[Discrepancy of ultrasound biometric parameters of the head (HC--head circumference, BPD--biparietal diameter) and femur length in relation to sex of the fetus and duration of pregnancy].

OBJECTIVE: To compare female and male fetuses in terms of intrauterine ultrasound growth measurements (HC--head circumference, BPD--biparietal diameter, FL--femur lenght) depending on gestational age. DESIGN: A prospective study. SETTING: Department of Obstetrics and Gynecology, Department of Medical Genetics and Fetal Medicine, Department of Preventive Medicine, University Hospital, Olomouc. METHODS: All ultrasound biometric measurements were performed according to the methodology published with the reference charts. Risk pregnancies, multiple pregnancies and breech presentations were excluded. RESULTS: Fetal HC, BPD and FL were measured in 427 ultrasound examinations at 16-38 weeks. Male fetuses had significantly larger HC and BPD measurements compared to female fetuses and these differences increased with advancing gestation. In the 16-21 week scans estimated difference was (HC + 3.9 days, 3.0% and BPD + 4.1, 3.2%), during the 21-30 week scans (HC + 6.8 days, 4.3% and BPD + 6.9, 4.4%) and in the 31-38 week scans (HC + 12.3 days, 5.6% and BPD + 12.9, 5.9%) for males. Male fetuses had significantly larger HC compared to FL measurements. In the 16-21 week scans, estimated difference was + 2.1 days (95%CI 1.7-2.6, P < 0.001), during the 21-30 week scans + 3.4 days (95%CI 2.5-4.2, P < 0.001) and in the 31-38 week scans + 9.7 days (95%CI 7.3-12.1, P < 0.001). CONCLUSION: This study suggests that male fetuses have significantly larger head circumference (HC) and biparietal diameter (BPD) measurements compared to female fetuses. These prenatal sex-related differences are established by as early as 16 weeks of gestation and tend to increase with advancing gestational age. In the case of discrepancy finding between head (HC, BPD) and femur lenght (FL) measurements the fetal gender should be taken into account.

[Adiponectin and insulin sensitivity].

BACKGROUND: Adiponectin is a fat tissue protein that plays a role in maintaining the homeostasis of glucose and lipids, along with counteracting a number of risk factors associated with atherosclerosis and obesity. In addition, adiponectin has an effect on insulin sensitivity. The aim of this work was to assess concentrations of adiponectin in predefined groups of individuals and to analyse the associations between adiponectin and other metabolic parameters. METHODS AND RESULTS: The studied population comprised four groups of individuals, A-D. A--healthy controls, B--patients with impaired lipid metabolism, C--the obese, and D--patients with metabolic syndrome. When comparing the levels of adiponectin in groups of patients with impaired lipid metabolism (B), the obese (C), and patients with metabolic syndrome (D) with healthy controls (A), no statistically significant difference was observed between groups B and C and healthy individuals. In contrast, statistically significant difference was found when concentrations of adiponectin in patients with metabolic syndrome (D) were compared to those in healthy controls. Individuals with metabolic syndrome had the lowest levels of adiponectin - 5.3 mg/1 (men) and 5.6 mg/1 (women). The correlation coefficient for the association between adiponectin and HDL was R=0.57, for adiponectin and triglycerides R=-0.46, for adiponectin and BMI R=-0.37, for adiponectin and glycemia R=-0.34, for adiponectin and insulinemia R=-0.39, and for adiponectin and the QUICKI index R=0.44. CONCLUSIONS: One possible method of the complex evaluation of the metabolic syndrome development and its metabolic consequences is the assessment of adiponectin levels. Low levels of adiponectin indicate the development of insulin resistance.

[Assessment of the incidence of metabolic syndrome].

BACKGROUND: Metabolic syndrome represents a disease with high prevalence and influence on the cardiovascular morbidity and mortality. The aim of this study is to estimate frequency of metabolic syndrome in the district Sumperk, in years 1979-1981 based on the analysis of large Preventive check-ups database. METHODS AND RESULTS: Database of Preventive oncologic check-ups from Sumperk district comprising 40 099 subjects with follow up in years 1979-1981 has been used to assess the metabolic syndrome prevalence. Incidence of the risk factors of the metabolic syndrome have been calculated and compared with current situation in the Czech Republic. The estimated incidence of metabolic syndrome ranged from 2.2% to 8.4%, according to the selected criteria. As risk factors diabetes type II or glycaemia higher than 6.1 mmol/l, systolic or diastolic hypertension and BMI higher than 30.0 kg/m2 were considered. It has been found that metabolic syndrome is increasing with increasing age in both male and female population. CONCLUSIONS: It is possible to assume, based on the results of this study that the incidence of the metabolic syndrome is increasing in the last decades. It is necessary to focus our attention on the simple and practical methods for early detection of metabolic syndrome. Important is also the prevention because metabolic syndrome plays important role as a predictor of serious cardiovascular and metabolic diseases.

[Risk factors for tumors, cardiovascular and metabolic diseases in the population of the Sumperk District].

BACKGROUND: Cardiovascular diseases and cancers are responsible for almost 80% of the total mortality in the Czech Republic. For their prevention, identification of risk factors becomes indispensable. Presented article analyses frequency of risk factors in the population of Sumperk district on the bases of the first and second preventive oncological check-ups. METHODS AND RESULTS: Diagnoses have been unified according to the International Classification of Diseases. For data analysis SPSS statistical software has been used. Frequency of obesity, hypertension, hyperglycemia, hypercholesterolemia, hyperuricemia and smoking has been assessed in subjects during their first and second preventive check-ups. In the course of the first check-up 49,426 subjects have been examined, out of them 22,857 males (46.2%) and 26,569 females (53.8%). 40,386 subjects participated in the second check-up (18,378 males and 22,008 females). The assessment of the frequency of the individual risk factors has been done on the bases of the examination results. CONCLUSIONS: Total incidence of obesity was lower in males comparing to the data from the population of 6 districts of the Czech republic (MONICA). Due to the treatment of subjects, there was a mild decrease of prevalence of the systolic hypertension in the second check-up in comparison with the first one. In comparison with MONICA project, the incidence of hypertension was more than twice higher. No difference in frequency of hyperglycemia in males and females was found. In both genders, high prevalence of hypercholesterolemia (about 70%) was found. Between two check-ups, decrease of hypercholesterolemia prevalence was observed. It corresponds with the existing trend in the Czech population. Decrease of smoking prevalence has been observed in the second check-up mainly in males and less in females. In comparison with MONICA project, there was a higher smoking prevalence in males and a lower one in females.

The 2001 serologic survey in the Czech Republic--tetanus.

In 2001, a serological survey of antibody against tetanus was carded out in the Czech Republic among 499 individuals aged between 40 and 64 years. It revealed that 2.8% of them had antibody titres below protective levels and 43.3% and 53.9% respectively had medium and high antibody titres, i.e. protective levels of antibodies. These results show a good immunity level against tetanus among the Czech population aged between 40 and 64 years, likely to be ascribed to the good immunization coverage achieved. The individuals with antibody titres below protective levels need revaccination.
