Expression of histone H3 cell cycle-related gene, vimentin and MYC genes in pediatric brain tumors. A preliminary analysis showing the different malignant cell growth potential.

Eleven pediatric brain tumors were studied for the histone H3, Vimentin and MYC gene expression. H3, an S phase cell cycle-related gene (ccr), was found prevalently expressed in tumors with a high mitotic index (MI). Vimentin gene, which contributes to maintaining the cell structure but is also demonstrated to be an early responder gene to growth stimulation was found variously expressed. The different expression of Vimentin gene in the examined samples suggests the active proliferation of the tumor cells. Analysis of MYC gene expression was found increased only in a mesenchymal chondrosarcoma while in other samples MYC mRNA was undetectable. Medulloblastoma, chondrosarcoma, and choroid plexus carcinoma have high S phase H3 gene expression associated with a high MI. Differently an astrocytoma shows a low MI associated with high H3 gene expression. This first preliminary report of H3, Vimentin and MYC gene expression in brain tumors demonstrates that malignant cells are characterized by a different gene expression and different growth potentials.

N-myc oncogene amplification in a pediatric case of glioblastoma multiforme.

A sample of a primary brain tumor of glial cell origin was surgically removed from a 7-year-old girl. The histopathological analysis showed a heterogeneous tumor containing highly cellular areas composed of small, poorly differentiated cells with frequent mitoses suggestive of a glioblastoma multiforme. There were also areas presenting as features of lower-grade astrocytoma. Strong immunohistochemical staining for glial fibrillar acidic protein was demonstrated, while vimentin, neurofilament, and S-100 protein were all positive in just the astrocytic part of the tumor. The DNA extracted from a fresh tumor sample at diagnosis was processed by Southern blot analysis and hybridized with a 2.0 kb N-myc oncogene probe recognizing the first intron and the second exon of the human gene. A 20-fold amplification of the oncogene was found. The possible role of such a molecular alteration is discussed in light of the clinical presentation and histopathological features.

Occult spinal dysraphism: neuroradiological study.

We present a retrospective study of occult spinal dysraphism in 47 children aged 0 to 14 years, all studied with plain X-rays, 60% with CT and myelo-CT, and 40% with MR. We consider the classification and grading of these malformations, clinical, neuroradiological patterns, and indications for surgery. In the light of our findings and of the published data MR emerges as the key investigation. Only in a few cases of great anatomical complexity is it now necessary to perform CT and myelo-CT as well. A case in point is when the conus and thickened filum terminale are inextricably bound together and can no longer be considered separate structures. We propose the term "neurofibrous structure" to define the conus-thickened-filum-terminale unit when these structure are no longer distinguishable.

[Cerebral arteriovenous malformations. Natural history. Therapeutic approaches].

Intracerebral hemorrhage is the most frequent clinical manifestation of arteriovenous malformations (AVM). That clinical manifestation is more frequent and more severe in youngest patients. Cerebral angiography demonstrates the location and size of the AVM and identifies its arterial supplies. These informations are related to the neurological status of the patient to decide the most adequate treatment. Surgical treatment, embolization or radiosurgery can be performed or associated to eradicate totally the AVM. Several procedures are sometimes necessary to obtain the definitive cure of the malformation.

Cyst of the choroid plexus of the left lateral ventricle.

A cyst of the choroid plexus of the left ventricle is described. The cyst was lined by a choroidal epithelium and filled with cerebrospinal fluid. It produced an intermittent obstructive syndrome. The cyst was almost completely invaginated into the third ventricle, simulating an expanding process in the third ventricle. After microsurgical removal, the recovery of the patient was complete. Electronmicroscopy of the cyst wall is reported. Histological and neuroradiological features are discussed.

Spinal evoked responses recorded from the epidural space in normal and diseased humans.

In 22 subjects, including normal subjects and patients with radicular or spinal cord lesions, the authors studied the spinal evoked responses recorded extradurally after stimulating mixed limb nerves. The responses obtained are discussed with particular reference to the clinical value of the changes in amplitude and latency of the spinal evoked potential with particular lesions.

The lateral recess of the fourth ventricle examined with a water-soluble contrast medium.

Difficulties still remain in the routine radiologic examination of the lateral recesses of the fourth ventricle. Ventriculography with the direct introduction of water-soluble contrast medium into the third ventricle has improved the results. The normal anatomic and radiologic appearances and pathologic changes of the lateral recesses are discussed.
