[Postoperative urine transport disorders after supravesical urinary diversion].

By means of the intravenous urography we controlled the behaviour of the deriving urinary tracts after supravesical deviation of the urine using the intestine. In 70 patients 131 uretero-enterostomies were evaluated. The urogram pictures made postoperatively in different time intervals independent upon the intestinal segment used and upon the operation technique revealed after 4 weeks in 85.4% of the cases a clear disturbance of the transport of urine, partly up to the full picture of retention kidney. Under application of antibiotics and prednisolone these pathophysiological changes significantly receded. After 24 weeks the percentage decreased to 12.2.

[Effect of ionizing energy on the bladder dilated by dura, with special reference to the implantation technic. Animal experimental study].

On 60 female rabbits we examined the influence of the postoperative irradiation on the urinary bladder dilated by liophilized human dura after resection of the wall. Compared with former examinations as to the implantation technique the submucous deposition of the dura and fixation with single-button U-sutures (chromic catgut 000) has proved as superior. Vascular trophic disturbances on the margins of the resection could not be proved. When 4,500 R, 6,000 R and 7,500 R were irradiated the regeneration process through the dura serving as "live" rail retarded considerably. Problems in connection with the suture technique and the total focal dosage are discussed taking into consideration literature.

[Urine cytology studies in bladder tumors].

The stained urine smears of 151 patients were cytologically examined. The correspondence between histological and cytological diagnosis of the urine was 72.3% in 101 tumours of the urinary bladder. 5 of 11 patients with histologically benign papillomas of the urinary bladder showed malignant cells in the sediment. After four weeks 3 of them could histologically be ascertained as papillary carcinomas. Cytological investigations of the urine are suited for the early recognition of tumours of the urinary bladder, but also for the period between operation and radiotherapy as well as in the dispensary care.

[Histologic structure and spread of malignant neoplasms within the urinary bladder (author's transl)].

In 96 out of 250 cases of bladder cancer, neoplastic tissue was found by biopsy in areas within the bladder but remote of the primary and unsuspiciously looking mucosa. Neither the histologic type of the tumour nor the degree of infiltration of the primary allow predictions regarding remote cancerous lesions within the bladder. It must be concluded: In bladder cancer, indication and choice of treatment must take into consideration that the neoplastic process involves the urinary bladder as a whole.

New aspects in combined treatment of urinary bladder carcinoma.

To further improve the results of treatment of malignant processes of the urinary bladder, the authors reviewed in detail 420 cases under combined treatment. They found the following results: (1) In 96 (38.4%) out of 250 cases, the cystoscopically normal mucosa distant from the tumour showed histologically definite cancer tissue. (2) The destruction of residual tumour elements by ionising energy after operation depends on the relation between tumour and connective tissue. If there is sufficient connective tissue residual tumours may be destroyed in all probability, otherwise prognosis is poor. (3) In case of tumour cell mass formation in the microspace without active connective tissue, a curative effect by means of ionising energy after operation is not to be exptected, whether there is a multilocular development of carcinoma or not. Surgical treatment has to be extended according to the condition.
