Cytoplasmic tubular structures in the diagnosis of oral discoid lupus erythematosus.

Biopsies from oral lesions of 5 patients with discoid lupus erythematosus (DLE), 5 with lichen planus (LP), 5 with leukoplakia (LEUK) and 3 patients with uncertain diagnoses termed DLE?, LP? were examined in the electron microscope for presence of cytoplasmic tubular structures (CTS) in vascular endothelium. Five vessels were examined in each of two randomly selected sections from each biopsy, without knowledge of the clinical diagnosis. All five DLE biopsies showed presence of CTS in 4-9 out of 10 vessels, as compared with none of 10 vessels in biopsies from LP or LEUK, the difference being significant. Further, CTS occurred in 4 out of 10 vessels in one DLE?, LP? biopsy from a patient with serologic signs of systemic lupus erythematosus. As CTS seem to be absent in oral lesions of LP and LEUK, which are the most important differential diagnoses for oral DLE, the identification of CTS in vascular endothelium sing electron microscopy may be an auxiliary diagnostic procedure in oral DLE.

Relief from pain localized to the lower back in early labour. A double blind trial of ketocaine, a new, highly effective, local, cutaneous analgesic (A 2358, Ane-Pad), and placebo.

Pain localized to the lower back in the first stage of labour in 47 primiparous women was treated by epicutaneous application of ketocaine or placebo compresses. Twenty-three patients were treated with ketocaine compresses (Ane-Ped), and 24 patients with compresses soaked with isotonic saline. The pain relieving effect was almost the same in both treatment groups. Pain localized to the lower back was reduced in about 50% of the patients after one hour's application of ketocaine or placebo compresses. Supplementary analgesics had to be administered in about 60% of the patients in each group. No maternal or foetal systemic side effects related to the treatment were registered. Local erythema on the application area was only reported in the ketocaine group. The frequency of feeling of warmth during the application of compresses of ketocaine were high compared to the placebo group, 78% and 29% respectively.

Cell junctions in squamous epithelium during wound healing in palatal mucosa of guinea pigs.

The nature and distribution of cell junctions of the epithelium of wounded guinea pig palate were studied. Biopsies, 32 in total, were taken 18, 48, 96, and 120 h after wounding. Desmosomes were present in the wound epithelium throughout the observation period. Primitive desmosomes, indicating de novo formation of desmosomes, were observed at the front of advanced migratory lips (48 h) and in the central region and the wound epithelium after bridging (96 and 120 h). Similarly, de novo formed hemidesmosomes were seen. They occurred along epithelial cell membranes exposed to the fibrinous wound exudate during the migratory lip stages (18 and 48 h) and after epithelial bridging. The observations suggest that wounding induced changes in the mechanical properties of the tissue. Gap junctions were present in migrating epithelium preferably at the stem of the lips and in the epithelium after bridging, which suggests their participation in regulation of the epithelial cell differentiation. Tight junctions, capable of acting as seals, were absent or fragmentary at best. A relative decrease in the volume density of intercellular spaces in migrating epithelium as compared to intact epithelium, suggests an in vivo tissue reaction.

Ultrastructure of squamous epithelium during wound healing in palatal mucosa of guinea pigs.

The ultrastructure of the epithelium of wounded guinea pig palate was studied. Twenty-four biopsies (animals) were used. Biopsies were taken 18, 48, 96, and 120 h after wounding. Essential changes in epithelial cell structure in response to wounding were as follows: microfilaments, suggested to represent actin-containing filaments, became evident in the cortical cytoplasmic zone. Microtubules prevailed in the peripheral cytoplasm. Tonofilament bundles assumed the character of short tufts or delicate bundles and the amount of non-bundled tonofilaments was increased. These observations suggest changes in the cytoskeletal/contractile system facilitating cell motility. The structure of lysosomal profiles indicates an enhanced phagocytic capability. Influence of fixation on preservation of non-membrane-bound organelles is suggested based on comparable morphometric data.

Ultrastructural features of oral discoid lupus erythematosus.

Tissue from buccal and labial mucosal lesions of discoid lupus erythematosus (DLE) was studied in the electron microscope. The material comprised 10 patients, 7 of whom presented skin lesions as well. The epithelium showed a change in differentiation pattern from nonkeratinizing to keratinizing epithelium. Civatte bodies (filamentous bodies) were present intercellularly in the basal and suprabasal cell layers, and in the juxta-epithelial lamina propria. Dyskeratotic cells, suggesting immature stages of Civatte bodies, were recorded in the lower stratae of the epithelium. The epithelium-connective tissue interface was abnormal, presenting detachments, gaps and multiplications of the lamina densa. Cytoplasmic tubular structures were observed in epithelial and endothelial cells, as well as in fibroblasts, lymphocytes and macrophages. The oral tissue changes recorded were common in both groups of patients studied, i.e. with and without skin lesions, and similar to earlier reported changes of DLE skin lesions.

Quantitative analysis of epithelial changes during wound healing in palatal mucosa of guinea pigs.

The epithelium of wounded guinea pig palate was subjected to stereologic analysis. A total of 18 biopsies (animals) were used. Biopsies were taken at 18, 48, 96 and 120 h after wounding. Point counting procedures were employed to analyse electron micrographs sampled from one (18 h) or two epithelial strata (48, 96 and 120 h). The essential modulations in epithelial structure as wound healing proceeds were as follows: During the early phases characterized by formation and advancement of epithelial lips (18 and 48 h), migrating cells converged towards a cell type which structurally was less differentiated than normal basal cells. This alteration was expressed by a decrease in volume density of cytoplasmic organelles, mainly mitochondria, free ribosomes and tonofilament bundles, coupled with an increase in volume density of lysosomal bodies. Concomitantly, the volume density of cytoplasmic ground substance rose markedly. Subequent to fusion of contralateral migratory lips (96 and 120 h) reversion to normal epithelial structure was indicated by the increment in magnitude of basal cell parameters. Further structural density gradients from basal towards upper cell layers appeared. This pattern was mainly displayed by mitochondria, free ribosomes, and tonofilament bundles. The magnitude and gradation of most tissue and cell parameters were not yet re-established at 120 h. The density of tonofilament bundles and the density level of cytoplasmic ground substance in particular deviated.

Quantitative analysis of squamous epithelium of normal palatal mucosa in guinea pigs.

The epithelium of intact guinea pig palate was subjected to stereologic analysis in a study of structural alterations in the keratinizing epithelium in response to wounding. Point counting procedures were employed to analyse electron micrographs sampled from three epithelial strata in biopsies collected from five animals. The differentiation pattern of the guinea pig palate epithelium displayed the following structural density gradients from basal to granular layers: descending gradients of metabolically active organelles, ascending gradient of bundled filaments coupled with the appearance of membrane coating granules and keratohyalin granules, and a plateau-like gradient of cytoplasmic ground substance. This pattern of epithelial differentiation is basically identical to that of human hard palate epithelium and epidermis. Regional and species variations in structure of keratinizing epithelia are suggested based on interepithelial differences in morphometric parameters.

Effect of experimental neutropenia on oral wound healing in guinea pigs.

The influence of experimental neutropenia on the wound healing process in standardized excisional palatal wounds was studied in 16 guinea pigs. Corresponding wounds in 15 normal animals were used as controls. Neutropenia was induced and maintained by daily injections of heterologous anti-neutrophil serum. One-micron sections from biopsies of palatal mucosa sampled 6, 24, 48 and 120 h after wounding were evaluated with special reference to the progress of epithelial wound repair and to the occurrence and location of inflammatory cells and bacteria. In animals treated with anti-serum, neutrophilic leukocytes disappeared from the wound cavity, whereas other inflammatory cells remained unchanged. Further bacterial invasion deep into the cavity was observed. The epithelial repair took place at a deeper level in the wounds of neutropenic animals compared with those of normal animals. The rate of re-epithelialization was the same in neutropenic and control wounds. The experiment indicates that the neutrophils in oral wounds provide a principal agency for host defense against microorganisms and probably thereby indirectly determine the direction and level of epithelial cell migration.

Epithelial differentiation patterns in buccal mucosa affected by lichen planus.

The epithelium derived from buccal mucosa affected by lichen planus reticularis was subjected to histometric and stereologic analysis. Point counting procedures were employed to analyse a total of 2,220 electron micrographs sampled from 5 epithelial strata in 9 biopsies, 7 with distinct and 2 with faint Wickham's striae. Histologically the reticular elements presented an atrophy of the entire epithelium which had altered its differentiation pattern from that of normal buccal epithelium. The epithelium of faintly striated lichen planus differed from that of distinctly striated lichens in regard to a series of cytoplasmic organelles representing key features of stratified epithelia, i.e. the volume densities of glycogen, membrane coating granules, keratohyalin granules, cytoplasmic filaments and the cytoplasmic ground substance. The data emphasize that the clinically discernable features of reticular lichen planus of the buccal mucosa are produced by at least two different stages in the deviation of the epithelial differentiation pattern from normality, one stage (distinct striae) being more drastically altered than the other (faint straie).

Yersinia enterocolitica infection in patients with acute surgical abdominal disease. A prospective study.

The rate of yersiniosis in patients with acute abdominal disease was studied in a 16-month prospective investigation in 1972-1973 of 205 acutely ill patients referred to a surgical clinic of a Copenhagen City hospital with complaints of abdominal pain suggestive of appendicitis. Yersinia enterocolitica, biotype 4, was isolated from 11 patients (5.4%), 8 of whom were children. Yersinia was grown from faeces in 8 cases and from appendix of all 9 patients operated upon. Rising or falling agglutinin titres larger than or equal to 100, indicative of yersiniosis, were found in 22 patients (10.7%) including all bacteriologically verified cases. Eight additional patients (3.9%) had less significant titres larger than or equal to 100, suggestive of recent or present infection. 28 patients (13.7%) had insignificant titres, including 3 with antibodies against serotype 9. In all cases except these 3, antibodies were against Y. enterocolitica, serotype 3. A differential diagnosis between Y. enterocolitica infection and other types of appendicitis could not be made within this highly selected group of patients using available clinical data. All cases were rather mild and self-limiting. It is suggested that in future epidemiological and other studies of yersiniosis, early bacteriological and serological examinations be carried out.

Oral giant cell granulomas. An ultrastructural study of the vessels.

Eight peripheral and 2 central oral giant cell granulomas have been studied in the electron microscope in order to examine the structure and nature of the vessels. The vascular wall was composed of a continous layer of endothelial cells resting on a basal lamina, which was frequently multilayered. The lamina also surrounded scattered subjacent pericytes. The endothelial cells overlapped slightly so as to form narrow intercellular spaces. Based on these observations the vessels are supposed to be postcapillary venules. Dark cells exhibiting degenerative features were encountered in the endothelial lining of most vessels. Small gaps and larger defects in the vascular lining allowed direct continuity between the lumen and the perivascular tissue. It is suggested that the extreme extravasation of red blood cells frequently observed in oral giant cell granulomas may be mediated through this incomplete lining.
