Infections associated with cantaloupe consumption: a public health concern.

Fresh produce is an important part of a healthy diet and is consumed in greater quantity in the United States than ever before. Consumption of cantaloupe has recently been associated with several large outbreaks of infections in North America, highlighting the need for a better understanding of practices and processes that may contribute to contamination. We reviewed all cantaloupe-associated outbreaks that were reported to the Centers for Disease Control and Prevention (CDC) and published in the literature. Twenty-three outbreaks occurred between 1984 and 2002; 1434 people became ill, 42 were hospitalized, and two died in these outbreaks. Aetiological agents in the outbreaks included five serotypes of Salmonella enterica, Campylobacter jejuni, Escherichia coli O157:H7, and norovirus. We reviewed processes contributing to cantaloupe contamination, conditions affecting survival and growth of bacterial pathogens on melons, and potential methods for sanitization. For maximum safety, industry, federal, and international partners must collaborate to ensure that appropriate interventions are in place to minimize the risk of contamination and prevent the growth of pathogens during cantaloupe production, processing, storage, and preparation.

[Serologic and immunologic evaluation in uveitis].

Following a brief discussion of the various general immunologic examinations useful in uveitis, the author goes on to stress the need for a goal of serologic examinations, taking the clinical picture and clinical context into account. In conclusion, a schematic approach is proposed which should be adhered to in paraclinical investigations.

[Abnormalities of retinal function (light sense) following idiopathic central serous chorioretinopathy and diffuse retinal epitheliopathy].

Although central serous retinopathy is considered a "benign" condition, it is associated in healed forms with frequent and troublesome impairment of visual function. The abnormalities are even more severe after diffuse retinal pigment epitheliopathy. Since Snellen visual acuity testing is a relatively imprecise index of visual function, it does not enable the subtle visual disturbances experienced by these patients to be fully appreciated. The authors evaluated visual function disorders by static automated perimetry and contrast sensitivity in 30 patients with healed central serous retinopathy and in five patients with diffuse epitheliopathy at a cicatricial stage. The results of this study are examined and discussed.

[Malignant lymphoma of parotid origin mimicking Melkersson-Rosenthal syndrome].

The Melkersson-Rosenthal syndrome is classically described as the association of a unilateral tumefaction, a facial hemiparesia and of a furrowing of the tongue. Granulomatous blepharitis is an unusual localisation of the facial tumefaction, and chronic periocular oedema may represent a difficult differential diagnosis. The reported case has erroneously been diagnosed as a Melkersson-Rosenthal syndrome and further showed to be a parotid gland involvement of a systemic malignant lymphoma.

[Acquired postcontusion keratoconus].

Anterior keratoconus is rarely complicated by acute ectasia. Acute hydrops may occur suddenly when Descemet's membrane ruptures, spontaneously or after a trauma in a formely thinned cornea. The case of acute ectasia we describe is surprising; it occurs after a trauma in a normal eye or eventually affected by a minor form of anterior keratoconus.

[Vision aids for patients with senile exudative macular degeneration. Comparison of spontaneous fibrovascular scars and scotomas induced by photocoagulation].

Aging exudative macular degeneration frequently results in a fibrovascular scar which entails the loss of central vision. This scar implies an often large and deep scotoma which makes it very difficult for the patient to be fitted with low-vision aids. A Goldmann static perimetry study of retinal sensitivity in the treated areas showed that krypton laser at liminar doses does not induce deep scotomas.

Optic nerve hypoplasia: papillary diameter and clinical correlation.

Hypoplasia of the optic nerve is often subtle, and reduction in papillary diameter can be difficult to detect during ophthalmoscopic examination. Using a slit-lamp and a posterior fundus contact lens, we evaluated papillary diameter by bringing margins of the light beam to coincide with the limits of the optic nerve head. The method was standardized with 140 presumably normal eyes. Then we studied 17 consecutive eyes showing optic nerve hypoplasia. Hypoplasic discs were separated into two categories: (1) nine cases defined as having "moderate" hypoplasia, in which average disc diameters ranged between -2 and -2.5 standard deviations under the mean normal value; and (2) eight eyes with "severe" hypoplasia, showing average disc diameters less than the normal mean value by 2.5 standard deviations or more. Double peripapillary ring sign and low disc/artery ratio are clues often used in the diagnosis of optic nerve hypoplasia. The incidence of these signs was also evaluated in the hypoplasic discs studies. The double ring sign was present in two of the nine eyes with moderate hypoplasia, and in seven of the eight eyes with severe hypoplasia. Of eight eyes with moderate hypoplasia, only one showed a disc/artery ratio inferior to the mean normal value -2 standard deviations, whereas this ratio was significantly reduced in all cases of severe hypoplasia. These ophthalmoscopic clues therefore appear to be much less valuable than direct measurement of the optic disc for detecting a subtle reduction in disc diameter. This is of particular interest when one considers that all the nine eyes with moderate hypoplasia had alteration in visual function upon presentation.(ABSTRACT TRUNCATED AT 250 WORDS)

[Intraocular listeriosis].

Listeria monocytogenes endophthalmitis is an uncommon localization. Its evolution is usually complicated by disabling functional visual loss. We report a case occurring in a peculiar epidemiological context. Early diagnosis by paracentesis allowed consequent antibiotic therapy. The evolution has been dramatically favorable. Early treatment allowed an excellent functional recovery when compared to the reported cases of the literature.

[Evaluation of retinal function following central serous retinopathy and diffuse epithelial retinopathy].

The so called "benign" central serous retinopathy is associated in healed forms with frequent and troublesome visual function impairment. The abnormalities are still more severe after diffuse retinal pigment epitheliopathy. Snellen visual testing is relatively imprecise index of visual function, so it does not permit to appreciate the subtle visual disturbances experienced by these patients. We evaluated visual function disability with static automatized perimetry and contrast sensitivity in 30 patients with diffuse epitheliopathy in a cicatricial phase. Colour vision was also evaluated by 16 patients after central serous retinopathy. The results of this study are examined and discussed.
