[Thyroid metastases of digestive carcinomas (author's transl)].

The authors report a case of thyroid metastasis of a rectal carcinoma 9 years after excision of the primary tumour. They study the prevalence of thyroid secondary localisations of the chief primary tumours responsible, the time before appearance of the metastasis, and clinical and histological features on the basis of cases reported in the literature. They then describe the pathogenic factors and criteria in prognosis which is always grave, as a result of the dissemination of metastases.

[Pseudo-Hirschsprung's disease (author's transl)].

Eight children with pseudo-Hirschsprung's disease are reported. They can be divided into three groups: alimentary ganglioneuromatosis, schwann cell hyperplasia and those with normal appearance by stantard histology. In view of the heterogeneity, histology cannot predict the extent or the prognosis of the disease. Several biopsies at various levels of the alimentary tract and the use of advanced techniques are necessary to assess fully the extent and the type of the lesion and whether it is congenital or acquired. The majority of pseudo-Hirschsprung's disease tend to affect most of alimentary tract, which may lead to the formation of stagnant loops with all their risks. Treatment includes ileostomy, continuous hypoallergenic oral feeding, and manual expression several times a day.

[Cytology in the diagnosis of prostatic cancer (author's transl)].

We are reporting our 20 years experience in the cytological diagnostic study of prostatic cancer. We studied at the same time, urinary cytology and prostatic histology in 217 patients. There were 50% false positive results with urinary cytology. Franzen aspiration enabled us to diagnose 81% of all prostatic cancers. These two methods are possible on an out-patient basis and enable us to determine high risk patients and to diagnose prostatic cancer earlier.

[Varicocele and male hypofertility. Report of 100 cases].

100 cases of man-hypofertility associated with a varicocele have been submitted to a varicocelectomy using Ivanissevitch's procedure. Pre-operative spermogram and spermocytogram as well as testicular biopsy proved to be of no prognostic value. The only difference between pre-operative and post-operative spermocytograms concerns sperm motility. The main prognostic factor seems to be normal findings in the gynecologic examination of the female partner. Ligation of spermatic artery appears to be associated with a higher rate of failure.

[Papillary cystadenocarcinomas of the kidney].

The authors report their experience of a rare tumor of the kidney : papillary renal cell-carcinoma. This tumor was composed of papillary vegetations sometimes with infiltrating macrophages. They report the pathological and clinical features and the hypovascularized appearance of this tumor. The prognosis is better than that of clear cells adenocarcinoma.

[Upper tract urothelial tumors following bladder tumors. Report of seven cases (author's transl)].

Among 138 new cases of bladder tumours treated between 1974-1979, 7 patients developed an upper urinary tract urothelial tumor, 2 after prostatocystectomy, 2 after radical irradiation curing the bladder disease, and 4 after multiple trans-urethral procedures. Four patients had a nephro-ureterectomy out of the 7 cases 3 died of their tumor, 2 before any procedure, and 4 are alive, 1 with diffuse metastatic disease.

[What are waiting for chemotherapy in urology (author's transl)].

The place of chemotherapy inthe treatment of urological cancers remains imprecise. According to numerous publications, all urological cancers should benefit from chemotherapy. This review's goal is to point out the benefits that may be obtained with this type of treatment. Reduction of tumor volume, improvement of quality of survival, length of survival and toxic side effects are taken into account. Two different situations are considered: cases in which chemotherapy has proved to be efficient and should be prescribed, and, in most cases, situations in which chemotherapy is designed to help finding active drug and define efficient therapeutic protocols. This can only be indicated when prospective randomised studies are done in specialised centers with the collaboration of urologists and radiotherapists.

[Association of early-onset nephrotic syndrome and microcephaly. Apropos of 4 cases in 2 families].

The authors report 4 cases in 2 different families of a syndrome characterized by nephrotic syndrome of early onset (during the first 2 years of life) and microcephaly. Such an association was previously reported in 5 cases. In 4 it was familial. The study of families suggests an autosomal recessive transmission. Microcephaly was associated with psychomotor retardation, sometimes dysmorphic facies and various neurologic abnormalities. The nephrotic syndrome was characterized by its early onset and prognostic severity. However, the renal histologic lesions were heterogeneous: either minimal glomerular changes with focal and segmental hyalinosis or mesangial sclerosis, or, so-called "microcystic dysplasia". This heterogeneity does not suggest a single genetically determined disorder.
