Identification of Beck Depression Inventory items related to multiple sclerosis.

The percentage contribution of each item on the Beck Depression Inventory (BDI) to the total BDI score was compared across patients with multiple sclerosis (MS), patients diagnosed with major depressive disorder, and normal college students. We considered an item to be confounded by MS-related symptoms if its percentage contribution to the total BDI score was significantly greater in the MS group than the major depression and control groups. Items measuring work difficulty, fatigue, and concerns about health met this criterion. These items accounted for 34, 17, and 19% of the total BDI score in the MS, major depression, and control groups, respectively. Using the 18-item BDI (BDI-18) which resulted from excluding the 3 confounded items, MS patients found to be were more depressed than controls but less depressed than the major depression group. The identification of signs of depression not confounded with MS which could be substituted for confounded signs was also discussed.

Reliability and validity of the short form Beck Depression Inventory with older adults.

This study investigated the reliability and validity of the short form Beck Depression Inventory with older adults. Sixty-one clinically depressed older outpatients and 57 non-patient older volunteers comprised the sample. The patient group completed the BDI prior to and at completion of depression treatment. Estimates of internal reliability suggest that the short form BDI possesses adequate Spearman-Brown and alpha coefficients. Congruent validity estimates were less satisfactory, as the correlation between the BDI and the HRSD was statistically significant, but low. Criterion group validity, diagnostic sensitivity, and sensitivity to change estimates were all acceptable. Thus, the short form BDI is adequately reliable to suggest its use as a research and clinical tool with older adults, although further study of the instrument's validity is needed.

Glycoprotein Ib and glycoprotein IX are fully complexed in the intact platelet membrane.

Two new murine monoclonal antibodies, AK 1 and SZ 1, reactive with the human platelet glycoprotein (GP) Ib-IX complex have been produced by the hybridoma technique. Both AK 1 and SZ 1 immunoprecipitated the GP Ib-IX complex from Triton X-100-solubilized, periodate-labeled platelets. With trypsinized, labeled platelets, AK 1, SZ 1, and FMC 25 (epitope on GP IX) immunoprecipitated a membrane-bound proteolytic fragment of the GP Ib-IX complex consisting of GP IX and an congruent to 25,000 mol wt remnant of the alpha-chain of GP lb disulfide-linked to the beta-subunit. Unexpectedly, although AK 1 and SZ 1 immunoprecipitated purified GP Ib-IX complex, neither antibody immunoprecipitated the individual components of this complex, GP Ib or GP IX. When GP Ib and GP IX were recombined, however, AK 1 and SZ 1 again immunoprecipitated the reformed complex, strongly suggesting that both antibodies were recognizing an epitope present only on the intact complex. Cross-blocking studies indicated that AK 1 and SZ 1 recognized a very similar or identical epitope that was proximal to the epitope for FMC 25. Both AK 1 and SZ 1 bound to a similar number of binding sites (congruent to 25,000) on intact platelets as monoclonal antibodies directed against either GP lb or GP IX. The combined data suggests that GP lb and GP IX are fully complexed in the intact platelet membrane.

Dialysis for schizophrenia: an uncontrolled study of 11 patients.

The authors evaluated hemodialysis as a treatment for schizophrenia in an uncontrolled study of 11 patients. Eight men and two women with chronic schizophrenia who had responded poorly to conventional treatments or who had sought alternate treatments received three dialyses weekly for 3 weeks; 1 additional subject dropped out after eight treatments. MMPI, Psychiatric Status Scale, and Inpatient Multidimensional Psychiatric Scale scores were obtained before, immediately following, and at intervals after the nine treatments. Preliminary results, including 1-month follow-up, suggest that subjects with pretreatment anxiety and depression improved. No endorphins were discovered in the dialysate.

Group therapy: an alternative treatment approach for rape victims.

The current study describes a pilot project in which rape victims were treated in group therapy. Objective measures of psychological disturbance before and after 10 weeks of treatment revealed the victims experienced substantially reduced anxiety, fear, and hostility levels. This finding was independent of the amount of time between the rape and the initiation of treatment. The data also suggest that some victims do not benefit and may even worsen with treatment.
