[The TRH test in latent thyroid pathology].

TRH-test is an usefull tool for diagnostic purpose in thyroid disease. We challenged with TRH 4 groups of patients (1 degrees non toxic diffuse goitre, 2 degrees nodular goitre (Plummer's disease), 3 degrees euthyroid ophthalmic Graves' disease, 4 degrees hemithyroidectomy) with different thyroid affections who showed no symptoms and presented normal values of total thyroid hormones, in comparison with a control group. These groups displayed normal and pathological (blunted or absent and exaggerated) responses of TSH to TRH. Therefore TRH-test ra presents a good mean to recognize subclinical forms both hyperthyroidism and hypothyroidism.

[Role of conventional radiology and computed tomography in pre-surgical TNM staging of lung tumours: experience on 41 cases (author's transl)].

Comparison between standard radiographic and CT evaluation with surgical exploration was carried on 41 patients affected by bronchogenic carcinoma. The standard radiographic and CT evaluation was mae only for "T" and "N". In the named 41 patients the surgical evaluation of the anatomic extent (only for T and N) was made. T was underestimated by standard radiographic; on the contrary N appeared underestimated by the two radiological methods.

[Skeletal manifestations of primary and secondary hyperparathyroiditis. Differential radiological diagnostic problems].

Primary and secondary hyperparathyroidism are both marked by widespread skeletal demineralisation, subperiosteal erosion of the cortex, brown tumours, osteosclerosis, and extraosseous calcification. Differential diagnosis is guided by the different association of these findings. Brown tumours and more extensive erosion are marks of the primary form, whereas osteosclerosis and extra-osseous calcification are a prominent feature of secondary hyperparathyroidism. Radiologists, therefore, should direct their attention to features suggesting the presence of secondary forms in addition to looking for bone alterations associated with hyperparathyroidism.

[Aneurysmal dilatation of the superior vena cava].

A case of aneurysmatic dilatation of the vena cava superior is presented. This lesion is rare and not commonly taken into consideration in the differential diagnosis of anterior mediastinal masses. Chest X-rays taken during inspiration and expiration and in the standing and reclining positions (particularly the second and fourth) revealed changes in the size of the mediastinal mass and pointed clearly to its venous nature. This deduction was then confirmed angiographically.

[Combined radiochemotherapeutic approach to bronchogenic carcinoma: criteria of applicability and limits].

The knowledge of the natural history of pulmonary cancer had led to both an improvement of the techniques and methods of radiation treatment and the use of chemotherapy in association with radiation therapy. Such an association leads to graver and more frequent pulmonary reactions. The development of a particular discipline (the toxicology of radiation) could contribute to predict the incidence and the extent of reaction. A more precise combined approach to the treatment of pulmonary cancer would therefore be possible.

Chronomodulated infusion of 5-fluorouracil, folinic acid and carboplatin in colorectal cancer: a pilot study.

We investigated if chronomodulated infusion of fluorouracil, folinic acid in combination with carboplatin shows antitumor efficacy in patients advanced metastatic colorectal cancer. Thirteen patients entered into the study. Each treatment cycle consisted of a 5 day course of continuous venous infusion of 5-fluorouracil (5-FU; 500 mg/m2/die), folinic acid (FA; L-form, 150 mg/m2/die) and carboplatin (CBDCA; the dose being calculated according to the Calvert's formula). Patients received the drugs according to the circadian-modified infusion schedule with a sinusoidally modulated delivery with peak flow rate at 4.00 AM for 5-FU and FA and 4.00 PM for CBDCA. Overall a total number of 54 cycles were administered. Two partial responses and one minor response were observed among the 5 untreated patients. Five patients had progression of disease. Stabilization of previously progressive disease was obtained in the 5 remaining patients. These preliminary results show that the combination of 5-FU, FA and CBCDA infused at circadian-modulated rate has moderate activity in colorectal cancer, specially in previously untreated patients, with a mild and acceptable toxicity.
