The study aimed to produce solid lipid insulin-loaded micro-particles by the solvent-in-water emulsion-diffusion technique, using isobutyric acid as solvent phase, glyceryl monostearate or cetyl palmitate as lipid, soya lecithin and taurodeoxycholate as emulsifiers. Isobutyric acid, a partially water-miscible solvent with low toxicity, was used due to its high insulin-solubilization capacity. Solid lipid micro-particles of spherical shape were prepared by simple dilution of the emulsion with water. To increase the lipid load the process was conducted at 50 degrees C, and in order to reach sub-micron size, a high-shear homogeniser was used. Insulin encapsulation efficiency was about 80%. Analysis of microsphere content after processing showed that insulin did not undergo any chemical modification within the micro-particles. The in vitro release of insulin from the micro-particles was very low, and an initial burst effect of 20% of the dose was observed. After treatment of the solid lipid micro-particles with pepsin solution, an insulin loss of about 24% of the total englobed insulin was observed. The solid lipid micro-particles appear to have interesting possibilities as delivery systems for oral administration of insulin.
The case of anastomotic rupture of the subclavian artery following infection of a subclavian-to-carotid bypass grafting is reported. Emergency endoluminal stent graft repair was life saving and aided wound healing, but stent graft thrombosis occurred. Such a complication raises some concern about the safety of this procedure in an infected setting. The use of autologous saphenous vein-covered stent graft may provide some advantages in avoiding graft infection and thrombosis.
Capacitation is a prerequisite for successful fertilization by mammalian spermatozoa. This process is generally observed in vitro in defined NaHCO3-buffered media and has been shown to be associated with changes in cAMP metabolism and protein tyrosine phosphorylation. In this study, we observed that when NaHCO3 was replaced by 4-(2-hydroxyethyl)1-piperazine ethanesulfonic acid (HEPES), hamster sperm capacitation, measured as the ability of the sperm to undergo a spontaneous acrosome reaction, did not take place. Addition of 25 mM NaHCO3 to NaHCO3-free medium in which spermatozoa had been preincubated for 3.5 h, increased the percentage of spontaneous acrosome reactions from 0% to 80% in the following 4 h. Addition of anion transport blockers such as 4,4'-diiso thiocyano-2, 2'-stilbenedisulfonate (DIDS) or 4-acetomido-4'-isothiocyanatostilbene-2,2'-disulfonic acid (SITS) to the NaHCO3-containing medium inhibited the acrosome reaction, with maximal inhibition at 600 microM, and with an EC50 of 100 microM. Increasing either extracellular or intracellular pH did not induce the acrosome reaction in NaHCO3-free medium. In contrast, addition of 500 microM dibutyryl cAMP (dbcAMP), alone or together with 100 microM 1-methyl-3-isobutylxanthine (IBMX), induced the acrosome reaction in spermatozoa incubated in NaHCO3-free medium. These compounds also partially reversed the inhibition of the acrosome reaction caused by the DIDS or SITS in complete medium. In contrast to these results, IBMX or dbcAMP did not induce acrosome reactions in cells incubated in Ca2+-free medium. When hamster sperm were incubated in the absence of NaHCO3 or in the presence of NaHCO3 and DIDS, cAMP concentrations were significantly lower than the values obtained from sperm incubated in complete medium. Protein tyrosine phosphorylation has also been shown to be highly correlated with the onset of capacitation in many species. During the first hour of capacitation, an increase in protein tyrosine phosphorylation was observed in complete medium. In the absence of NaHCO3, the increase in protein tyrosine phosphorylation was delayed for 45 min, and this delay was overcome by the addition of dbcAMP and IBMX. The induction of the acrosome reaction by calcium ionophore A23187 in NaHCO3-free medium was delayed 2 h, as compared with control medium. This delay was not observed in the presence of dbcAMP and IBMX. Taken together, these results suggest that a cAMP pathway may mediate the role of NaHCO3 in the capacitation of hamster spermatozoa and that protein tyrosine phosphorylation is necessary but not sufficient for complete capacitation.
We describe a case of an isolated aneurysm of the left hypogastric artery which came under our observation because of a syndrome of compression on the homolateral iliac vein. Having excluded the presence of aneurysms in other sites, it was decided to perform a percutaneous embolization of the hypogastric aneurysm. The follow-up revealed the disappearance of the compression on the ipsilateral iliac vein and a 30% reduction in the diameter of the thrombosed aneurysm. The method limited invasiveness, the reduction in volume of the aneurysmal mass, the disappearance of compression problems, and the shortened hospitalization time support the choice of embolization treatment.

In the Observation Department of S. Giacomo Hospital the authors carried out a study on 242 patients, in whom they found a latent vascular pathology in some cases and a manifest vascular pathology in other cases. The patients were divided into two groups: (a) patients at risk, (b) manifestly vasculopathic patients and they were then subdivided according to the compromised region. The subjects underwent sero-haematological and radiological tests, instrumental tests not involving operations such as Doppler ultrasonography and rheography, and tests involving operations such as angiography. Both medical and surgical therapeutic means were used. Then the authors carried out a therapeutic trial with buflomedil chlorhydrate on subjects with obliterative vasculopathy with different aetiologies.

The introduction of christiansen's prosthesis for the head of the femur has altered the balance between the two alternative surgical solutions for subcapital fractures of the neck of the femur (osteosynthesis and endoprosthesis). The writers report their results on the first forty-five patients treated surgically with the new prosthesis, illustrating its functional advantages as compared with the "single block" endoprosthesis. The risk of wear and tear of the acetabulum is practically abolished.
The authors review eighty cases of pertrochanteric and subtrochanteric fractures of the femur treated surgically with the vitallium Jewett nail-plate between May 1975 and November 1977. They draw attention to the possibility offered by this fixation device of starting weight bearing at an early stage, a factor that is unanimously regarded as essential for rapid recovery in the elderly fracture patient. In fact, fifty-six patients commenced independent walking with some support within two weeks of operation. The fairly low incidence of complications (six in eighty operations), and the relative simplicity of the surgical technique, support the Jewett nail-plate as a suitable device for rigid fixation of trochanteric fractures of the neck of the femur.
