Decidual cells in the human ovary at term. I. Incidence, gross anatomy and ultrastructural features of merocrine secretion.

Decidual tissue occurring within the human ovarian cortex was examined by light and electron microscopy. Of 21 ovarian specimens obtained at term (36-42 weeks of gestation), decidual cells were confirmed in each. Decidual cells were found within the tunica albuginea as single cells, in nodules, in polyps or in confluent sheets. Decidual cells exhibited several characteristics of cells engaged in secretory activity: abundant rough and smooth endoplasmic reticulum, numerous profiles of the Golgi complex and a large, euchromatic nucleus devoid of heterochromatin and displaying a prominent fibrous lamina. Peduncular protrusions at the periphery of the cell contained numerous dense bodies 0.4-0.9 micron in diameter. These dense bodies were bounded by a single membrane and contained granular subunits 30-60 nm in diameter. These granular subunits were observed in the process of apparent exocytosis, as well as free in the extracellular space. Secretory bodies and their granular content also were observed both in the region of the Golgi complex and partially extruded into peduncular processes. By far the greatest number of secretory bodies occurred within peduncular processes where they may be stored prior to release. Migration of a secretory body into a peduncular process and exocytosis from such a process appears to be an unusual mode of meocrine secretion, perhaps unique to decidual cells.

Fetal activity determinations and oxytoxin challenge tests for assessment of fetal well-being.

Fetal activity determinations followed by oxytocin challenge tests were performed in 101 patients. When fetal heart accelerations occurred following fetal movement, the oxytocin stress test was negative in 99.5% of the cases. An abnormal fetal activity determination was followed by a positive oxytocin challenge test in 12% of the cases. It is suggested that a normal fetal activity determination is indicative of good fetal reserve and therefore reduces the need for oxytocin challenge test. An abnormal fetal activity determination, however, is unreliable and requires that an oxytocin challenge test be performed.

Effect of exercise on cardiac output and distribution of uterine blood flow in pregnant ewes.

This study was designed to determine what effect physical training has on heart rate and stroke volume responses to exercise stress and to determine if exercise altered the distribution of uterine blood flow. Measurements were made in ten pregnant ewes at rest and immediately following exercise on a treadmill. Five ewes underwent physical training for 3 wk prior to measurement. An increase in heart rate with no change in stroke volume was observed following exercise in both trained and untrained ewes. Total uterine blood flow was not changed following exercise, but distribution was altered in favor of the placenta. Blood flow was evenly distributed within the placenta before and after exercise. The redistribution of flow to the placenta that occurs after exercise. tphe redistribution of flow to the placenta that occurs after exercise might represent a compensatory mechanism for the fetus.

Treatment of acute leukemia during pregnancy.

Cytarabine and thioguanine therapy for acute myelocytic leukemia, initiated during the 26th week of pregnancy, led to complete remission of the leukemia in a 22-year-old woman, and allowed for delivery of a normal infant at term. No chromosomal abnormalities were detected in the infant. Cytarabine and thioguanine in combination are effective agents in the treatment of acute leukemia in adults. Their use appears warranted in pregnant patients after the first trimester.

Aromatizing activity of placental microsomal fractions from ewes in late gestation.

Placental microsomes from eight domestic sheep at 136-146 days of gestation were incubated with radioactive androstenedione, testosterone and dehydroepiandrosterone. Aromatizing activity was examined in the presence and absence of cortisol and the rates of both oestrone and oestradiol synthesis were measured. Oestrone predominated in preference to oestradiol in most of the incubations, a result opposite to that found with human placentae. The sharp increase in the rate of oestradiol production found in the 144- to 146-day-old placentae incubated with testosterone may indicate a more rapid increase of aromatizing than of 17beta-hydroxysteriod dehydrogenase activity. The presence of cortisol in the mixtures did not significantly affect the placental aromatizing activity, indicating that there is no direct effect of cortisol on the enzyme system as measured in vitro. The dramatic rise of overall mean aromatizing activity from 4.86 plus or minus 0.22 (S.E.M.) at 138-141 days of gestation to 12.96 plus or minus 0.38 pmol/mg protein/min at 144-146 days (with a greater relative increase in the rate of oestradiol formation), suggests that changes in placental aromatizing activity may play an important role in maternal and foetal plasma oestrogen surges before ovine parturition.
