[The NACA scale. Construct and predictive validity of the NACA scale for prehospital severity rating in trauma patients].

The NACA-scale is used in many Austrian, German and Swiss emergency medical systems for demographic description of emergency patients. Little attention has been payed to the evaluation of its construct and predictive validity. In 427 consecutive trauma patients rescued in primary mission the NACA-Scale and the Injury Severity Score (ISS) were determined. Outcome data were obtained from medical charts and by written or telephone requests. Data were analysed with Spearman-Rank-Correlation. NACA-Grade and ISS-values showed only a moderate correlation with a considerably large spread (Rho = 0.721). Both severity scores demonstrated a good correlation to mortality (Rho = 0.976/0.994) and to transfer to an ICU (Rho = 0.964/0.943), as well a moderate correlation to the duration of ICU-stay (Rho = 0.722/0.756) and of hospital stay (Rho = 0.558/0.694). The NACA-scale adequately describes life threat in trauma victims and correlates well with morbidity and mortality. Thus, it is a valuable tool for demographic purposes in emergency medical systems. For more precise prehospital severity rating in trauma patients, the NACA-scale should be supplemented or replaced by a physiologically based prehospital severity score.

[Airway control and ventilation in resuscitation].

Oxygenation and ventilation of the lungs is one major objective of cardiopulmonary resuscitation [CPR] to provide oxygen to the heart and the brain. Prompt and correct airway management requires immediate opening of the airway and performing rescue breathing, preferably with supplemental oxygen. Endotracheal intubation provides definitive airway management, is the gold standard, has no equivalent substitute, and should be performed as soon as possible. If an endotracheal tube cannot be placed correctly, sufficient ventilation must be achieved by a face mask. Cricothyrotomy can be a life saving action in case of suffocation if airway control is impossible and airway obstruction cannot be relieved. Skills in techniques of airway management as part of CPR must be acquired in training programs under controlled conditions. In rescue breathing with the mouth technique, a barrier device against transmission of infectious diseases is recommended.

[Monitoring during secondary transportation].

Stabilization of the vital functions of critical patients prior to transport is mandatory even in urgent interhospital transfers. Although distances may be short, hasty transport is potentially harmful for the patient. The monitoring systems available for emergency care today facilitate mobile intensive care medicine. However, there are still technical limitations which may compromise the continuous invasive monitoring already installed before transport. Special care is needed in developing combinations of monitoring and treatment systems, especially for the transport of critically ill and injured patients.

[Domperidone after ether anaesthesia. Prevention of postoperative hyperemesis (author's transl)].

In catastrophe situations ether is an important alternative to the usual anaesthetics being easy to manage and economical. We wanted to study the effects of the new antiemetic Domperidone on vomiting after either anesthesia. Two groups of patients were anaesthetized for general surgical procedures by means of the EMO (Epstein-Macintosh-Oxford) Vaporizer. Half an hour before the end of the operation one group received 0.2 mg/kg domperidone while the randomized control group received no prophylactic antiemetic medication. The domperidone group showed better, statistically significant, results (p less than 0.05). Domperidone proved to have a good prophylactic antiemetic effect which does not cause side effects or circulatory alterations. In preparation for catastrophe situations ether anesthesia under improved conditions was again included in the regular training programme.
