Effects of chemotherapy on mice submitted to multiple Schistosoma mansoni infections. A controlled randomized prospective study.

In an attempt to reproduce experimentally as close a possible the conditions found in endemic areas of mansonian schistosomiasis, a controlled trial in mice submitted to weakly infections with cercariae of S. mansoni was performed. One group (I) was not treated, the other (II) was treated with a single oral doses (100 mg/kg) of oxamniquine at the 13th week. After treatment infections were maintained bi-weekly. Mortality was significantly higher in group I (p less than 0.0001). A comparative morphological and immunoenzymatic study of the liver in the two groups was also carried out, showing a tendency to smaller granulomas and to more efficient antigen restriction in the treated group.

Immunopathology of experimental schistosome (S. Mansoni) egg granulomas in mice--possible defence mechanisms mediated by local immune complexes.

A ten month experimental S. Mansoni infection was followed in mice by studying schistosome egg granulomatous inflammation, antigen and immunoglobulins deposits in the granulomata and the circulating antibodies. During the experimental infection it was demonstrated that while the granulomatous reaction matures there is a progressive restriction of the antigens to the miracidia. Furthermore, immunoglobulins, particularly IgG and C3 deposits were demonstrated in the granulomata. Our data support the concept that schistosome egg granuloma is a hypersensitivity high turnover granuloma that is particularly efficient in walling off the antigen. Whereas the granuloma is built up largely by a cell mediated immune reaction, antibody deposits are also found, probably functioning as a local humoral antibody barrier which allows a slow and progressive neutralisation of the antigens. Viewed as a whole, experimental schistosome egg granuloma might be considered as a mixed, chiefly cell but also humoral antibody mediated immune reaction.

Praziquantel in the treatment of the hepatosplenic form of Schistosomiasis mansoni.

UNLABELLED: 40 patients with the hepatosplenic form of schistosomiasis mansoni were selected for this study. Among 29 patients submitted to oral endoscopy there were 28 in whom oesophageal varices were demonstrated and 20 out of 21 patients who presented gastrointestinal haemorrhage had been operated on. All patients received a single oral dose of 40 mg/kg b.w. of 2-cyclohexylcarbonyl-1,2,3,6,7,11b-hexahydro-4H-pyrazino[2,1-a]isoquinolin-4-one (praziquantel, EMBAY 8440, Biltricide). Unwanted side effects were recorded when spontaneously reported. These were rather frequent but generally mild and disappeared within hours without additional medication. Comprehensive laboratory tests as well as ECG and EEG dit not show any significant changes. 27 patients were parasitologically followed up for 3-6 months; 26 of them were egg-negative. In the 18 patients who could be followed up for more than 6 months, the cure rate was 94.4%. CONCLUSION: praziquantel can be safely given also to patients with hepatosplenic complications.

Class specific antibodies and fluorescent staining patterns in acute and chronic forms of schistosomiasis mansoni.

Sera from 40 patients with acute (10) and chronic (30) forms of schistosomiasis mansoni were studied in order to correlate class specific circulating antibodies with fluorescent patterns developed in sections of both worms and liver granulomata. At the acute stage of the infection, IgA antibodies were present, IgM titers were high (about 10 times those found in the chronic stage), and IgG, IgM, IgA, and IgE antibodies were shown by focal fluorescent staining of worms and granulomata. At the chronic stage, IgA antibodies were absent and IgG antibodies showed mostly a diffuse staining pattern in both kinds of sections. The relevance of these observations to diagnosis in clinical cases or epidemiological surveys is discussed.

Kidney biopsy in the hepatosplenic form of infection with Schistosoma mansoni in man.

A study of early glomerular lesions was made in 8 patients infected with Schistosoma mansoni but having no clinical evidence of renal disease. Electron-microscopy of renal biopsies showed the presence of electron-dense deposits in basement membranes and of laminated bodies near the mesangial cells. Immunofluorescence showed that the deposits corresponded to IgG in 8 cases and to IgM in 2 cases. These lesions are comparable with those found in the kidneys of patients with cirrhosis of the liver.
