The obturator foramen bypass: an alternative procedure in iliofemoral artery revascularization.

The obturator foramen bypass has been described as an alternative procedure to solve complicated vascular problems in the groin. This extra-anatomic bypass has been used successfully in cases of infection, mycotic aneurysms, traumatic lesions, irradiation ulcers, and excessive scar tissue in the femoral triangle. With this technique, the vascular continuity can be restored immediately. Although data on the long-term results of this operative procedure are not yet available, out report and a review of the literature suggest that the obturator foramen bypass is a very useful technique for saving a patient's leg and in some cases his life.

The femoro-femoral cross-over graft.

The femoro-femoral cross-over graft is a good alternative to direct reconstructive vascular surgery in high-risk cases of unilateral iliac artery occlusion. The technique is simple and can readily be performed under local anaesthesia. A series of 21 patients is discussed briefly. Blood-flow measurements in seven patients confirmed the observation of other workers that a steal syndrome from the donor leg only occurred when the outflow tract in the donor leg was worse than in the ischaemic leg. Flow in the donor iliac artery was considerable increased. Four patients died in the observation period, one graft occluded three months after operation. There was no late failures.

Four years experience with dacron-velour vascular prostheses in the femoro-popliteal region.

A 4 year retrospective analysis of the results in a consecutive series of 83 dacron-velour-prosthesis and the factors that were of influence on the results are described. A total of 117 primary and secondary operations were performed on the 83 limbs. The overall patency rate was 80% when the distal anastomosis could be made above the knee, but was only 54% when the distal anastomosis lay below knee-level. Seven limbs had to be amputated below the knee. In the above knee situation the dacron-velour-prosthesis proved to be a usefull alternative for the autogenous vein graft. Below the knee, the prosthesis should only be used in cases of threatening limb-loss.
