An anatomical study has enabled the authors to propose a diagrammatic description of pre-prostatic veins, the haemostasis of which is one of the difficult stages in total cystoprostatectomy for cancer. These veins are surrounded by the pubovesical ligaments but they are separated from the membranous urethra by a cleavage plane which can be used to control venous bleeding.
The authors report five cases of sarcoma of the bladder and make a distinction between homotypical and heterotypical sarcomas and lymphosarcomas presenting specific therapeutical problems. The prognosis is severe because of very frequent local relapse. They recommend disregard of the invasion of the muscular wall, and the performance of total cystectomy following a positive histological diagnosis. In their view, this is the basis for the treatment of sarcomas.

50 patients with urethral strictures (35 bulbar, 40% iatrogenic) were treated by a one-stage skin inlay urethroplasty, using either a free full thickness patch (29 cases) or an island flap (21 cases), and followed up from 6 months to 3 years. Good results were achieved in 79% of the bulbar strictures irrespective of the type of the procedure. Penile and multioperated bulbar strictures are best treated using cutaneous island flap urethroplasty.
Thirty patients with advanced prostatic cancers (8 stage C, and 22 stage D) were treated for at least three months with D Trp6 LH-RH, a powerful LH-RH agonist. In all the cases in which the testosterone levels had been normal before treatment, the treatment led to a decline to the levels observed after castration. In seven cases, the drop was preceded by a rise in the first few days of treatment. Twenty two patients reacted favorably to the "medical castration" resulting from LH-RH agonists (11 improved and 11 remained stable). The improvements were mainly noted in patients who had not undergone previous treatment. Four deaths were registered in patients already at an advanced stage. Two of these patients presented with visceral metastases in circumstances such that a sudden deterioration due to the agonists cannot be excluded.
Six adults, including one woman, with an average age of 23 years, presented with urinary disorders associating dysuria and partial incontinence, which had been developing for an average of seven years. A diagnosis of occult spinal dysraphia (two fixed spinal cords and four spinal lipomas) was based on clinical examination (neurological and cutaneous signs), and radiology (myelography). A neurosurgical management provided satisfactory freeing of the spinal cord in three cases. In the other three, all that could be achieved was decompression, with no improvement in the urinary disorders. It is important for urologists to be aware of this cause of dysuria, in order to be able to initiate an early treatment and avoid repeated urological operations, which may ultimately lead to a cutaneous derivation of the urine.
Forty-two patients with infiltrating bladder tumor were submitted to pedal lymphography with percutaneous fine needle node aspiration biopsy on an out patient basis. Of 41 evaluable patients, 59% had a positive lymphangiogram. Adequate material was obtained in 81% of the patients. Of 12 patients with a normal lymphography, none had a positive aspiration whereas 40% of the positive lymphangiograms correlated with a positive aspiration. The high rate of positive lymphangiogram negative aspirate is attributed to inflammatory changes due to previous TUR of the tumor. Twenty-one patients were submitted to radical cystectomy: 12 negative aspirations correlated with 11 negative lymphadenectomies, and 4 positive aspirations with 4 positive lymphadenectomies. There was no mortality and only mild morbidity in this series. Lymphography with fine needle percutaneous node aspiration biopsy has a role in the pre-operative staging of bladder carcinoma. Positive pelvic wall lymph nodes upgrade bladder carcinoma from a local to a systemic disease that is no longer amenable to any form of local treatment. Nodal assessment is therefore critical to treatment planning in order to spare the N+ patients unnecessary radical surgery. Pelvic lymph nodes have been evaluated, in the past, radiologically by bipedal lymphography or C.A.T. scanning. The high false positive and negative rates of these procedures (Benson et al., 1981; Boccon-Gibod et al., 1982) preclude their routine use (Correa 1982; Walsh et al., 1980).(ABSTRACT TRUNCATED AT 250 WORDS)
While operative traumas of the excretory tract are rare, mictional and sexual sequelae often complicate rectal resections for cancer. Although relatively rare, mictional disorders, which mainly affect males, are as often neurogenic as obstructive, and are, in that event, frequently due to iatrogenic urethral stenoses. The neurogenic disorders require an extremely sensitive semiological analysis before contemplating the slightest procedure on the prostate, which may provoke definitive urinary incontinence. Retrograde ejaculation and impotence are frequent, and the latter can be attenuated by a penial prosthesis.
Thirty seven patients have been hospitalized for vesico-ureteral reflux (VUR) at the Clinique Urologique de l'Hôpital Cochin between january 1978 and december 1982. Sixteen unilateral VUR have been treated by conservative surgery; in 14 cases the rebuilding of the uretero-vesical (UV) junction has been unilateral, twice a reflux appeared at the controlateral UV junction after operation. The case for bilateral surgery in an apparently unilateral VUR is discussed. As a rule, we only perform surgery on UV junction units with a documented reflux (even once and even long ago).
Thirty five patients operated between january 1978 and december 1982 at the Clinique Urologique de l'Hôpital Cochin has suffered of recurrent urinary tract infection (UIT); 30 of them had had acute pyelonephritis. After surgery 30 patients have no longer any reflux (homo- or controlateral). Seven of them keep lower UTI, one has had 1 attack of acute pyelonephritis. Nine records had to been rejected. These results are in accordance with the medical literature: the surgical treatment of reflux prevents nearly all acute pyelonephritis and about two third of lower UTI.
The authors report twelve cases of secondary kidney tumors. In seven cases, they found antecedents of primary tumors, contrasting with the clinical, urographic and echographic data suggestive of a typical kidney tumor. Only arteriography, when performed, could visualize a hypovascularized tumor. The authors make a distinction between two situations: unilateral kidney tumors, which require total nephectomy, since, in 95% of cases, they are primary growths, and bilateral tumors, which require further exploration, since, in 20% of cases, they are metastatic proliferations.

Two familial cases of intestinal aganglionosis with lack of innervation concerning the whole digestive tract from the rectum to the esophagus are reported. The relationships between this histologic picture and that found in classical Hirschsprung's disease are discussed. The lack of obstructive symptom makes the diagnosis difficult; however, it should be suspected in the presence of intraluminal calcifications in the small bowel. The familial incidence of the condition seems to be high. A recessive autosomal transmission is likely. Neurologic signs resembling those in dysautonomia lead to suspect an extensive disorder of the development of the neural crest.

A study was conducted in 113 children with recurrent attacks of rhinopharyngitis, a frequent affection treated in ENT practice, to determine a classification and establish the etiopathogenicity of the disorder. Children were divided into two groups according to age, and clinical, radiologic, bacteriologic, biologic, and histologic data analyzed. An allergic or immunity deficiency origin could be confirmed in some cases, but etiopathogenicity was impossible to determine in many individual cases.
