining the position on the oxyhemoglobin dissociation curve].

P50 values, as measured on DCA-1 analyzer and calculated according to Lichtman formula based on Hill equation, or calculated according to standard Severinghaus curve have been compared. No significant differences in P50 values have been observed. The method of direct oxyhemoglobin dissociation curve registration seems preferable, as it gives the idea not only of the position, but also of the form of curve changes. However, it cannot be widely recommended for clinical practice for lack of home-made equipment. Calculation methods are simpler, however, they depend on the accuracy of SO2 measurements and the necessity to use for calculations the results of blood pO2 measurements with SO2 in the range of 25-75%.
