Enteroviruses comprise a group of commonly encountered small RNA viruses with genetic similarities. We report a case of transverse myelitis associated with an enterovirus infection. By use of PCR, enterovirus-specific RNA sequences were detected in the cerebrospinal fluid and in swabs from the throat and the rectum at the time of admission. Routine cultures and serology gave no other explanation for the clinical condition. The patient was treated with intravenous immunoglobulin and steroids and improved gradually. She was fully recovered 18 months after onset of disease.
Reviewing the literature published during the last 30 years we found comparable systematic studies of outcome for 1042 term infants born alive after likely intrapartum hypoxia-ischaemia. Fifty-two percent had no sequelae, 8% had developmental delay without associated handicaps, 4% had a single handicap, 11% were multihandicapped and 14% were dead as a consequence of the intrapartum hypoxia-ischaemia. The frequency of single handicaps exceeded the frequency found among the controls and in population studies. The frequency of children with developmental delay did not differ from that found among the controls. Outcome is closely related to the severity of hypoxic-ischaemic encephalopathy in the newborn.
Clinicopathological features of four cases of low-grade stromal sarcoma (LGSS) are presented. Established diagnostic criteria have been followed. Differential diagnoses are discussed with emphasis on the difficulties in separating LGSS from the very common benign uterine leiomyomas. The finding of infiltrating cells, morphologically similar to the cells of the proliferating endometrium, has been decisive for our diagnoses. Prognosis and treatment are discussed on the basis of a compilation of data collected from 31 clinicopathological studies. Of 91 patients with initial disease confined to the uterus, 37 (41%) developed recurrence within 10 years. In contrast to most other uterine sarcomas LGSS is amenable to surgery. We recommend total abdominal hysterectomy with bilateral salpingo-oophorectomy in cases of initial disease confined to the uterus, and radical or palliative surgery in cases of extra-uterine involvement. Long-term follow-up with surgical intervention in cases of recurrence or metastases is advocated.

The normal insertion of a contraceptive diaphragm into the vagina of five young women induced a dramatic increase in the vaginal oxygen tension (131 +/- 13 mm Hg, mean +/- SD, n = 5), which slowly fell at an initial rate of 0.9 +/- 0.2 mm Hg/min. Withdrawal of the diaphragm immediately restored vaginal oxygen tension to the normal hypoxic state (11 +/- 12 mm Hg). The change in gaseous equilibrium created by the trapped air of the diaphragm could have significant actions on the microbial activity and balance of the vagina.
Vaginal oxygen and carbon dioxide tensions were measured continuously in a group of normal young women on the second day of menstruation during a 90-minute period. PO2 averaged 3 mm Hg (+/- 11 SD) and PCO2 averaged 64 mm Hg (+/- 13 SD). The value rose to that of atmospheric air when a tampon was inserted and gradually fell, giving a mean value of 112 mm Hg (+/- 18 SD) during the following 90 minutes; preinsertion values were reached in about 8 hours. Carbon dioxide rose rapidly to almost preinsertion values (mean value of 50 mm Hg +/- 12 SD) during the 90-minute period and remained steady at this level during extended periods. As in vitro studies have indicated an oxygen-dependent production of a toxin-like protein from Staphylococcus aureus, it is suggested that intravaginal tampons may be a risk factor in the development of toxic shock syndrome by supplying oxygen, thus changing the vaginal microenvironment from anaerobic to aerobic.
