ITHACA. Telematics for integrated client centred care.

ITHACA is a project supported by the healthcare telematics programme of the European commission's Fourth Framework Programme. The user organisations involved in ITHACA shared a philosophy of community based care that focuses on the client and development of the multi-disciplinary care team, involving a range of professionals delivering care to clients in their own homes or community facilities. The focus of ITHACA system is to support client case management which includes client assessment, care planning, delivery and evaluation of care outcomes. System analysis and design process identified that practices and procedures in health and social care are very similar throughout the diverse sites represented in ITHACA and they are represented in a generic model that describes the great majority of local health and social care requirements. The ITHACA demonstrator will consist of a client/patient centred community care information system, which is distributed across a number of community care centres within a given region and covers the homes of selected clients.

European prototype for integrated care.

Describes the EPIC (European prototype for integrated care) project which was set up in 1992 with the aim of ¿improving the quality of care provided to vulnerable people living in the community through the development of a prototype information system for integrated care capable of supporting the sharing of information between health and social care professional¿s. The main application was for care of the elderly with pilot sites in Northern Ireland, Finland, and Spain. The prototype supports multidisciplinary assessment, care planning, and delivery of care. The project ended in 1994 and further development of the prototype as a generic community information system is being undertaken in the ITHACA project, within the European Commission Fourth Framework Health Telematics Programme.

General practice data retrieval: the Northern Ireland project.

OBJECTIVE: To develop an epidemiological database of morbidity in the community as presented to general practitioners and to support epidemiological research in general practice. DESIGN: The project is a sentinel network of 23 general practices in Northern Ireland that report the incidence of a list of selected diseases to a central unit for analysis. RESULTS: Results are presented for depression, diabetes, and myocardial infarction. The age--sex distributions of diabetes and myocardial infarction are comparable with other published data but the incidence of depression is under estimated. CONCLUSIONS: The importance of complete accuracy of data reported within sentinel networks depends on the purposes for which information is to be used. For some diagnoses, such as diabetes and acute myocardial infarction, the accuracy of the reported incidence is high while for other diseases, such as depression, where diagnostic behaviour varies more between doctors, the figures are much less reliable.

European prototype for integrated care (EPIC).

The EPIC project has devised a means by which care workers can share client information between different professions and different locations through the use of a common domain information model for the client dossier, the client reference dossier and the EPIC message. This approach will be tested by its application to the assessment, planning and delivery of care to individual elderly clients in the community with the intention of extending and applying it to other aspects of the business of the community care service and other client groups in the future.
