Gastric polyps--a clinical study.

In 72 of 13,500 patients who underwent endoscopy of the upper digestive tract during an 8-year period, 99 gastric polyps were found. All the polyps were biopsied; 18 were also removed. Histological examination of the 99 polyps showed that 25 were inflammatory, 23 hyperplastic, 17 hyperplasiogenic, 10 adenomatous, 3 hamartomatous, 3 with intestinal metaplasia and 18 with normal mucosa. The histological diagnosis was changed following polypectomy in 50% of the polyps that had been removed. Dysplasia was discovered in two adenomatous polyps; no carcinoma was found. In two cases the polyps were syncronous to carcinoma; in two other cases, they were metachronous to carcinoma. Inflammatory polyps were found in association with inflammation of the upper gastrointestinal tract, such as duodenal and gastric ulcer, esophagitis, gastritis and duodenitis. No correlation was demonstrated between the symptoms and the type or location of the polyps. In 10 patients, who were under observation for an average duration of 3.5 years, 3 polyps disappeared, 1 was removed and 11 had not changed. We conclude that endoscopic polypectomy of gastric polyps may not always be indicated and should be reserved for polyps that were adenomatous, according to the biopsy, or that had grown and changed their shape in a follow-up endoscopy.

Adenocarcinoma

Endoscopic Gruntzig balloon dilation of benign strictures in the biliary system.

Five patients with benign strictures of the biliary tract were treated by Gruntzig balloon dilation during endoscopic retrograde cholangiopancreatography. All of the strictures were effectively dilated. One patient with a retained stone in the proximal biliary tree underwent surgery 3 weeks after the dilatation because of cholangitis. The other four patients were well at a mean follow-up of 6 months. There were no complications related to the procedure. We conclude that Gruntzig balloon dilation of benign bile duct strictures is safe and effective, and is a potential endoscopic retrograde cholangiopancreatography.

Spectrum of extracolonic gastrointestinal tract involvement in Gardner's syndrome.

To define the prevalence of extracolonic gastrointestinal tract neoplasms in association with Gardner's syndrome, 17 patients with the syndrome underwent a systematic endoscopic and radiologic investigation. Adenomas were found at one or more sites in the upper gastrointestinal tract in all patients, most frequently at the papilla of Vater (64.7% of patients). Focal adenomatous changes in normal-appearing mucosa were seen in five patients, at the ampulla of Vater and in the duodenum in four, and in the gastric antrum in two patients. Adenomas were also seen in the duodenum (47%), stomach (29.4%) and small intestine (23.5%). Hyperplastic polyps were found in the gastric antrum, duodenum or small bowel in five patients. Adenomas of the papilla of Vater showed greater degrees of dysplasia than did adenomas of the stomach or small bowel, which might explain the more frequent occurrence of malignancy at the papilla of Vater in patients with Gardner's syndrome. A systematic investigation of the gastrointestinal tract is mandatory in all patients with Gardner's syndrome prior to surgery, and at periodic follow-up thereafter.

Adenoma

Regeneration of rectal mucosa and recurrent polyposis coli after total colectomy and ileoanal anastomosis.

Two patients with adenomatous polyposis of the colon who had been treated by colectomy, stripping of rectal mucosa, transrectal pull-through of terminal ileum, and ileoanal anastomosis were found--after seven and three years--to have recurrent polyposis in the rectal segment, consequent to regeneration of the rectal mucosa. The latter might have resulted form retraction of the implanted ileum or sloughing of its mucosa, followed by upward growth of epithelium from the strip of anal mucosa retained above the dentate line at the time of surgery. Colonic metaplasia of the implanted ileal mucosa may be an alternative explanation. In all patients who have had this operation for polyposis coli, the rectal segment should be reexamined by endoscopy and biopsy to rule out recurrent polyposis or replacement of ileal mucosa by colorectal mucosa.

Adenoma

Pyloric channel duplication in a newborn infant.

This is the first reported case of pyloric channel duplication demonstrated preoperatively by endoscopy and contrast radiography. Endoscopically, the lesion resembled a mucosal flap overlying the pyloric orifice. On radiography the lesion presented as a polypoidal filling defect in the duodenal bulb. The infant had, in addition, postduplication stress ulcer.

Ketamine anesthesia for pediatric gastrointestinal procedures.

Ketamine HCl anesthesia was used for gastrointestinal therapeutic and diagnostic procedures in 26 children aged 3 mo to 16 yr. In addition, atropine was used for the gastroscopies and diazepam for the colonoscopies. Ketamine, 5 to 8 mg/kg body wt administered i.m., produced rapid anesthesia of short duration which allowed easy performance of the procedure. No serious side effects were noted, but care must be taken as ketamine is a general anesthetic with potential side effects during and after the period of anesthesia. This satisfactory initial experience should be confirmed by a controlled study of anesthetic agents for pediatric gastrointestinal procedures.
