Postoperative Tc-scintimetry in femoral neck fracture. A prospective study of 46 cases.

13 men and 33 women with 8 undisplaced and 38 displaced fractures of the femoral neck treated with internal fixation were included in a prospective study of Tc-scintimetry performed early (1-3 weeks), as well as after 2 and 12 months. 9 fractures redisplaced and 5 had late segmental collapse. Healing complications were associated with decreased uptake in early and 2-month scintimetry, but the specificity was only 50 percent. With normal or increased uptake, uncomplicated healing of the fracture was predictable with 90-100 percent sensitivity.

[The thromboprophylactic effect of low molecular weight heparin (Fragmin) in hip fracture surgery. A placebo controlled trial].

A prospective, randomized, double-blind trial concerning prophylaxis of thrombosis in 82 patients undergoing hip fracture surgery was conducted to compare a new low-molecular-weight heparin (Fragmin) with a placebo. Deep venous thrombosis (DVT) was detected by I125-fibrinogen uptake test followed by ascending phlebography when positive. Sixty-eight patients completed the study and a 50% reduction in the incidence of DVT was demonstrated: 9/30 (30%) patients in the treatment group and 22/38 (50DVT. This significant difference was achieved by one daily dose of 5.000 IU Fragmin subcutaneously, commencing preoperatively and continuing for six days. No differences in bleeding nor other complications in the two groups were observed. Fragmin administered once daily offers an effective and safe prophylaxis of thrombosis in hip fracture surgery.

[Accessory soleus muscle].

A case of an accessory soleus muscle is reported. This is a rare anomaly. Frequently, a bursa or some type of soft-tissue tumour such as a lipoma, haemangioma or sarcoma is suspected. CT scanning is diagnostic and is recommended before surgical excision on account of different types of insertion.

[Purulent coxitis after blockade treatment].

A case of bacterial arthritis of the hip in a 23 year old male athlete following local injections is presented. The patient was admitted with history of pain in the buttock and femur for 14 days but no fever. The diagnosis was made with bone scintigraphy and needle aspiration. Following surgical drainage and antibiotic treatment, rapid recovery was observed. At four months follow up there were no clinical or radiographic signs of involvement of the hip.

Thromboprophylaxis by low-molecular-weight heparin in elective hip surgery. A placebo controlled study.

In a double-blind, randomised study of thromboprophylaxis in patients undergoing total hip replacement, we compared a low-molecular-weight heparin with a placebo. Of the 120 patients enrolled, 112 completed the trial; 58 in the treatment group and 54 in the placebo group. Nine (16%) patients in the treatment group and 19 (35%) in the placebo group developed deep venous thrombosis, diagnosed by the 125I-fibrinogen uptake test (p < 0.02). Verification was obtained by phlebography in 86% of the patients. Prolonged surgery increased the risk of thrombosis in the placebo group but not in the treatment group (p < 0.05). There were significantly more cases of deep venous thrombosis in the placebo group during the first four postoperative days (p < 0.02). The groups did not differ with respect to peroperative and postoperative bleeding. Low-molecular-weight heparin offers safe and easily administered thromboprophylaxis in total hip replacement.

[Prevention of thrombosis in orthopedic surgery].

The high incidence of thromboembolic complications in orthopedic surgery necessitates an effective thromboprophylactic regime. Current methods of prophylaxis are discussed, and principles of thromboprophylaxis with low-molecular weight heparin are presented.

Resection arthroplasty of the forefoot in rheumatoid arthritis cases.

Resection arthroplasty of the forefoot was performed in 30 patients (48 feet) with rheumatoid arthritis. A Keller/Clayton procedure yielded good results, whereas a less radical operation (Hybbinette) created poor results. Reoperation following failed Hybbinette operations was possible.
