Interference of manganese on neuroendocrinal system in exposed workers. Preliminary report.

The neurotoxic effects of Manganese (Mn) are well known, and are usually caused by high occupational exposure over long periods of time. The effects caused by low doses of this metal have been poorly evaluated. For this reason, we decided to evaluate neuroendocrinal tests in a group of 14 male workers employed in a ferrous-manganese foundry (exposed to Mn air concentrations within the TLV-TWA) and in 14 male control subjects, by analyzing FSH, LH, prolactin, and cortisol. The urinary Mn; prolactin, and cortisol levels were significantly higher in the worker's group. The preliminary results of this research seem to show that for exposure below the TLV, Mn can somehow interfere with the neuroendocrine system. In order to confirm the existence of these effects and to verify their possible correlation with the dopaminergic control system, further studies are necessary.

Chromosome and biochemical studies in women occupationally exposed to lead.

Chromosome and biochemical studies were conducted for 18 healthy females occupationally exposed to lead in the manufacture of electrical storage batteries and in 12 comparable female controls. Biochemical indicators of dose (blood lead) and effect (erythrocyte protoporphyrin, delta-aminolevulinicacid dehydratase activity of red blood cells) were significantly different in the two groups. Cytogenetic studies were performed on lymphocytes cultured for 2 and 3 days with phytohemagglutinin to investigate the possible effect of culture time with respect to yield of chromosome aberrations. The results showed significantly increased rates of metaphases with chromatid and chromosome aberrations--excluding gaps--in the exposed vs. controls at both culture times. The yield of abnormal metaphases was higher in the 3-day than in the 2-day cultures, both in the exposed and controls; the difference was moderately significant for controls, while highly significant for those exposed. These results demonstrate the occurrence of increased numbers of abnormal metaphases also in women occupationally exposed to lead, as previously found in males.

Meningeal melanocytoma: a rare lesion of the central nervous system.

Two cases of meningeal melanocytoma, both localized in the cerebellopontine angle, were studied. The patients underwent surgical excision, and in one case there was a recurrence. All the pathologic criteria of melanocytoma were fulfilled by the 2 reported cases. Immunohistochemical stainings were very important ancillary tools in the differential diagnosis with other pigmented meningeal tumors.

Long-term exposure to "low levels" of manganese oxides and neurofunctional changes in ferroalloy workers.

Occupational exposure to manganese can cause early neurobehavioral effects in low- or a-symptomatic workers. A battery of neuropsychological tests was administered to a group of 61 ferroalloy male workers and 87 controls. The average (geometric mean) manganese concentrations in total dust at the plant have changed from 1981 to 1997 respectively from 1597.03 micrograms/m3 to 239 micrograms/m3 at the furnace area; from 151.53 to 255.76 micrograms/m3 at the casting area; from 167 to 54.7 micrograms/m3 at the maintenance (welding operations), yielding a current overall value of 54.25 micrograms/m3. A cumulative exposure index was calculated for each alloy worker and the average value (geometric mean) resulted to be 1204.87 micrograms/m3 x years, which divided by the average length of exposure (15.17 years), showed the concentration of 70.83 micrograms/m3 of manganese in total dust. Blood and urinary manganese geometric means resulted significantly higher in the exposed workers (9.18 micrograms/l and 1.53 micrograms/g creatinine, respectively) than in controls (5.74 micrograms/l and 0.40 microgram/g creatinine, respectively). A positive correlation was observed between the airborne manganese concentrations in total dust and blood manganese (n = 55; R = 0.36; R2 = 0.13; p = 0.0068), whereas no association resulted between cumulative exposure index and both blood manganese and urinary manganese. Higher prevalence of symptoms reporting was observed in the alloy workers concerning irritability, loss of equilibrium and rigidity. Tremor parameters including the central frequency and its dispersion, resulted to be statistically different in the exposed workers compared to the controls. Motor functions exploring the coordination of rapid and alternating movements and memory functions resulted to be impaired in the manganese workers. Dose-effect relationships were observed between the cumulative exposure index and some of the test results, whereas no relationship was found with the airborne manganese concentrations and the biological indicators of exposure. These findings are consistent with the existing knowledge of a cumulative mechanism of action of manganese, which must be carefully considered when setting safe exposure levels. In order to be protective for the entire working life, the average annual exposure level should be lower than 100 micrograms/m3.

[A report of 3 cases of pleural mesothelioma with unusual asbestos exposure].

The occurrence of malignant pleural mesothelioma (MT) is a sentinel event in occupational and environmental medicine. The association of pleural MT and asbestos exposure is well documented and it is well known that no threshold can be demonstrated below which there is no risk of developing MT. During the period 1994-1998, 3 patients were referred from the Pneumology Division to the Occupational Medicine Department of the Spedali Civili in Brescia. They had MT and they had a peculiar asbestos exposure. A thorough occupational and environmental history was taken by means of a standardized questionnaire. The first patient was environmentally exposed to crocidolite when she lived near an asbestos mine in Australia. The second was a teacher and the third was a goldsmith and both were occupationally exposed to asbestos. The case descriptions revealed the importance of a standardized evaluation of occupational and environmental exposure to asbestos. In this way otherwise ignored asbestos exposures can be identified therefore avoiding an underestimation of MT attributable to asbestos. The role of the occupational physician, both in hospital referrals and while taking standardized histories, is also stressed along with the importance of this contribution on the one hand to the epidemiological recording of sentinel events and on the other to the etiological definition of the cases, which was in fact our principal aim.

[100 years of lead poisoning studies from a reading of articles published in La Medicina del Lavoro].

In preparing this paper we considered the articles published in La Medicina del Lavoro from 1901, its first year of publication. This scientific journal was founded in Milan, when an animated debate arose in Italy on the necessity of treating and, above all, preventing occupational diseases. In the same city, the "Clinica del Lavoro" (i.e. Institute of Occupational Medicine) was inaugurated in 1910. Its founder, Professor Luigi Devoto, had to overcome numerous obstacles caused by the hostility of the Rector of the University of Pavia--the future Nobel prize winner Camillo Golgi--and the clinicians of the main hospital of Milan, founded by Francesco Sforza in the XV century. From reading a century of articles which appeared in La Medicina del Lavoro, it is clear that for occupational physicians lead is an exemplary topic by which to evaluate the evolution of research in the field of occupational diseases. The numerous pathological features of lead poisoning, the successive therapeutic responses of physicians, and the gradual development of preventive techniques constitute a paradigm that has subsequently been applied to all other fields of industrial toxicology. Reading the papers of 100 years gives a clear picture of the evolution of clinical syndromes over the decades. The pathological picture of lead poisoning gradually became less serious and progressively changed into aspecific, subclinical manifestations. The categories of workers in which lead poisoning had a high incidence changed over the years: painters, printers and munition makers had the highest incidence in the first three decades of this century; afterwards, those engaged in lead smelting, alloy production, painters, and in the last few decades those employed in battery, ceramic and PVC production. Prevention consisted mainly of early diagnosis of lead poisoning and instruction in proper hygiene measures. Later, in 1929, insurance of occupational diseases was made compulsory in Italy, and among the few risk factors covered by law were lead and its compounds. This law was a great advance not only in the diagnostic and insurance fields but also for prevention. Two aspects of occupational lead poisoning are particularly instructive: treatment on the one hand and the use of laboratory analysis on the other. In treatment, the initial approach was mainly empirical and physicians insisted on evacuation of the bowel. Laboratory analysis started in the 20's with analysis of erythrocytes with basophilic stippling and continued with the study of urinary porphyrins. This was followed by the determination of lead in blood and urine. These tests were used initially as diagnostic tools, and only since the 60's they have been used for biological monitoring of workers for preventive purposes. The identification of indicators of dose, of critical/subcritical effect, and of critical organ started with studies on lead poisoning. Since then, following this model, biological monitoring has been applied to numerous other metals, solvents, and pesticides. The evaluation of the Italian scientific literature on lead over one hundred years in La Medicina del Lavoro has been a very exciting experience. It suggests that knowledge of the evaluation of lead poisoning and lead exposure should be taught to medical students and young physicians, thereby stimulating them to put into practice the maxim that was engraved on the foundation stone of the Clinica del Lavoro: in aliis vivimus, movemur et sumus.

[Health surveillance: scope, tools, efficacy, prospects].

According to experts of the European Commission, NIOSH and OSHA, health surveillance's goal is the both health protection and occupational diseases prevention (including injury and uneasiness). This goal is pursued by assigning workers according to his or her working capacity without any health prejudice. Currently available instruments to protect workers' health include: (i) medical surveillance before employment, (ii) medical surveillance before placement in a job involving exposure to toxics, (iii) medical surveillance at periodic intervals during employment, (iv) medical surveillance at termination of employment. Studies carried out on the efficacy of medical surveillance show the capacity both of identifying health problem and of being perceived as a benefit for workers. An appropriate application of medical surveillance should take into account points as efficacy, guidelines, costs and benefits, and effectiveness. This entails the need to carry out research aiming (i) to make the importance of currently used preventive instruments explicit and (ii) to study new approaches to be applied in occupational health care.

[Prevalence of carpal tunnel syndrome in workers executing complex tasks with the upper extremities].

In 50 female workers of a manufacturing plant employed on the assembly line of metal and plastic products with cloth covering, we found a high prevalence of Carpal Tunnel Syndrome (16 cases, 32%, 5 of whom had already undergone surgical treatment). The subjects with positive diagnosis of Carpal Tunnel Syndrome also had a longer occupational exposure to repetitive movements, compared to the other subjects. Among the subjects with this disorder, the length of exposure was also significantly associated with the reported symptoms and the extension threshold measured on the 2nd finger of the dominant hand. Ergonomic assessment of the assembly lines revealed high-frequency repetitive movements and application of force, with insufficient rest intervals.

[Organization of health and safety activities in health care centers].

The paper proposes a model for health and safety organization in health care units and hospitals which takes account of the risk assessment procedures required by law and the quality assessment of the measures thus taken. A redefinition is given of the role of Medical Director and of the functions, aims and standards on which health and safety service and the services of an authorized occupational health physicians must be based.

[Round Table "Role of scientific and professional associations in proposing occupational health programs to regional and national authorities].

The six participants at the round table were experts in the field of Occupational Prevention working respectively in the Ministry of Health, the National Institute of Occupational Health, the Health Council of Lombardy, the Law-court of Milan, the CGIL Trade Union, and the Lombard Employers Association. All the participants were of one mind on the establishment of working groups within the scientific and professional associations with the aim of preparing and updating occupational exposure limits, diagnostic records, programs of biological and environmental monitoring, etc. Moreover, they stressed the need to establish permanent cooperation between the scientific and professional associations and the regional and national health authorities. These associations can make an important contribution, for example in preparing new preventive guidelines, because they can act faster than the legislative bodies. The validity of these proposals was confirmed by the judge attending the round table. He stated that technical standards proposed by groups of professionals or by scientific associations have usually been taken into consideration when relevant legislation has been applied.

[Exposure to latex in the health setting: practical experience].

Latex allergy is progressively increasing due to the widespread use of individual protective disposals, especially latex gloves among health care workers. A multidisciplinary Working Group of the Association of Health Care Workers' Preventive Medicine published, in 1995, the Guidelines for the prevention of allergic reactions both in workers and patients. The Group identified a diagnostic protocol, including the assessment of predisposition, cross-reactions, level of exposure and symptoms; etiological diagnosis needs specific, allergologic and provocative tests. Primary prevention, pre-employment education and work restrictions for sensitised workers were particularly taken into account. A short questionnaire was prepared to identify latex sensitisation in patients undergoing invasive procedures; tests are performed only in selected subjects. Preventive measures require the use of nonlatex gloves and the identification and substitution of all latex products in health care settings; such measures are necessary when emergency procedures are performed.

[Medical surveillance and occupational identification of workers exposed to repeated movements of the upper limbs].

Repetitive motions of the upper limb at work can induce muskulo-skeletal alterations, therefore a specific medical surveillance is needed, according to the intensity of risk. In low risk conditions, the medical surveillance is aimed at the identification of hyper-susceptible individuals and the administration of specific information programs. When the risk intensity is higher, medical surveillance should add the recognition of early symptoms and signs of muskulo-skeletal disturbances, in order to remove workers showing early alterations from the exposure and allow medical rehabilitation. The diagnostic protocols with the adequate instrumental evaluation are provided, and the required legal reporting is discussed.
