[Laparocele in the aged].

Especially in the old patients (over 70 years) the incisional hernias represents an invalidating pathology whose treatment, for the high incidence of associated diseases of respiratory and cardiocirculatory apparatus in the aged, offers difficulties connected both to surgical methods and to the perioperative evaluation and preparation of patients. The infections of the surgical incision are very important in the pathogenesis of these diseases. In order to reduce the incidence of incisional hernias the systematic use of perioperative antibiotic therapy is desirable in every operation of abdominal surgery. In order to offer the best guarantees of recovery and to improve the postoperative course of patients, the perioperative antibiotic prophylaxis, together with the use of synthetic prosthesis material, is also essential at the moment of surgical correction of incisional hernia. Although the incisional hernia may sometimes stay silent and asymptomatic for years, it inevitably ends sup by representing a reason for acute and subacute pathologic events; an early surgical treatment is therefore desirable once the incisional hernia has been diagnosed. By this way it is possible to avoid the treatment in the old patients, as the old age is a less favourable period for people who have to be subjected to a surgical operation.

Calcitonin deficit syndrome in thyroidectomized patients.

The results of a study carried out on 10 patients who underwent total or subtotal thyroidectomy to determine the effects of a prolonged deficit in calcitonin on bone turnover, are reported. The following parameters were evaluated: serum calcium and phosphate, serum alkaline phosphatase, urinary hydroxyproline, serum iPTH, serum iCT and bone mineral content (BMC) of the distal radius. In ten patients controlled 24 months after surgery, a lowering in circulating iCT levels was noted together with a significant decrease in BMC. These observations while supporting the theory that a CT deficit can determine osteoporosis, suggest that further consideration should be given to substitutive therapies for totally and subtotally thyroidectomized patients.

[Use of cephalosporins in surgical patients].

A clinical and bacteriological evaluation of antibiotic therapy in surgical sepsis is performed on two groups of patients examined in 1979 and 1980 with particular regard to use of cephalosporins. In the first group have been administered cephalosporins not beta-lactamase resistant, tetracyclines, chloramphenicol and aminoglycosides on the basis of antimicrobial susceptibility testing; in the cases of urinary tract infections nitrofurans and nalidixic acid were also employed. In the second group the chemotherapy has been performed with beta-lactamase resistant cephalosporins only or associated with aminoglycosides. The results of this study suggest that the use of beta-lactamase resistant cephalosporins may be clinically advantageous, but the incidence of bacterial selection and resistances may avoided with a constant vigilance and bacteriological screening.

[Clinical applications of the carcinoembryonic antigen (CEA) determination in colorectal cancer (observations of 119 cases)].

The AA., on the base of their personal experience and the data of literature, value clinical uses of dosage of carcino-embryonic antigen (CEA), made on the blood of patients who suffered of colon-rectal cancer. Their personal experience is based on 119 patients, 70 who had radical exeresis of neoplasm and 49 the palliative treatment. In the patients who had radical treatment the evolution of disease was overseen with this method for a period from a minimum of 1 year to 3 years and in 10 cases they could put in evidence the restarting of neoplasm too early. In regard to the patients who had a palliative treatment, with the observations of CEA works, they obtained a quantitative valuation of antiblastic chemiotherapy effects and electrocoagulative treatment. The AA. end that, while CEA dosage has not negligible limits for a diagnosis, this method is very useful for the precocious diagnosis of neoplasm restarting. On the contrary the information, that we can obtain about the effects of palliative therapy, is limited.

[Urodynamic evaluations after abdomino-perineal resection of the rectum (author's transl)].

An uro-dynamic evaluation has been performed on 25 patients, who had been submitted to abdomino-perineal amputation of the rectum and had thereafter shown functional disturbances of micturition. On the latter, causes and mechanisms are discussed, particularly neurological as well as mechanical and inflammatory ones, with regard to their relationship with sex and other possible aspects in the field of pathology. In 16 cases urinary retention developed, in 4 cases urinary incontinence, in 4 cases urgency but complete voiding. The authors conclude by stressing the role of pre-operative uro-dynamic assessment in every patient who is going to be submitted to rectal exeresis, in order to detect sub-clinical functional impairment of micturition, which could be suitably treated, during the main operation itself, by either desobstructive or reconstructive measures.
